Injured Person was replacing the classifier bearing assembly. The assembly became lodged due to alignment. The injured employee had hands underneath the assembly, attempting to correct the alignment. Subsequently, the assembly dislodged and fell onto the injured employee's hands. This incident resulted in a broken ring finger and lacerations to the ring and index fingers.

Were replacing a bad sprocket following troubleshooting of a gearbox. Removed the 4 bolts from the mounting plate, chain slipped and motor & gearbox fell. The falling equipment impacted the IW left pinky finger, requiring stitches, antibiotics and light duty.

The Operator was loading a truck out of 8 silo. EE attempted to hold the hose and ladder simultaneously. Employee shoe slipped and gate sprung back into its original position. This resulted in a strained shoulder. EE was given instruction to take over the counter medication, and not to lift anything over than 10lbs.

Employees were unclogging a baghouse and a hard hat fell into the screw at the bottom of the baghouse. The hard had was drawn into the screw and stopped at the rotary valve. They workers locked out the screw and valve and attempted to retrieve the hard had. When the hard hat came out, there was stored energy in the valve and it turned and caught the workers fingers.

Employee was called out to install a hose for 10 silo. The clamp failed and EE had to come back out and install it again. During the second installation the hose was found in the car, full of clay. Another Employee helped EE pull the hose out of the car and this was when the employee felt a pain in the lower stomach.

Maintenance employee directed the mixer operator to stand on a ladder and see if the valve was opening all the way prior to reassembling the pipe. Mixer operator put their middle finger into the pipe opening. The valve closed on the tip of the finger. The impact caused an avulsion of the workers left hand middle fingertip.

While installing the bucket on a skid steer, the employee could not close the latch easily. EE utilized a pipe to gain leverage on the latch and was able to close it. After this, the pipe was jammed on the latch handle. When EE pulled the pipe from the latch, EE smashed EE's left thumb on the skid steers frame. The thumb has a laceration and an open fracture of the distal phalanx.

Employee states was bagging 50 lb paper bags, on the 8th row of the pallet when rotating to stack the bag when EE felt pain. The pain felt to have come from EE's back, to neck, to shoulder. Management wasn't aware that it was work related until employee brought a doctor's note on 7/1/24.

EE was installing a new 4 wide drive belt on the drive belt on the Dryer Air Supply Fan for the Furnace. Belt was on driven pulley and they were rotating non driven pulley to install belt. When the belt jumped into place EE finger was between the belt and pulley.

A maintenance supervisor was walking up the Kiln 4 upper product belt carrying a piece of chute to be installed and EE's right forearm contacted a piece of expanded metal which was in close proximity to the catwalk. This contact caused a laceration which required seven staples to close.

Employee was shoveling approximately .5 inches of snow off of sidewalk. As EE turned to throw a scoop of snow to the side, EE felt pain in EE's lower back and dropped to EE's knees. Doctor labeled lower back injury as either a pulled muscle or pinched nerve.

We were dumping the grinding media balls (metal balls) out of the CF1 Ball Mill into barriers (constructed of 4x8 plywood, etc). One of the balls bounced out of the barriers, bounced up and hit a supervisor in the forehead. It caused a laceration that required 5 stitches.

On 04/07/2021, an Imerys employee approached me at 8:00AM in my office, in reference to a possible shoulder injury EE sustained several weeks ago. EE stated was restacking a pallet at the Lapp Warehouse when EE felt a pain in the shoulder. EE had surgery on 7/08/2021 to repair torn rotator cuff.

Operator of CAT 745 Articulated Truck stopped to refuel. They placed the nozzle into the refueling port. They turned away and left the nozzle unattended. The fuel nozzle slipped out of the port and sprayed diesel fuel on the fender and steps. Operator lost footing and fell from second step from the ground.

EE was replacing a hose at the B-Tank manifold. After they had fabricated a new hose, they arrived at the manifold and disconnected the damaged hose. When EE went to pick up the hose, one of the ears on the camlock end got caught in the deck grating. EE proceeded to bend down to free it, at which time EE tweaked EE's back.

The employee was working on a hub assembly of a cage mill. While trying to turn the hub and shaft employee's hand slipped. The rough worn surface had a sharp edge that cut through employee's glove causing a laceration on the palm near right thumb. It required 4 stitches with no restrictions. The employee removed own stitches on 5-25-2020.

While employee was picking up the 50 lbs bags off the floor to stack them on the pallet the employee complained of pain at the back of shoulder. The employee was diagnosed with a muscle strain and have a restriction of a 20 lbs lifting capacity.

On April 21, A Bulk Loader bumped elbow on safety railing while putting on harness, it resulted in swelling to the right elbow. EE was seen by a doctor on May 4th, EE was given a restriction of lifting no more than 20 lbs. Therefore it became reportable on May 4th.

Two employees thought they had a piece of pipe secured when the pipe started to roll off of the compressor. Once employee reached out to stop the pipe from rolling and did keep it from falling off of the compressor. The next day the employee woke up with a sore shoulder and suspected that EE had aggravated an old injury.

Employee pulled a groin muscle trying to adjust a bulk bag on a pallet as EE was beginning to fill. EE felt a sharp pain. EE was taken to the local ER where EE was diagnosed with a strained left groin. EE has been placed on light duty for one week.

IW was beginning to disassemble floating gearbox for a screw conveyor. The IW removed guarding and was in the process of removing the drive belts between the motor and gearbox. As the employee loosened the belt tension bolts, the gearbox and motor fell. As it fell, the employee's right hand and arm were pulled downward, straining the employees right shoulder.

Miner was attempting to set up a slurry railcar load out. Employee stated when EE released the loading hose from its stored position it swung into EE. A 6" victaulic clamp that had a bolt protruding, struck EE in the RH inside elbow puncturing the skin and nicking an artery.

Temporary Employee got dust in left eye. Rinsed at eyewash station, continued to work. Reported to work the next day with eye still irritated. The employee had shoveled up dust into a bucket and upon dumping the bucket into a larger hopper, got dust into the eye. EE was wearing safety glasses but not safety goggles.

Employee was lifting bags of off spec material into a dumpster and felt a pop in the neck. EE's neck started to get stiff and the pain started to increase. EE contacted the supervisor, went to the clinic, and was diagnosed with a cervical strain. EE was placed on restricted duty for 10 days with a potential to be released to full duty after a follow-up visit.
