Tunnel feeder had concrete chunks get stuck on crossbeams and needed to be removed before any more material is fed into the feeder. Employee took a 12lbs sledgehammer and tried to break material, this did not work. After this task the Employee felt pain in their right hand. Employee did not report the pain until 03/27/2026 at 630am after medical treatment

Team member was assisting rolling pipes on the dredge. Tool became disengaged when team member went to pull down on handle, causing them to lose balance and fall backwards onto a metal box.

An electrical fire in the MCC initiated upon startup of operational controls. Damage includes a total loss of MCC and appurtenant wiring/electrical systems. No persons were injured.

ON 8/16/18, MINER REPORTED EXPERIENCING SEVERE BACK PAIN AFTER SITTING IN A CHAIR WHILE OPERATING THE DREDGE ON 8/14/18, FOLLOWED BY A 12 HR SHIFT PERFORMING MAINTENANCE ON 8/15/18. MINER INITIALLY REPORTED TO SUPERVISOR AND A GROUP OF MORE THAN FIVE OTHER MINERS THAT BACK PAIN WAS NOT RELATED TO WORK CONDITIONS.

Employee was putting an eye on the end of cable. Cable slipped out of employee's hands, striking EE on the chin and causing lacerations to chin and breaking tooth.

Miner was using a utility knife to cut the tip off tube of silicone and sliced thumb.

Fall to the walkway or working surface

Employee was replacing a 10"" diameter pipe that went through the catwalk (expanded metal) that he was working on. After he removed the pipe and co-workers were raising the new pipe, the employee turned to retrieve an air hose and his left leg fell into the 10"" hole in the catwalk where the old pipe used to be.

Employee was bent over dragging hose under conveyor. Hose became stuck. Employee pulled hard to dislodge it and felt a pulling sensation in his lower back that gradually became painful during remainder of shift.

EE WAS TAKING A SAMPLE OF 10-0 WITH SAMPLE TRAY SHE SET TRAY ON OTHER SIDE OF BELT & PULLED THROUGH-STREAM OF MATERIAL WHILE PULLING IT THROUGH,SOMETHING GOT CAUGHT ON THE TRAY, FLIPPING IT UP AND PINCHING HER FINGER BETWEEN THE SAMPLER AND BELT CAUSING THE CUT FINGER.

LOADING 4' X 6' WIRE SCREEN, APPROXIMATELY 40 POUNDS, ONTO SERVICE TRUCK, MISSED, RACK DROPPED ONTO LEFT FOOT, MISSING STEEL TOE OF BOOT. PUNCTURED HOLE THROUGH BOOT AND INTO FOOT.
