The roof adjacent to the last shield on the face fell in and blocked travel into the tailgate. The face was located at survey station 12+16 at the time of the fall. The fall went approximately 5 ft outby the face in the TG entry.

During an onshift examination, the section foreman discovered an intersection with deteriorating roof conditions. The intersection was dangered off and the condition reported. While the intersection was dangered off, and before any additional support could be delivered to the area, the roof fell in.
A roof fall occurred that blocked the tailgate at x-cut 19 in LW-203. The fall started in the x-cut between No. 1 and No. 2 entries and ran out in to the tailgate entry.
Material above the tailgate shields sloughed in around the tailgate drive and face conveyor. The Tailgate entry itself remained open in the active workings. The blockage was removed as the shearer cut out the tailgate on that same pass.
A roof fall occurred adjacent to the last shield on the face causing the entrance to the tailgate to become blocked, preventing egress off the tailgate end of the longwall.
At approximately 4:45 pm the roof adjacent to the last shield on the face collapsed. The fall resulted in a tailgate blockage that lasted for 2 passes.
Lowered air filter boxes from above head level. Felt some discomfort 30 minutes later. Next day, right shoulder sore, two days later, more sore. Monday (2/27) felt like small needles in fingers. Reported numbness in fingers to foreman on Tuesday 3/01, (5 days later). Send to clinic on 3/02. Diagnosed with minor AC separation in right shoulder (shoulder strain) .
The injured employee was working on the rear electrical junction box of a Kress Haul Truck. The right side dump door of the truck was resting on a 10 ton floor jack approximately 10-12 inches from the floor. The employee unknowingly had his foot under the edge of the door when the jack suddenly slipped from under the door allowing the door to fall on the employees foot.
When EE exit the truck he was pre-shifting, he struck his left elbow on the edge of door. He immediately felt pain and tingling in his arm from the elbow down to his hand. On 1/18/06, EE stated he was still feeling the numbness & tingling in ring/pinky finger. Went to Mesa Occ & Sport Medicing Clinic and was released to full duty. On 5/18/06 received surgery and off since.
Employee was off work on Monday due to neck pain and suspects a cumulative trauma situation. He stated he was leaning against the rib and felt a sharp burning pain go right side of back near spinal column. Felt no other discomfort during the remainder of the shift. The next morning he woke up and was unable to move his head. The condition is believed to be work-related.
Employee's right foot slipped when he stepped on the relay bar while reaching for the panline, causing him to twist his ankle. Sought medical treatment and was released to restricted duty.
Employee felt a sharp pain to his lower back, while carrying roc-prop for approximately 40 feet with another employee.
Employee strained her lower back while handling a belt scraper & manuevering it around the feeder. EE was released with restriction from the clinic.
Employee was carrying a Roc-prop with another employee and stepped on uneven ground causing him to strain his left calf muscle.
Employee struck a roof bolt with his hardhat while walking through crosscuts, knocking him to the floor and sustaining minor injuries.
The winch on the coal car broke and struck EE's right hand causing laceration. Employee received stitches at Mesa Occupational & Sports Medicine Clinic.
Employee was struck in the thigh with a monorail component. Ee was examined at Mesa Occup & sports Medicine Clinic was diagnosed with a deep bruise to his thigh.
Employee stepped on a wet metal panel, causing him to slip and resulting in a sprained ankle.
Employee reported pain in his wrist and up into his forearm. Employee has been swinging a sledge hammer extensively for several shift while working on shield repair. He was examined at Mesa Occupational & Sports Clinic and as been placed on restricted duty.
Employee was injured while hanging ventilation tubing. While he raised the vent tube, he experienced pain in his back.
Employee was installing a light fixture in the scale house and the light fixture slipped cutting the right thumb.
Employee slipped on a loose coal, causing him to fall forward and landing on the floor with his left hand first. Resulted into dislocation of left pinky finger.
While employee was repairing a 5th wheel for a Getman tractor, the plate slipped from his hands. crushing his left index finger, resulting in a laceration. He received suturing.
Employee was climbing on board her unit when a gust of wind kicked up and blew dust into her eye. As it did not bother her immediately, she did not have the eye irrigated until an hour later. She was taken to a clinic for extra precaution where the doctor found a scratched cornea. She was given prescription medication for the irritation.
Employee pinched his hand between drill steel extension and the drill steel. Employee received five sutures to his right hand and was released full duty with no restriction.
Emp was holding a straight edge to cut a template out of cardboard, while using a utility knife to cut the template, employee's hand was placed along the edge and while cutting along the straight edge the little finger of his left hand was cut. Emp received 6 stitches to close wound.
Employee was breaking loose a high pressure hose with a pipe wrench and hammer. When the fitting of the hose broke loose, the fitting struck employee's hand resulting in a laceration. Employee received four sutures at Mesa Clinic.
Repairing haul truck. Employee opened his toolbox and a welding rod fell out. As he attempted to catch the rod, his hand contacted the lid of the box, cutting his hand.
Employee smashed his right ring finger while moving rock prop plate, causing laceration. Received stitches from the clinic.
