The man was sitting on the mantrip. As the mantrip began to move, he noticed the lunch bag on top of the mantrip was rubbing against the roof. He stated he told the operator to stop and reached for the bag. The operator did not hear him and continued to tram. The mantrip struck a rock causing the man's arm to momentarily be pinched between the mantrip and the roof.
The man had repaired the shuttle car cable. He was sitting in the shuttle car winding up the cable slack. He leaned out of car to see how much slack was left when the cable came up quickly and struck him on the head.

While placing cribs under the other end of the tail, one lifting chain slipped from the overhead hoist causing one end of the tail to drop. The man's hand was pinched between the edge of the crib and tail.
Employee was changing a cutting edge on a WA180. He was working on removing a nut, when it fell. He reached under the bucket to turn bolt to engage the square hole, to finish removing the nut. The bolt head was worn down and sharp. It cut through his leather glove and middle fingertip on his left hand.
