Struck by flying object

The employee alleges, while unscrewing the cap of the fuel tank on the haul truck 05014, the tank was full of pressure and when opening fumes and possible dust blew up and got into EE's eyes. The pressure release function in the cap was working properly.

Worker was helping another worker put a pump in submerging. when a large chunk of ice became dislodged from the quarry wall and struck the worker, in neck, left arm, bicep and forearm causing severe pain, bruising and laceration.

EE has been out since 7/21/06 as a result of another injury. While out, he became aware of alleged hearing loss & filed a claim with WC. Operator notified thru ins provider on 5/8/07. EE has been a loader/mobile equip operator for more than 20 years.
