Getting out of the loader one foot on the ground they turned to step away their other foot caught on the step causing them to fall to the ground.

While attempting to drill through a steel plate, drill slipped from plate, landing between member`s index finger and thumb. Drill entered approximately 1/4"" into the skin. Drill bit was dull, requiring excessive downward force to be applied in order to complete drilling operation.

They were tightening down the top deck panels. When EE's left index fingertip was caught and crushed in between the washer and top deck steel frame.

Employee struck hand while attempting to loosen nuts to adjust belt. Employee rendered first aid on day of incident however did not request medical attention until 5/10/22. X-Rays revealed broken left middle finger. 7000-01 filed on same day of X-Ray results on 5/10/22.

EE had removed a worn 4' X 6' - 5/8"" screen and was attempting to lift the screen, at this time EE felt a sharp pain and a popping sensation in EE's left thigh.

Worker was flashed by welder while performing plant maintenance and received a welder burn to the eyes and face.

Struck against a moving object

While operating a Volvo Crawler excavator, worker was jostled and bounced around which resulted in neck and shoulder pain. The doctor advised EE to miss work for the rest of the week.

WHILE EXITING THE MECHANIC'S TRUCK, WORKER STEPPED IN A HOLE IN THE GROUND, TWISTED AND ROLLED THEIR FOOT RESULTING IN A FOOT SPRAIN.

Fall from ladders

AFTER FUELING THE PLANT, EMPLOYEE SLIPPED AND FELL ON THE FLATBED OF A TRUCK WHILE CLIMBING DOWN A LADDER. EE INJURED SHOULDER AND WILL RECEIVE FURTHER TREATMENT FOR A POSSIBLE TORN ROTATOR CUFF.

On 7/27/17, it was reported to John S. Lane & Son, Inc. by a company engaged to evaluate hearing test results, that an employee had incurred a reportable hearing loss. Employee was notified and retested on 8/30/2017. (A recent noise survey reports very low noise level for this employee's job.) Placement on the MSHA Log was overlooked and not reported at that time.

WHILE OPERATING A POWER HAND DRILL, EMPLOYEE WAS STRUCK WITH FORCE IN THE LEFT SIDE OF EMPLOYEE'S CHEST WITH THE DRILL HANDLE WHEN THE DRILL BIT GOT STUCK IN THE HOLE AND THE DRILL TURNED TOWARD EMPLOYEE. EMPLOYEE RECEIVED A CONTUSION TO THE CHEST WALL.

WHILE USING AN AIR ARC CUTTING TORCH DURING A WELDING OPERATION, WORKER SUSTAINED A BURN IN EAR CANAL WHEN AN AIRBORNE FRAGMENT OR SPARK FLEW INTO EAR. EE'S HEARING BECAME MUFFLED AND WAS SEEN BY AN EAR SPECIALIST WHO DETERMINED THAT AFTER TIME AND PRESCRIPTION MEDICATION, HEARING SHOULD RETURN TO NORMAL.

WHILE KNEELING AND TWISTING, EMPLOYEE TWISTED KNEE WHILE CHANGING SCREENS IN A SMALL AREA IN PLANT. EE'S KNEE BECAME SWOLLEN AND MORE PAINFUL DURING THE EVENING AND EE WAS SEEN AT THE EMERGENCY ROOM. EE IS NOW BEING EVALUATED BY AN ORTHOPEDIC SURGEON AND IS UNABLE TO RETURN TO WORK FOR AN UNSPECIFIED LENGTH OF TIME.

Struck by falling object

While performing mill maintenance, worker attempted to pick up a screen when it fell on his foot. The contusion to his right foot became swollen and painful and was instructed by the doctor to miss one scheduled work day.

WHILE CLEANING OUT THE CRUSHER, WORKER WAS STRUCK IN THE BACK OF HIS HEAD WITH FALLING ROCK WHEN HIS HARD HAT WAS KNOCKED OFF. HE RECEIVED A HEAD LACERATION AND WAS TREATED AT A HOSPITAL EMERGENCY ROOM WHERE HE RECEIVED SUTURES AND HAD TO MISS ONE SCHEDULED WORK DAY.

WORKER WAS ASSEMBLING CRIBBING TO SUPPORT A CRUSHER HEAD WHICH NEEDED WELDING. WHILE LIFTING AND PULLING, WORKER FELT PAIN IN LOWER BACK. WORKER AWOKE DURING THE NIGHT IN EXTREME PAIN AND HAS BEEN UNABLE TO WORK. CURRENT DIAGNOSIS IS BACK STRAIN.

Worker received a small cut on his hand when he accidently hit the valve cover of the excavator that he was servicing. Worker's hand swelled overnight, and he developed a blood infection as a result of the injury.

WORKER WAS HAMMERING A WELD WHEN A PIECE OF SLAG BECAME AIRBORNE, FLEW UNDER HIS SAFETY GLASSES AND LODGED IN HIS RIGHT EYE.IMBEDDED MATERIAL HAD TO BE REMOVED BY A DOCTOR AND WORKER WAS GIVEN A PRESCRIPTION. EMPLOYEE DID NOT LOSE TIME FROM WORK.

On 10/30/08, worker was changing a screen in the mill and felt pain in upper right arm. He was diagnosed with a strain and lost no time from work. During the next year he sought further medical attention and was diagnosed with a torn rotator cuff. Since there was no lost time and only diagnostic medical treatment, injury did not become reportable until surgery on 1/6/10.

WHILE CLEARING SNOW FROM A STAIRWAY, WORKER SLIPPED ON ICE/SNOW AND HIT HIS SHOULDER ON HAND RAIL. THIS INJURY DID NOT BECOME REPORTABLE UNTIL 1/28/09 WHEN, AS HIS PAIN WORSENED, WORKER WENT TO DOCTOR AND WAS DIAGNOSED WITH A TORN ROTATOR CUFF. WORKER WAS ON SEASONAL LAYOFF AS OF 12/24/08.

While removing cutting edge bolts on loader bucket with an air wrench, & while holding wrench on the nut & injured hand on bolt head, the bolt head spun & cut his finger through his glove. Worker went to emergency room & received sutures. Employee was allowed to return to work the next day & did not lose time from work.

Worker injured left thumb while pulling down overgrown tree limbs that had been cut around the mill area. He went to hospital ER where they put a splint on the hand to immobilize it & referred him to another dr. for possible ligament damage. Extent of injury has not as yet been determined & worker will remain on light duty (per ER dr) until his visit to specialist.

Worker experienced back pain while lifting screens in mill on 6/27/07. Did not report incident until 6/29/07 when back pain got increasingly worse.

While operating front-end loader, digging shot rock, EE was repeatedly jarred in cab. He experienced back pain when he woke up the next morning.

Struck by falling object

While unloading a truck, worker was struck on left knee cap by falling pallet. Knee became swollen and worker was instructed by doctor to remain out of work for two days. He returned to work with no restrictions.

Employee lifted a cutting edge plate into a loader bucket and then experienced back pain after he drove the loader over a bumpy road.

Employee slipped on truck steps while inspecting water tank on water truck. He received a muscle strain in groin area. He returned to work the next day but pain did not subside. He saw the doctor again on 9/11/06 for further treatment and started to lose time on 9/12/06 at which time this injury became reportable.

While EE was using a grinding wheel, an airborne metal particle flew under his face shield and became embedded in his right eye.

Employee was attempting to clear a jammed stone bin by pushing and pulling the bin handle. He experienced back pain but continued to work until he started to hoe out material under the feeder belt and pain got worse.

Employee was attempting to put an angle iron stand underneath a drainage pipeline when the last remaining bolt holding the pipes together broke and the ankle iron frame kicked out and struck his left leg causing a contusion and laceration that required sutures.

Employee was unplugging a chute under the crusher when a falling rock hit his knee. He lost his footing, fell and pulled a muscle on the right side of his lower back.

WHILE EMPLOYEE WAS WORKING IN THE MILL, WIND BLEW A FOREIGN PARTICLE INTO HIS EYE. PARTICLE BECAME EMBEDDED & HAD TO BE REMOVED.

Employee fell and injured his shoulder when the weld securing the stair tread to the stair stringer broke.

DRIVER DUMPED HIS LOAD & PULLED OUT OF STOCKPILE AREA W/HIS TRUCK BODY IN THE UP OR DUMP POSITION. HE TRAVELED APPROX. 170'BEFORE CROSSING ONTO MINE PROPERTY AGAIN & TRAVELED APPROX. 30'BEFORE CONTACTING 600 VOLT OVERHEAD POWER LINES. THE DRIVER REMAINED IN THE TRUCK FOR APPROX. 45 MINUTES UNTIL CONDITIONS WERE DEEMED SAFE FOR HIM TO EXIT. HE WAS NOT INJURED.

EE WAS LIFTING BUSHINGS WHILE DOING MILL MAINTENANCE AND EXPERIENCED PAIN IN LOWER BACK AND NUMBNESS IN LEFT LEG. HE CONTINUED TO HAVE LOWER BACK PAIN RADIATING INTO HIS LEFT LEG, AND AFTER A DIAGNOSIS OF A HERNIATED DISC. EE HAD SURGERY. THERE WAS NO MEDICAL TREATMENT OR LOST TIME UNTIL THE DATE OF SURGERY 7/23/02 AT WHICH TIME ACCIDENT BECAME REPORTABLE.

THIS INJURY DIDN'T BECOME REPORTABLE UNTIL FINAL DIAGNOSIS RESULTEDIN SURGERY AND LOST TIME.
