Struck by flying object

Cut off wheel on grinder exploded and a piece of the disk went into the hand through the glove. Required 3 stitches .

EE was opening a discharge valve on Silo 17 when the right pinky finger struck the handle on an access door resulting in a fracture of the pinky finger.

Fall to the walkway or working surface

On 4/20/2017 an employee was walking on a sidewalk on the ports property and misstepped and fell. On 5/26/2017 the employee reported they were having discomfort with the knee and went to the doctor. On 8/10/2017 the doctor determined that the discomfort was caused by the fall. Physical Therapy has been prescribed. This did not happen at the Mine site.

This accident did not happen on the mine site. Contributing Factors were: Rough walking surface (Grating) Worker stumbled on grating while desending railcar. Injuries: 3 Fractures on right foot. Involved railcar NAHX 60563
