Getting out of the loader one foot on the ground they turned to step away their other foot caught on the step causing them to fall to the ground.
Low Light Conditions. Step into old probe 4-inch borehole, twisting right knee.
Employee was digging out the sump pond near pump #1 with an excavator. While dumping the last pile, the rocks bounced off of each other and struck the front of the excavator, shattering the right side glass. This glass then struck the member on the hand and wrist causing a small (approximately 3") laceration to left wrist.
lifting a 5-gallon pail full of stone, the employee felt a pop in EE's right knee
A wrench was struck with a hammer to loosen a nut. When employee's thumb was struck, causing a fracture.
While attempting to drill through a steel plate, drill slipped from plate, landing between member`s index finger and thumb. Drill entered approximately 1/4" into the skin. Drill bit was dull, requiring excessive downward force to be applied in order to complete drilling operation.
Mild hearing threshold shift.
While installing emergency stop gate the gusset caught on the flange and the assembly came in contact with employee's left wrist. Originally thought to be a contusion but after MRI it was determined to be a fracture
They were tightening down the top deck panels. When EE's left index fingertip was caught and crushed in between the washer and top deck steel frame.
Placing the guard back into place when EE let go of the guard which fell into place landing on EE's left big toe.
While using a utility knife to cut the conveyor belt, EE accidentally cut EE's thigh.
While descending from the Mini Excavator, they rolled their ankle.
The member was sweeping stairs inside of the scale house when a truck arrived requiring attention. Member turned towards the console and missed the bottom step, losing balance, and landed on EE's hip and side.
When entering the driver's side of EE's car EE noticed dirt/sand on EE's seat EE stopped EE's entry action and tried to give the seat a quick brush with EE's hand, when beginning that motion EE hit the upper car door opening with the top of EE's head, jarring EE and closing EE's jaw quickly to "rattle" EE's teeth.
Injury to the right foot while lowering the skid steer bucket. Their right leg and foot were extended beyond the cab when they lowered the bucket catching their toes and crushing their foot.
Employee struck hand while attempting to loosen nuts to adjust belt. Employee rendered first aid on day of incident however did not request medical attention until 5/10/22. X-Rays revealed broken left middle finger. 7000-01 filed on same day of X-Ray results on 5/10/22.
EE was removing a pipe from a fitting with a 4ft. pipe wrench. When EE picked the wrench up to reset the pipe wrench to pull on it again when a finger got caught between the top of the wrench and the hydraulic tank frame which resulted in a laceration to right pinky, requiring stitches.
Injured employee was assisting an additional employee lift a 4x8x1/8" steel plate weighing approximately 160 lbs when employee alleged experiencing pain in shoulder. Employee was transported to urgent care facility by Plant Superintendent for evaluation. Injured employee issued work restriction by urgent care to remain out of work until evaluated by orthopedic.
The employee was hammering oversize rock at the entrance of stope at 12N/T13-L1 when an oversized rock came down and rolled onto the ROPES pinning EE in the cab.
The employee fell backwards off the tailgate of a pickup, striking EE's head on the ground.
The employee was disconnecting a chain (come-along), that was connected to the feeder box that was being taken down. EE slipped off the frame EE was sitting on (approximately 3-4'). When EE landed, EE rolled their ankle over. EE ended up breaking the ankle in two places.
Squatted down to pick up a tool, felt pain in left knee
The employee alleges, while unscrewing the cap of the fuel tank on the haul truck 05014, the tank was full of pressure and when opening fumes and possible dust blew up and got into EE's eyes. The pressure release function in the cap was working properly.
The Middle left finger was pinched between half-inch piece of steel and a quarter-inch piece of steel. While putting a new guard together for the HP500 crusher. The injury resulted in needing stitches.
While grinding on a skid steer bucket that was detached from the skid steer it slipped and struck EE's leg causing a long scrape. EE's first day missed from work was 2/10/2021.
Employee was assisting loading metal road plate onto flat bed trailer. Plate subsequently fell onto employees leg resulting in bone fracture to lower leg.
The employee was laying on stomach in the screen deck, pulled the center rail toward themselves and felt a pop in left shoulder. Never reported it until 12/11/2020.
While cleaning with a shovel employee strained back.
EE had removed a worn 4' X 6' - 5/8" screen and was attempting to lift the screen, at this time EE felt a sharp pain and a popping sensation in EE's left thigh.
Miner was lifting one end of a 8" x 20' long piece of channel while the other end was on the ground. EE was handing the end to another miner in the man lift as to upright it and install as a conveyor leg. After lift EE felt a strain in EE's lower abdomen.
Employee was grinding a steel plant when metal fragment got in right eye.
Employee claims they fractured a rib and has fluid on a lung, was working on the screen deck.
While operator was walking portable conveyor into place, EE inadvertently rolled the track over left foot. Fracturing EE's left foot (toe).
While walking to speak to the manager, slipped on the ice and fell on left knee.
While working to free a stuck battery box lid, the lid fell on EE's left thumb causing a fracture and laceration.
The miner came in contact with return idler on screen and was pulled in by the conveyor belt which burned & bruised EE's arm, straining the muscles.
While trying to get into the truck, EE slipped and fell to EE's back on the snow and ice.
While cutting steel for a staircase the torch snuffed out and when EE went to restart the torch gas entered EE's glove and ignited. Another EE heard a pop and then saw EE restart the torch and then pulling EE's glove off like something was wrong went to see what happened and saw that EE burned EE's hand.
While lifting a screen strained employee's back
Tripped getting out of skid steer, and leg got wedged between bucket and skid steer frame.
Working on Conveyor, sliding rollers out from under the belt and strained lower back
Pulling 30ft fuel hose from truck when EE felt a sharp pain in the left side of neck.
Stepped on a 1-1/2 stone and rolled ankle, causing EE to fall to the ground. Sprained left ankle, knee and low back.
motor was bad on the crusher, EE locked out the breaker, EE went to shop to get a picking eye, at this point IE was gathering tools off the truck to remove the motor/guard. IE proceeded to remove the guard, IE climbed on top of the hydraulic tank/reservoir to access guard. While unbolting guard it sprung up, knocked IE off balance, causing IE to fall 10'
Grinder slipped and cut left leg just above the knee.
Employee stated they were cutting a slit in a pipe, the knife broke causing employee to lacerate left hand. Requiring 10 stitiches
EE and another employee were carrying a PVC pipe. When the pipe slipped it cut EE's left index finger requiring 7 stitches. EE missed no time from work.
Changing Pitman Bearing with a hammer when a piece of the race broke off and entered left leg, had to go to the hospital to have it removed and stitches.
