Showing all 50Employee entered feeder at wet plant to complete end of shift clean out, to manually unplug and remove materials build up. EE was shoveling in far corner of hopper when the material gave way. They lost footing and fell about one foot hurting their shoulder. EE stated that they regained themselves & started to climb out when a knee gave out and they fell again, about one foot.
Miner was assisting with changing a return roller when the roller slipped out of the bracket and pinched EE's right finger between the roller and the bracket, causing a laceration that required seven stitches.
Employee was assembling legs for conveyor belt on portable crusher. Leg fell out of EE's hands and struck EE's left foot and ankle. Contusion to left foot.
Miner was helping to change cutting edges on a loader. As cutting edge bolts were being removed, EE reflexively reached to catch a bolt that had spun free and received lacerations to hand from spinning bolt head.
Employee was working on a 10' 11/2" steel pipe on a table with a vise. The pipe rolled and dropped on the edge of the table crushing employee's left pinky finger. Laceration to left 5th finger.
On June 21, 2022 employee was pushing in a screen rail and felt a pain in left hand, resulting in a strain.
Miner was cleaning the floor with a water hose when they slipped and fell, striking their left elbow, resulting in a laceration requiring stitches.
Employee was lifting oxygen tanks and strained lower back.
On Feb 1 employee was installing a roller on a dozer, on Feb 2 EE was changing a battery in a semi. At end of day noticed pain and discomfort in abdomen. Umbilical hernia. On 2/21/2022 determined this occurred on an MSHA site.
Miner was working on repairs when a piece of metal slipped from EE's hand and lacerated forefinger on right hand.
Employee was removing a belly pan from a dozer. As employee removed the last bolt the pan fell on employee's right hand. Laceration and contusion to right hand required stitches.

putting things down on a railroad tie, other employee dropped crowbar on IW left hand resulting in a laceration. IW received 8 stitches
lifting a 2 gallon container of coolant onto a dozer and felt a strain in right upper arm shoulder
pulling dirt out from under a conveyor with a pipe and flat blade attached. the pole became buried in the dirt so the operator lifted up on the end which caused it to break loose and hit coworker in the forehead.

Cleaning material stuck in the bed of a dump truck. EE lifted the bed so the bed would stay open, connected the chains as an extra precaution and started cleaning the material. During the operation she bumped the chains causing the tailgate to dislodge and swing shut smashing fingers on both of her hands.
sliding a screen across catwalk. the screen had a sharp piece of metal from vibration of shaker and he pushed it with his foot causing a very small puncture wound to his foot
Changing a roller on a conveyor. EE was holding the roller as the bolts were removed. When the last bolt was removed the roller, and co worker working above partially fell onto his shoulder and arm.
EE was replacing a screen cleaning bar when a piece of cutting edge bounced onto his finger smashing and cutting it.
Employee was lifting a screen, and felt a pain in his groin area. Employee didn't seek medical attention until March 25th. Upon a medical examination, a hernia was discovered.
Two miners were lifting a roll of geo grid into a loader bucket. The roll slipped out of one miner's hands and caused the other miner to lose his grip and drop the roll of geo grid on his foot. The miner broke the big toe on his left foot.
While closing door of motor grader, smashed right thumb in door.
Miner was changing a v-belt on a cone screen. He was rolling the belt on the pulley and pinched his thumb between the belt and pulley. He had placed his thumb in the wrong location. As a result he received a laceration to his thumb. During the investigation it was found that LO/TO was being used as well as necessary PPE.
Miner was helping a mechanic finish repairs on a GenSet. He was installing the raincap on the exhaust stack. He was pushing down and wiggling the cap and the cap slipped on suddenly pinching the miners finger (middle) cutting the tip of it off.
Miner trying to free up rock stuck in machinery. Standing on material, material shifted when rock dislodged, his ankle smashed between couple of rocks. Reason for late filing, conflicting stories reported by miner during investigation. Not hospitalized-observation & time off. Before date of inj EE scheduled day off 6/25, 1 holiday 6/28.
Employee was pressing out a cone liner. Flanges broke & metal I-Beam flipped up & hit him in the jaw.

THE LEG ENTERED HIS BOOT JUST BEH IND HIS STEEL TOE PROTECTOR, STRIKING NEAR THE BASE OF HIS RIGHT BIG TOE. X-RAYS CONFIRMED A BREAK TO THE TOE.
WAS TYING AN AIRLINE BACK WITH TIE WIRE, LINE SLIPPED AND WIRE POKED HIM IN THE EYE. HAD REMOVED SAFETY GLASSES BECAUSE THEY WERE DUSTY.
HOLDING A PIECE OF STEEL TO PROTECT ADJACENT HYDRAULIC LINES FROM DRILL BIT, DRILL BOUND UP AND THE CORD WITH THE KEY CHUCK HIT HIM IN THE MOUTH CHIPPED A TOOTH.
WEARING SNOW TYPE BOOTS, DROPPED A BOTTLE OF ACETYLENE ON FOOT.

MOVING SCREENS, ONE SCREEN CAME A APRT AND HIT HIM BELOW THE EYE.
PUTTING V-BELT ON THE FEEDER MOTOR, MOTOR BEING SUPPORTED BY BAR WHICH SLIPPED AND INPACTED FINGER.
