A conveyor tail of a cont. miner was in the shop for repair. It was being raised by an overhead hoist. One end of the tail was supported with crib blocks. While placing cribs under the other end of the tail, one lifting chain slipped from the overhead hoist causing one end of the tail to drop. The man's hand was pinched between the edge of the crib and tail.
The man was performing general labor duties, when he reported a pain in his left shoulder. He stated he did not know the cause of the injury.
The man was pushing on a wrench to loosen a tight bolt when he felt a pain in his side.
The man was kneeling down and started to bleed the control transformer to ground when he got shock.
Exact date and time are unknown as this is an occupational illness. Roxcoal received a letter from MSHA dated September 5,2007 stating that this employee has a sufficient degree of black lung to be eligible for Part 90 rights. He is currently off work on compensation from another matter. When he returns we will follow Part 90 requirements.
Employee was running rock duster out #8 entry when scoop ran over crib, jarring back. Employee had a history of back trouble, including a prior back surgery.
While hanging cable along rib, EE picked up cable on or near damaged area, receiving significant electrical shock.
Employee started his regular shift with no pain in knee. At some point in the shift, knee started to hurt; no mechanism of injury. Employee wasn't concerned. As week progressed, pain got worse and employee sought medical attention. Mine has 44"" height; employee on knees at all times.
One scoop was pushing another scoop that broke down when one bucket went over the top of the other. The injured employee who was operating the disabled scoop bounced in his operator's compartment injuring his lower back.
The grout in an unmapped borehole pushed out releasing a low oxygen condition into face of the # 7 entry of the Sioux working section. More air was introduced, Low O2 cleared under supervision of Federal Inspector and Section Foreman. Borehole was securely plugged from bottom.
Employee fell on ice exiting his vehicle in the parking lot. He fell on his back. He refused to be medically transported. Person worked the 27th of Feb, and reported off the 28th. Returned 3/6/07 - total 4 days last time.
EE was reaching into the automatic dump. The flap fell shut on his fingers, crushing them.
While loading 50 lb rock dust bags on scoop while lifting and turning employee felt pain in lower abodomen he had been diagnosed with a hernia for which he will recieved as operation.
While operating Johnson Industries mantrip, employee caught tire on coal rib causing steering wheel to spin causing laceration to left little finger requiring four stitches.
Employee was pushfing large wooden block from under suspended roof bolter in shop. When he experienced pain in right shoulder. He worked until 9-11-06 when he took off the have it repaired.
EE was pushing golf-cart on slippery hill when he lost footing and fell. Cart then ran over Rt. leg & hip area. EE brought in release from work starting 7/19/06
An area of dark shale with slicken sides fell pulling center bolts in last row of supports. Fall measured 15' long x 15' wide and from 3'to 5'high. Fall pinched miner behind hinge point.
Employee was operating shuttle car on uneven bottom when he trammed over bottom roll at fast pace car bottomed out slamming employee into seat. He sustained compression to back.
RECEIVED A LETTER STATING THE MINER HAD AN X-RAY INDICATING EVIDENCE OF PNEUMOCONIOSES.
Received letter stating miner shows evidence of pneumoconiosis.
While drilling hole for roof bolt EE got piece of foreign object in eye. Had to get medical treatment with prescription eye drops.
While standing along travelway, scoop went past employee dragging length of 3"" black plastic carlon pipe. End of pipe curled around striking employee on left ankle causing sprain.
Injured person was operating a shuttle car when the tires hit a hole in bottom. this action jarred his back. he sustained a bulged disc in his lower back which requires physical therapy and time off work.
While using hand held grinder in a confined area employee struck on right pointer finger with rotating wheel causing cut/abrasion to right finger. started/showing signs of infection. He went to dr 9-23-04 and recieved antibiotics.
WHILE DOING ON-SHIFT EXAMINATION OF BELTS EMPLOYEE LOST CONTROL OF GOLF CART/MANTRIP. LEFT TIRE CAUGHT RIB JAMMING STEERING KNOB INTO HAND CAUSING FRACTURE TO RIGHT WRIST.
WHILE MAKING REPAIRS ON MINER CABLE, EMPLOYEE PULLED UTILITY KNIFE OVER LEFT THUMB CAUSING LACERATION REQUIRING 5 STITCHES.
CONTACTORS ON MANTRIP WOULD NOT STAY ENGAGED. EE WAS USING A HAMMER TO MANUALLY KEEP CONTACTORS ENGAGED. HAMMER STRUCK ROOF SUPPORT SQUEEZING HAND BETWEEN SUPPORT AND HAMMER. EE RETURNED TO LIGHT DUTY.
WHILE OPERATING SCOOP IN THE #4 ENTRY OF 1 LT, OPERATOR CAUGHT BUCKET ON RIB OFFSET. SCOOP ARCHED PINCHING OPERATOR'S HEAD AGAINST ROOF CUTTING RIGHT EAR.
WHILE TRAMMING SHUTTLE CAR, OPERATOR RAN THROUGH HOLE IN BOTTOM. SHUTTLE CAR SLAMMED INTO THE BOTTOM JARRING OPERATORS NECK CAUSING SORENESS AND STIFFNESS. EMPLOYEE RETURNED ON LIGHT DUTY.
WHILE MINING IN THE ZERO ENTRY OF THE SHAWNEE RT SECTION MMU-004-0, LEFT SIDE OF FACE HOLED THROUGH TO AN UN-MAPPED PORTION OF AN UNDERGROUND MINE RELEASING A LARGE AMT OF TRAPPED WATER. MININ G OPERATIONS WERE HALTED UNTIL AN INVESTIGATION IS COMPLETED. NO PERSONS WERE HURT, TRAPPED OR ENDANGERED, NOR EQUIPMENT DAMAGED.
EMPLOYEE WAS LOADING ROOF CHANEL ON TOP OF BOLTER ATRS WHEN ATRS SHIFTED PINCHING FINGER BETWEEN ATRS AND ROOF RESULTING IN FRACTURE WITH STITCHES & SPLINT.

EE NOTICED SWELLING IN LEFT KNEE & WENT TO DR.. HE WAS DIAGNOSED WITH MINERS KNEE RECIEVED PRESCRIPTION MEDICINE.
EMPLOYEE WAS CUTTING PIECE OF MINE BELTING WHILE PULLING KNIFE TOWARD HIMSELF EMPLOYEE SLIPPED WITH KNIFE AND STABBED HIMSELF IN LEFT LEG REQUIRING SEVERAL STITCHES.
EMPLOYEE WAS TRAMMING SHUTTLE CAR IN X-CUT WHEN HE RAN OVER A ROCK CAUSING SHUTTLE CAR TO BOTTOM OUT JARRING LOWER BACK. WMPLOYEE WORKED UNTIL 6/6/03 AND STARTED MISSING TIME ON 6/9/03.
A SLICKEN SIDED SLIP WHICH ROLLED UP INTO THE ROOF INT HE UNSUPPORTED C4 FELL OUT AND CAME OVER HEAD OF MINER PULLING 3 ROWS OF BOLTS AND CHANNELS AND APPROX. 3 PARTIAL ROWS OF BOLTS. NO INJUR IES CONT. MINER WAS PINCHED FOR APPROX. 8 HRS.

EMPLOYEE LEANED OVER SIDE OF SCOOP BUCKET TO UNTANGLE CABLE WHILE SCOOP OPERATOR HELD SCOOP IN POSITION SCOOP DRIFTED APPROX 6"" OVER BOTTOM ROLL PINCHING EMPLOYEE BETWEEN BUCKET SIDE, EMPLOYEE'S SELF RESCUER AND MINE ROOF.
WHILE LOWERING SAFETY POST ON FLETCHER BOLTER, EE PLACED HAND BETWEEN TOP OF TELESCOPING SECTION AND DESCENDING POST SMASHING TIP OF RIGHT RING FINGER.
WHILE EMPLOYEE WAS UNLOADING BELT STRUCTURE A BELT RAIL SLIPPED OFF THE SCOOP BUCKET SMASHING 2 FINGERS ON HIS RIGHT HAND BETWEEN THE RAIL & MINE FLOOR RESULTING IN A BROKEN RING FINGER.
EE STATES WHILE THROWING CRIB BLOCK FROM KNEELING POSITION HE FELT POP IN RIGHT KNEE. HE THEN EXPERIENCED PAIN. HE WENT LATER FOR MEDICAL TREATMENT AND STARTED MISSING TIME.
EMPLOYEE WAS OPERATING SHUTTLE CAR IN SECTION WHEN HE STRUCK HEAD ON ROOF JOLTING NECK.
WHEN EMPLOYEE WAS LINING BOLT HEAD UP TO FIT IN BOLTER CHUCK HE ACCIDENTLY HIT THE ROTATION AND CAUGHT GLOVE BETWEEN BOLT HEAD AND CHUCK. HE RECEIVED FRACTURES TO LEFT POINTER & RING FINGERS.
EMPLOYEE MISSED WORK 6-10-02 WHEN CONTACTED HE STATED HE WAS GETTING SHUTTLE CAR LOADED ON 6-5-02 WHEN HE TURNED HIS HEAD HE FELT POP IN HIS NECK. HE STATED NECK KEPT GETTING STIFFER UNTIL HE SOUGHT MEDICAL TREATMENT ON 6-10-02.
A WATER SPRAY BAR BECAME CAUGHT IN THE SECTION FEEDER CRUSHER. EE SHUT OFF ELECTRICAL BREAKER BUT FAILED TO WAIT UNTIL CRUSHER STOPPED ROTATING. HE REACHED IN AS CRUSHER DID LAS REVOLUTION AND CAUGHT HAND BETWEEN SPRAY BAR PIPE AND FEEDER FRAME.
EE WAS PULLING LENGTH OF MINER CABLE WHEN HE FELT PAIN IN RT. KNEE. HE WORE A BANDAGE UNTIL KNEESTARTED TO SWELL AND CAUSE MORE PAIN. HE STARTEDMISSING TIME ON 2ND SHIFT 5-20-02.
WHILE CUTTING MINE BELT WITH UTILITY KNIFE, KNIFE SLIPPED OVER BELT SQUARE AND CUT EMPLOYEE ON RIGHT THUMB REQUIRING THREE STITCHES AND TETANUS SHOT.
WHILE DIRECTING MINER CABLE BEING DRUG ALONG SIDE MINER WITH LEFT LEG AND FOOT EMPLOYEE'S FOOT GOT CAUGHT ON CABLE TWISTING AND WRENCHING KNEE. EMPLOYEE STARTED MISSING TIME FOR OPERATION 1/29 /02.
EE SLIPPED AFTER GETTING OUT OF SHOWER, STRIKINGLEFT FOOT ON LOCKER. HE DID NOT MISS ANY DAYS. HE THEN AGGRAVATED THE SAME INJURY AND STARTED MISSING TIME 12/28/01.
AN AREA OF ROOF CONTAINING A SLIP WHICH OPENED AFTER BOLTING WITH 42"" BOLTS ON A 4 BY 5' CENTER FELL IN AN AREA APPROX 18 BY 20' WIDE TO A HEIGHT OF FROM 2' TO APPROX 4 1/2' HIGH. THE AREA WAS TIMBERED OFF AND DANGERED.
WHILE MINING IN THE #10 ROOM OF THE SIOUX SECTION APPROX. 100' OFF THE #9 ENTRY THE LEFT SIDE OF THE ACTIVE. FACE HOLED THORUGH TO WHAT APPEARED TO BE HIWALL BACK FILL.
WHILE CUTTING INSULATION OFF MOTOR LEADS WITH UTILITY KNIFE, KNIFE SLIPPED, CUTTING EE ON LEFT HAND, REQUIRING 5 STITCHES.
MAKING A SPLICE WITH A UTILITY KNIFE. KNIFE SLIPPED ON BELT, CUTTING RIGHT INDEX FINGER. EE WORKED 3 DAYS THEN WENT ON VACATION FOR WEEK BEFORE REPORTING STITCHES OF WHICH HE RECEIVED THREE WH ILE ON VACATION.
EE WAS FILLING IN ON THE ROOF BOLTER, LEFT SIDE. EE WAS ABOUT TO START DRILLING THE INSIDE HOLE OF THE SECOND ROW. WHEN THE DRILL STEEL MET THE ROOF, A 12"" X 12"" X 1 1/2"" PIECE OF ROOF FELL ON EE'S LEFT WRIST & HAND.
EE RECEIVED A CUT (LACERATION) ON RIGHT THUMB FROM A SHARP EDGE OF A ROOF PLATE WHILE LOADING BOLTER SUPPLIES.
EE WAS WALKING IN BELT ENTRY ALONG SIDE OF FEEDER. EE SLIPPED ON LOOSE COAL AND TWISTED KNEE.
WHILE ROOF BOLTING, THE BOLTER OPERATOR VICTIM WSHIFTED HIS BODY WHILE KNEELING DOWN AND INJURED HIS RIGHT KNEE.
EMPLOYEE WAS USING SLEDGE HAMMER ON SIDE PANEL OF A FROZEN CHUTE; HE RUPTURED LEFT BICEP MUSCLE- LOST TIME ACCIDENT STARTED 4/27/00 WHEN HE HAD HIS BICEP SURGERY.
WHILE EE WAS TRAMMING A SHUTTLE CAR TO THE MINER, THE METAL COVER OVER THE CABLE REEL CAME LOOS E. THE COVER BOUNCED ONTO THE TIRE WHICH IN TURNPROJECTED THE COVER HITTING THE EE IN THE BACK O F THE HEAD.
EE WAS REPAIRING A BROKEN TRACK PAD ON A FEEDER/BREAKER. THE FEEDER FRAME WAS BLOCKED UP WITH CRIBS. THE TRACK ASSEMBLY WAS NOT BLOCKED SO AS TO GAIN ACCESS TO THE BROKEN TRACK PAD. AS THE EE WAS INSERTING THE PIN IN THE TRACK PADS, THE TRACK ASSEMBLY DROPPED SMASHING HIS MIDDLE FINGER ON RIGHT HAND BETWEEN A CRIB AND TRACK ASSEMBLY.
