Accident type, without injuries

Discovered that the roof had come in around the cribs. The fall occurred between station 22+37 and extend to 22+20. A location just inby x-cut 12.

During the preshift examination of the isolation stopping in the tailgate, a roof fall was discovered just inby XC-25 in the Number 1 Entry. The exact time of the fall is unknown. The area had previously been dangered off due to hazardous roof conditions. Air velocities on the LW face were negatively impacted.

A roof fall occurred behind the substantial structure at XC-32 in the tailgate off LW-303. The fall occurred between No. 2 and No. 3 entries. The fall occurred in a worked out area, however the rubble from the fall knocked out a ventilation control, impacting the active workings. Because of this the fall was immediately reported.

Taking bolts off of the 4 foot piece of the steel set.

Struck by flying object

New 4"" hose on compressor blew off fitting when pressure was being brought up and hose whipped employee. Left arm contusion, left foot contusion, broken nose and bruised right knee and elbow.

Over-exertion in lifting objects

EE was standing up, but bent over because of low roof. He was packing crib in a low area. He bent over with crib block on his shoulder, his back got tight and then he felt a pinch in his mid-back.

Employee sustained a sprained ankle when walking while performing rock dusting. Employee was evaluated and released to restricted duty.

Struck by falling object

Employee went to rehang miner cable on to existing trolley on roof. While in the process of securing it to the tensar with a D ring, the cable and trolley came off the roof, striking employee on top of her hardhat. Resulting into neck strain and pain.

Over-exertion in pulling or pushing objects

employee sustained a strained muscle in the left buttocks after experiencing sharp hip pain while pulling cable. 8-31-10 employee released to full duty. 9-1-10 resleased to restricted duty. 9-2-10 released to full duty by Reliance Medical Group.

Over-exertion in lifting objects

Employee strained his arm while moving structure, resulted in some pain. Reliance Medical Group diagnosed him with right elbow strain and released employee to restricted duty.

Employee felt pain when installing idler in miner and didn't take is serious. Sought treatment 3-18-2010 and released to restricted work with no underground work.

Struck by falling object

While employee was removing a cleavis from the structure skid in LW303xc5, to use for pulling a loose cluster that was against the rib, it fell and hit employee's arm, knocking him to the ground.

Over-exertion in lifting objects

Employee was operating a shuttle car and stepped out to hang the cable, while doing so, employee felt a tingle in her neck. She felt it was nothing to worry about until she returned to her shuttle car and her neck was too stiff.
