A wrench was struck with a hammer to loosen a nut. When employee's thumb was struck, causing a fracture.

The Middle left finger was pinched between half-inch piece of steel and a quarter-inch piece of steel. While putting a new guard together for the HP500 crusher. The injury resulted in needing stitches.

Over-exertion in lifting objects

Miner was lifting one end of a 8"" x 20' long piece of channel while the other end was on the ground. EE was handing the end to another miner in the man lift as to upright it and install as a conveyor leg. After lift EE felt a strain in EE's lower abdomen.

While walking to speak to the manager, slipped on the ice and fell on left knee.

Struck by falling object

EE and another employee were carrying a PVC pipe. When the pipe slipped it cut EE's left index finger requiring 7 stitches. EE missed no time from work.

Miner was wearing a welding helmet, walked under a steel structure and hit his head. Resulting in staples to the head. No lost time.

Struck against a moving object

the miner operating a Genie S-85 manlift. While he was moving it from one location to another the front wheel fell into a washout in the roadway. Which caused him to fall in the basket injuring his left shin.

While cutting the head of a bolt with a grinder, when the bolt caught and was sent flying into his face/tooth

While operating loader, he came across some drill bits that he moved by hand, while moving them he felt a pain in his left hip.

Employee was welding, building a chute for the crusher when the metal started to get away from him, in his attempt to save it he pulled a muscle in his lower abdomen.

Fall to the walkway or working surface

Employee was dragging a length of 2"" sch40 steel pipe into the DOT shop to be welded. While employee was dragging the pipe into the shop he lost his footing and fell backwards. While falling backwards he put his right hand out to catch himself and sprained his wrist.

Employee states he was holding onto a piece of steel while another employee was welding it into place and although he turned his head, he reports ""flash burns"" to both eyes.
