While operator was walking portable conveyor into place, EE inadvertently rolled the track over left foot. Fracturing EE's left foot (toe).

The miner came in contact with return idler on screen and was pulled in by the conveyor belt which burned & bruised EE's arm, straining the muscles.

Employee was cleaning under conveyor, when his hard hat fell off, while retrieving his hardhat he stood up and struck his head on the return roller guard, causing a laceration to the top of his head. needing 5 staples.

Employee was using a 1/2"" drive ratchet with a 5/8"" socket to loosen bolt, bolt released unexpectedly causing employee to come in contact with a sharp corner edge of screen counter weight, thus cutting backside of right hand.
