The employee was using a HAO switch in an electrical room when the bucket for a different piece of equipment arc'd. A flash of light and smoke came from the arc'd bucket. The employee was having dry eye symptoms and was prescribed antibiotic eye drops for precautionary purposes.

EE was running wire when energized motor leads laying on the floor of the wire way were apparently sawed through wire insulation and caused a short and caused a arc flash. EE received burns to hands, arms, and face. EE was treated at hospital for burns release from hospital on 3/16/15.

Electrician sustained a small electrical shock from a 4160 motor. Power was pulled, but motor holds a small charge unless grounded. Electrician did not ground leads & when he reached in to disconnect, right forearm touched the side of the junction box creating a ground - ee was shocked. He was taken to hospital to have blood work & EKG to make sure heart was ok.

EMPLOYEE WAS CHECKING TO SEE WHY MECHANICAL BREAK WAS NOT WORKING ON THE CRANE.HE STARTED TO CHECK FOR LOOSE WIRES ON THE TERMINAL BLOCK WHEN THERE WAS A FLASH AND HIS RIGHT HAND AND 3 FINGERS WERE BURNED FROM THE ARCH.
