Showing 50 of 52Employee was in the process of changing wear iron on a cone crusher. While installing the feed plate into the bottom of the crusher, and getting it into its final position, the metal plate slipped unexpectedly, pinching their right ring finger between the feed plate and the screen deck.
Employee was attempting to remove a v-belt from a sheave on the main drive of a wet screen. Another emloyee was assisting to remove tension on the belt by raising the drive end of the belt pulley while the injured employee attempted to lift the belt off the sheave. While grasping the belt near the motor end the belt moved back over the sheave & pinched injured employee's finger.
Employee was lifting a belt motor on to an in place belt line for future installation. As ee went to set it on the belt with the help of another employee left middle finger was pinched between the motor and the belt causing a laceration on the pad of finger.
EE1 was picking up tools from his work area on the lower east side cat walk of primary #2 screen when EE2, who was located on the upper cat walk of Primary #2, picked up a swing gate he was installing. When EE2 picked up the gate it came apart in two pieces, one piece falling down and striking EE1 in the nose. EE1 suffered from a laceration on his nose.
EE had removed 4X6 - 1 ½ Screens from deck, he had lifted the screen up over Handrail and was lowering to lower level (work deck) approx. 10 ft. when he let go his left middle finger caught between Handrail & Screen causing injury to finger. Nail avulsion & fracture of finger tip.
Employee was removing a guard in order to perform required maintenance. As he pulled on the guard to remove, it sprung back pinching his left pinky finger between the guard and the mounting bolt. Employee was wearing leather gloves.
Mechanic was working out of a man lift and leaned against the hand rail while handing another employee a steel plate, the employee help a pop and sharp pain at his lower rib cage. The employee requested to see a doctor on 1-6 and was diagnosed with a sprained lower rib on his right side. He is on restricted duty with a recheck on 1-9-12.
At the end of shift on 5-10-11 EE removed glasses to wipe his face off and got dust in his eye. EE flushed his eye out before going home and felt that his eye was ok. On 5-12-11 EE woke up and his eye was red and inflamed; EE was taken to the clinic where a foreign object was removed and antibiotic drops were prescribed.
Employee was pouring concrete and contaminated knees with wet concrete causing a minor chemical burn. First aid was applied at that time. On 3/1/11 the knee had become inflamed and the employee was examined by a physician. The physician gave the employee an antibiotic to treat a potential infection.
While EE1 was holding the bottom of one side of the head pulley drive guard, EE2 lifted the other side, causing the guard to smash EE1's left 4th finger. Laceration to finger
Plant operator was stepping down from the back of his truck while wearing his fall protection harness with his lanyard clipped to his chest strap. In the process of stepping down the pelican style snap hook contacted the edge of a skirt box causing the snap hook to flip up and chip one of the employees teeth.
Miner opening rail car hopper doors when load in rail car became unbalanced causing car to suddenly lean, pinching miner between car and catwalk handrail. Rail car is a rotary dump car and material was not flowing out from one side of car when bottom dump hopper doors were opened.
Employee was removing polyurethane screen panels from the dewatering screen; when screen came loose, screen popped out and disloadged a rock. Dislodged rock hit and entered employee's right ear.
Employee was changing screens; while changing screens employee twisted and hurt lower back.

EE pulled on v-belt and caught his right hand finger in pulley. Equipment was locked/tagged out.
EE placing cribbing under rock breaker base to facilitate removing 4 pin for the hydrallic ram was struck on the left index finger by 2 1/2' diameter 4 'long bar of roundstake which was leaning against the Rock Breaker Base.
EMPLOYEE WAS SHOVELING DIRT IN A LIME PLANT WHEN DUST GOT IN HIS EYES. THE EMPLOYEE WAS WEARING SAFETY GLASSES AND RESPIRATOR AT THE TIME OF THE ACCIDENT. EMPLOYEE RECEIVED A CHEMICAL IRRITATION OF BOTH EYES.
Employee had to shovel some excessive sand under tail pulley and later experienced pain in his lower back.
Person was cleaning a conveyor guard.
Injured Worker (IW) picked up crane's outrigger pad from squatting position, stood up and took 3 steps while turning to his left when he felt a 'pop' in his low back. IW sustained a muscle strain in low back.
EMPLOYEE MISSED A STEP WHILE GETTING INTO THE LOADER. HOLDING LADDER WITH RIGHT HAND HE PULLED SHOULDER.
EMPLOYEE IS A SHOP MECHANIC AND WAS WORKING UNDER A ROCK TRUCK FOR ABOUT THREE DAYS AND HIS RIGHT KNEE BECAME SORE.
TWO EMPLOYEES WERE REMOVING A PADDLE WHEEL FROM THE END OF A CONVEYOR BELT STACKER. THEY WERE USING A MANLIFT TO GAIN ACCESS TO PADDLE WHEEL. AFTER UNBELTING THE PADDLE WHEEL INJURED EMPLOYEE GRABBED SHAFT WITH HIS LEFT HAND & PULLED IT OUT OF BARRING MOVEMENT. SHAFT DROPPED 3" & SMASHED THUMBNAIL ON LEFT HAND BETWEEN SHAFT & ANGLE IRON FRAME. THUMBNAIL WAS REMOVED AT HOSPITAL.
EMPLOYEE WAS GREASING THE JAW ON CRUSHER, KNEELING DOWN. A METAL BAR USED TO LOOSEN JAMMED ROCKS WAS STORED AT THE TOP OF THE CRUSHER, AND IT FELL APPROX 12 FT AND STRUCK EE ON THE BACK OF HIS HEAD, KNOCKING HIM TO THE GROUND. EE WAS WEARING ALL REQUIRED PPE (I.E. HARD HAT).
REPORTED AS LOADING CRANE CLAIM IS UNDER INVESTIGATION. EE DID NOT REPORT INCIDENT.
EE WAS PULLING CHAIN THROUGH HOLE IN K-RAIL, BURR ON LINK OF CHAIN SLICED FINGERS.
EE HANDLING PIECE OF STEEL WEAR PLATE IN A CONVEYOR FEED CHUTE WHEN THE PLATE DROPPED FROM ITS POSITION AND CUT EE'S FINGER. CUT REQUIRED 5 SUTURES.
EE WAS CLEANING UP FINES SPILLAGE IN THE ROCK PLANT, EE RESULTED WITH A LOWER LEFT BACK STRAIN.
INJURED WORKER ATTEMPTED TO DISCONNECT AND REMOVE 2 30' SECTIONS OF 8" DIA ALLUMINUM IRRIGATION PIPING, TO ALLOW A DOZER TO PASS THROUGH AREA. ON THE SECOND SECTION OF PIPE, ONE OF THE JOINTS WERE STUCK. THE IW LIFTED THE FREE END, PLACED IT IN A HEADLOCK POSITION WITH HIS RIGHT ARM AND MOVED SIDE TO SIDE WHILE PUSHING AWAY FROM THE STUCK JOINT. SUFFERED A HERNIA.

EE WAS CHANGING A FITTING IN A ROCK CRUSHER OIL TANK. THE OUTER LID AND INNER LID WERE RAISED UP AND RESTING AGAINST THE BACK WALL. THE INNER LID WAS BUMPED BY ANOTHER LABORER AND THE LID FELL , STRIKING EE'S HAND THAT WAS POSITIONED ON THE EDGE OF THE TANK. EE RECEIVED SIC STITCHES IN THE MIDDLE OF HIS RIGHT PALM.

EMPLOYEE HAD LEFT SHOVELING DIRT, RETURNED AFTER LIFTING A HOT PLANT SCREEN AND BEGAN SHOVELING AGAIN. SHORTLY AFTER RETURNING TO SHOVELING HE FELT A STRAIN IN HIS NECK AND BACK.
EE WATCHING REMOVAL OF LIMBS FROM TREE. TRIMMING OF BUSHES AND TREES REQUIRED FOR INSTALLATION OF NEW PERIMETER FENCING. TREES HAD BEEN CUT AND WERE ON GROUND. FOREMAN EE WAS STANDING BY TREE TRUNK WATCHING THAT CUT LIMBS CAME FREE. AN OLD ABANDONED POWER POLE GUY WIRE WAS SNAGGED BY LIMBS PULLING THE RULES OVER CONTACTING FOREMAN EE.
INSTALLING CONVEYOR, CABLE SMASHED FINGER AGAINST METAL CUT AND BROKE LEFT RING FINGER.
EE SMASHED HIS RT. LITTLE FINGER WHILE REMOVING PULLEY GUARD. MOVING PARTS WERE LOCKED OUT AND TAGGED. EE SMASHED FINGER ON GUARD.

6 X 20 SCREEN DECK MOUNTING RUBBER SHOT OFF AT EMPLOYEE. HE PUT HIS HAND UP TO PROTECT HIMSELF AND IT HIT HIS RIGHT HAND MIDDLE FINGER.
WHILE SPINNING AN UNBELTED SHEAVE ATTACHED TO A SHAFT ON A GRIZZLY FEEDER, THE EMPLOYEE CAUGHT HIS ELBOW ON A PIECE OF STEEL, WHICH CAUSED HIM TO JAM & TWIST HIS WRIST ON THE SHEAVE. THIS RES ULTED IN HIM CHIPPING THE RADIAL BONE IN HIS ARM
