A piece of rock struck a miner on the left hip/back while drilling a hole to install a roof bolt. The vibration of the rotating drill steel in adverse conditions caused the rock to fall. The miner attempted to get out of the way when EE noticed the rock moving.
Miner was standing between roof bolts when a piece of rock (grey shale) fell from between the bolts and contacted the injured miner. The area where the accident occurred was heavily rock dusted which made sight of fracture lines hard to see.
A miner was injured starting to drill a hole in the roof where adverse conditions existed. EE WAS INSTALLING ROOF BOLT AT CORNER OF NEWLY TURNED XCUT. ROOFBOLTING MACHINE WAS POSITIONED ACCORDING TO ROOF CONTROL PLAN. APPROX 30 LB PIECE OF ROCK FELL OUT BETWEEN EXISTING INSTALLED ROOF BOLTS, STRIKING EE ON HIS LOWER LEFT LUMBAR REGION. EE FINISHED SHIFT.
EMPLOYEE WAS OPERATING A ROOF BOLTER WHEN A SMALL SECTION OF ROCK FELL & STRUCK THE CANOPY. IT BROKE INTO PIECES & A SMALL PIECE STRUCK EE JUST ABOVE HIS UPPER LIP. HE SUSTAINED A CUT REQUIRING 8 STITCHES.
EE HAD FINISHED DRILLING THE OUTSIDE HOLE ON THE LAST ROW OF BOLTS. HE LEANED OVER TO REMOVE THE DRILL STEELS AND A PIECE OF ROCK 20' X 12' X 16' AND A PIECE OF COAL 16' X 14' X 6' FELL STRIKI NG HIM ON THE LOWER BACK.
INJURED PERSON WAS BENT DOWN ALONG THE OUTBY RIB WATCHING A COWORKER OPERATE THE CONTINUOUS MINER. HE WAS STRUCK BY A PIECE OF FALLING BROW CONSISTING OF MIXED COAL AND BONEY. IT MEASURED 4' X 2 1/2' X 15". HE SUSTAINED A SPRAINED NECK, BACK, BRUISED SHOULDER AND CONTUSION TO RIGHT FOREARM.
