Caught in, under or between a moving and a stationary object

A conveyor tail of a cont. miner was in the shop for repair. It was being raised by an overhead hoist. One end of the tail was supported with crib blocks. While placing cribs under the other end of the tail, one lifting chain slipped from the overhead hoist causing one end of the tail to drop. The man's hand was pinched between the edge of the crib and tail.

The man was performing general labor duties, when he reported a pain in his left shoulder. He stated he did not know the cause of the injury.

The man was pushing on a wrench to loosen a tight bolt when he felt a pain in his side.

The man was kneeling down and started to bleed the control transformer to ground when he got shock.

Inhalation of radiations, caustics, toxic and noxious substances

Exact date and time are unknown as this is an occupational illness. Roxcoal received a letter from MSHA dated September 5,2007 stating that this employee has a sufficient degree of black lung to be eligible for Part 90 rights. He is currently off work on compensation from another matter. When he returns we will follow Part 90 requirements.

Employee was running rock duster out #8 entry when scoop ran over crib, jarring back. Employee had a history of back trouble, including a prior back surgery.

Rock 20"" by 20"" fell from roof and hit him on right shoulder.

While hanging cable along rib, EE picked up cable on or near damaged area, receiving significant electrical shock.

Employee started his regular shift with no pain in knee. At some point in the shift, knee started to hurt; no mechanism of injury. Employee wasn't concerned. As week progressed, pain got worse and employee sought medical attention. Mine has 44"" height; employee on knees at all times.

Struck against a moving object

One scoop was pushing another scoop that broke down when one bucket went over the top of the other. The injured employee who was operating the disabled scoop bounced in his operator's compartment injuring his lower back.

Accident type, without injuries

The grout in an unmapped borehole pushed out releasing a low oxygen condition into face of the # 7 entry of the Sioux working section. More air was introduced, Low O2 cleared under supervision of Federal Inspector and Section Foreman. Borehole was securely plugged from bottom.

Fall from machine

Employee fell on ice exiting his vehicle in the parking lot. He fell on his back. He refused to be medically transported. Person worked the 27th of Feb, and reported off the 28th. Returned 3/6/07 - total 4 days last time.

Over-exertion in lifting objects

While loading 50 lb rock dust bags on scoop while lifting and turning employee felt pain in lower abodomen he had been diagnosed with a hernia for which he will recieved as operation.

While operating Johnson Industries mantrip, employee caught tire on coal rib causing steering wheel to spin causing laceration to left little finger requiring four stitches.

Employee was pushfing large wooden block from under suspended roof bolter in shop. When he experienced pain in right shoulder. He worked until 9-11-06 when he took off the have it repaired.

EE was pushing golf-cart on slippery hill when he lost footing and fell. Cart then ran over Rt. leg & hip area. EE brought in release from work starting 7/19/06

An area of dark shale with slicken sides fell pulling center bolts in last row of supports. Fall measured 15' long x 15' wide and from 3'to 5'high. Fall pinched miner behind hinge point.

Employee was operating shuttle car on uneven bottom when he trammed over bottom roll at fast pace car bottomed out slamming employee into seat. He sustained compression to back.

While standing along travelway, scoop went past employee dragging length of 3"" black plastic carlon pipe. End of pipe curled around striking employee on left ankle causing sprain.

Injured person was operating a shuttle car when the tires hit a hole in bottom. this action jarred his back. he sustained a bulged disc in his lower back which requires physical therapy and time off work.

WHILE EE WAS TRAMMING A SHUTTLE CAR TO THE MINER, THE METAL COVER OVER THE CABLE REEL CAME LOOS E. THE COVER BOUNCED ONTO THE TIRE WHICH IN TURNPROJECTED THE COVER HITTING THE EE IN THE BACK O F THE HEAD.

EE WAS REPAIRING A BROKEN TRACK PAD ON A FEEDER/BREAKER. THE FEEDER FRAME WAS BLOCKED UP WITH CRIBS. THE TRACK ASSEMBLY WAS NOT BLOCKED SO AS TO GAIN ACCESS TO THE BROKEN TRACK PAD. AS THE EE WAS INSERTING THE PIN IN THE TRACK PADS, THE TRACK ASSEMBLY DROPPED SMASHING HIS MIDDLE FINGER ON RIGHT HAND BETWEEN A CRIB AND TRACK ASSEMBLY.
