Showing all 26EE was removing a punch plate from the shaker tower when the plate moved unexpectedly pinching two middle fingers on right hand against the frame of the shaker.
Team members replacing return roller, shaft began to slip, team member grabbed the shaft, causing pain to the right wrist.
Team member installing new exhaust gaskets, bolting up the flanges when debris fell onto and behind safety glasses, into right eye. Flushed eyes at the shop, debris remained behind eyelid.
large rock got stuck in the jaw of the crusher. A TM attempted to use a chain to wedge between the rock and the jaw dies to break it up. As the rock and jaw pulled the chain down the TM tried to pull the chain up, resulting in smashing EE's hand between the chain and the railing causing laceration to left palm and a fracture to the 5th metacarpal
Miner was lifting a train door opening bar when the door bound causing EE to strain a back muscle.
Miner was manually opening a dump train car when material sloughed down onto the door causing it to rapidly open jerking the miner's arms causing a shoulder strain
An operator, needed assistance getting a loader out of the shop. They had a 10ft stand under the lift arm, supporting the bucket. When I went to pull the stand out from under the loader, I injured my back. I felt a sharp pain in my lower back immediately. I laid down for a few minutes, then left work to go to the chiropractor.
Individual attempted to lift a 5-6 ft. long chunk of I-beam (weighing approximately 150 lbs.) and toss it over the handrail of an elevated screen deck to the ground below. The I-beam shifted and pinched their right thumb between the beam and handrail. Individual complained of extreme pain and was transported to Urgent Care where an X-Ray discovered a broken bone in the right thumb.
The team member was assisting with the installation of a new Dredge bucket when their left index finger became caught in-between a becket and the work platform causing a laceration.
WHILE EXITING SKID STEER, EMPLOYEE FELT POP IN STOMACH.

employee lifted the motor up to put it down where we could use the Crain to hoist it the rest of the way out. while lifting the motor employee felt something tear in the abdominal area. incident occurred 8/26/24 - it was reported 8/28/24 and found out EE was going to receive surgery 9/20/24 then surgery was moved up to 10/7/24
Team member got dirt and a small cut on their left eyelid on 6/24/2024. By 6/26/2024, the eye was swollen and became painful. Team member sought medical care at a local eyecare center.
Team member was removing washer bearing from back of log washing machine. While pushing the bearing flange through the wall, their finger was pinched between the wall and the flange causing a laceration to pinky finger which required stitches.
Employee was moving a conveyor belt onto a stand, upon moving the belt EE's fingers were caught in between the conveyor belt and the screen guarding. the screen went through EE's glove and cause a laceration to the right index finger. resulting in three stitches. employee was released for work with no lost time.
Team member was guiding the leg of a crusher into the foot pedestal. As the foot fell into place, it caught the right ring finger when the leg slid into the stand. Team member sustained a right ring fingertip amputation without bone loss.
EE was lifting a return roller and handing it to a co-worker when EE twisted at the waist and strained back. (Lifting a Return Roller). Update: EE resigned, was not terminated by Knife River South Dakota.
MINER WAS INSTALLING A BRACKET ON THE WALL AND BRUSHED THEIR ARM AGAINST ONE OF THE PROTRUDING BOLTS THAT HAD NOT YET BEEN FULLY SECURED RESULTING IN A LACERATION TO THEIR ARM THAT REQUIRED SUTURES.
Employee was hit by an anvil from a VSI crusher. The anvils were being removed and thrown out of the machine by another employee. The employee that was struck walked into the area where the anvils were being tossed to the ground. EE was struck in the lower back and buttocks. EE had scrapes and bruising.
Dragging cords feeling pop in lower back. Tried first aid when injury occurred. First visit to see a Dr. was on 8/7/23 with no additional treatment. 8/14/23 for a recheck appointment receiving additional treatment making injury reportable.
Injury to right shoulder while opening rail car doors to dump gravel at receiving facility. Injury exhibited as tear of long head bicep tendon.
Working on conveyor belting, hand slipped and hit back of hand on cross brace angle iron. Small cut on the back of right hand. Required 4 stitches
Employee was cleaning the deck of the dredge off using a hose and was backing up while pulling the hose. They tripped over the anchor cable and their leg was momentarily caught beneath the cable during the fall causing a dislocation of the kneecap and fracture of the right femur lateral condyle.
