The employee fell backwards off the tailgate of a pickup, striking EE's head on the ground.
Squatted down to pick up a tool, felt pain in left knee
The employee alleges, while unscrewing the cap of the fuel tank on the haul truck 05014, the tank was full of pressure and when opening fumes and possible dust blew up and got into EE's eyes. The pressure release function in the cap was working properly.
