Accident type, without injuries

On October 24, 2012, San Juan Mine experienced a roof fall in the Longwall 301 panel Headgate entry at approximately 14.5 cross cut in the belt entry. The fall is approximately 7 ft high, 18 ft wide and approximately 45 ft long. No injuries or equipment damage occurred.

Accident type, without injuries

A roof fall was discovered in the East Mains, Entry 5, at crosscut 53. The area had previously been supported with a set of steel arches installed in an overcast area as a King and Queen style set. The roof fell in above these arches and blocked the roadway.

Caught in, under or between a moving and a stationary object

Employee closed his finger in his truck door in the parking lot, he had not started his shift. he reported the incident to his supervisor & drove himself to the ER @ San Juan Regional. He declined to have someone drive him. Doctor noted:12-25-12 to rtn to wk in 5 days & no heavy lifting until condition improves. 12-31-2012 released to restricted duty by Reliance Medical Group.

Fall onto or against objects

While getting into the cab TRD0775 CAT D11T dozer the operator slipped on the left track causing abrasions on left abdomen and tailbone. Employee was released to restricted duty, diagnosis left side back pain and strained.

Over-exertion in lifting objects

while lifting 50# rock dust bags, employee strained mid-back. release to no lifting restriction.

Over-exertion (Not Elsewhere Classified)

while using high volume fire hose to wash ASH Truck window, employee stepped forward on a mud and strained right knee.

Struck by falling object

Initial injury 4-10-12. EE sought treatment 4-14-12 & released full duty. 10-16-12 he brought in a doc note to excuse him from work until further notice. he stated he was having continuous pain.

Fall from machine

Employee was conducting a PM on 1318 End Dump in the shop. he was coming off the ladder from the deck above the engine compartment to the top of the transmission area. The 2nd step on the ladder broke loose on one side, causing him to fall off the ladder. diagnosis lumbar strained and do not return to work.

Employee swung a 2# sledge hammer to remove the bolt and the hammer ricocheted off the cylinder striking employee on the right knee cap, resulted to contusion and released to light duty.

Over-exertion in lifting objects

Employee was carrying a 20' chain link fence panel up from the bottom of a settling pond when he heard a pop and felt pain from his left calf. Reliance released him to restricted duty - left calf strained.

Fall to the walkway or working surface

Employee stepped on the edge of the shield toe and caused him to slip and fall, landing on his right hip. Diagnosis: contusion right lower back and released to full duty. On 12-27-12 doctor noted: Do not return to work until further notice.

Employee was conducting service on CME0408, while turning the cutter head to change bits, his foot slipped and he felt a sharp pain in his right leg calf muscle. diagnosis calf tear from Reliance Medical Group.

Employee's back went out when getting dress for work. Reliance Medical Group diagnosis low back pain and released him to light duty.

Over-exertion in lifting objects

Lifting rock prop onto unit with 2 people on each side repetitively. Reliance diagnosis umbilical hernia.

While performing PM service per work order; Transmission Oil Sampling. Service Mechanic was attempting to attach sampling bottle to port, cock R. Elbow and same Elbow was struck by a direct drive cooling fan. Resulting injury was a 1 inch laceration. Work was stopped and worker sent for Medical Attention.

Over-exertion in lifting objects

EE was removing a crate from a pallet & felt a slight strain in the shoulder. EE returned home and woke up the next morning with a great pain in shoulder.

Struck against stationary object

Employee was coming down from ladder his left boot hung up on lower step which caused a twisting action to left knee and pain. His leg buckled & he stepped on air hose rolling right ankle which popped. was release full duty 8-13-12 from Reliance Medical. 8-15-12 he reported that his knee was very painful, this time he was released to restricted duty.

While moving a jack under a D11 tractor the left ring finger was caught between the jack and the ripper table

Employee was hanging tubes in 400 Sec in LOX. While pulling strap, the strap broke and the hook hit employee's tooth, causing it to fracture.

Employee was walking past CMT & ducked under the hose handler. He stood up too soon and struck the edge of it with his head. He was knocked out for about 10 second. he was taken to emergency room and released full duty.

Employee smashed finger tip when chain hoist slid on truss leg. This caused slack in the chain pinching finger between chain and driscol pipe. employee received 3 sutures and released to full duty.

EE became fatigued using his right hand to cut strips in lagging on pulley. Switched to his left hand and shortly after slipped and poked himself in the left thigh just above the knee. He sustained a 3/8"" cut that was managed with first aid. After his shift he went to the dr for a Tetanus shot and was convinced that he need stitches to prevent infection. Reportable injury.

Inhalation of radiations, caustics, toxic and noxious substances

Operator notified that employee has a sufficient degree of pneumoconiosis to be eligible for rights under Title 30, Code CFR, Part 90.
