Employee struck hand while attempting to loosen nuts to adjust belt. Employee rendered first aid on day of incident however did not request medical attention until 5/10/22. X-Rays revealed broken left middle finger. 7000-01 filed on same day of X-Ray results on 5/10/22.
EE was removing a pipe from a fitting with a 4ft. pipe wrench. When EE picked the wrench up to reset the pipe wrench to pull on it again when a finger got caught between the top of the wrench and the hydraulic tank frame which resulted in a laceration to right pinky, requiring stitches.
Employee stated they were cutting a slit in a pipe, the knife broke causing employee to lacerate left hand. Requiring 10 stitiches
Employee was splicing conveyor belt with proper splicing equipment. As the belt was being tightened with comealongs the clamp became wedged against a trough roll. It was not noticed that it was stuck. As the belt was pulled tighter it finally let loose and slid off the end of the clamping bar struck him in the right shin.
Employee was using a 1/2" drive ratchet with a 5/8" socket to loosen bolt, bolt released unexpectedly causing employee to come in contact with a sharp corner edge of screen counter weight, thus cutting backside of right hand.
EE Was in the process of removing wedges from the upper liner (Gyro Crusher). During the process while swinging a sledge hammer he struck a handle of another hammer. The hammer became airborne striking him in the head.
while prying screen panel loose to move it for lifting, pry bar slipped out of his hands while under tension striking his right elbow.
Employee was cutting belt with bad splice with left hand due to his Right hand being broken (none work injury). Knife slipped while trimming belt and struck him in his right leg causing a deep cut
Employee was adjusting the jaw crusher w/4' rachet, rachet slipped off nut. Employee fell on top of loader bucket, hitting his leg on the gusset on top of the bucket. At the time of the accident the loader bucket was down on the ground and shut off, bucket was tilted forward w/material behind bucket so it wouldn't move.
EMPLOYEE WAS PREPARING A CONVEYER TO BE MOVED. CONVEYER BELTING HAD TO BE CUT. WHILE CUTTING BELT WITH A UTILITY KNIFE, KNIFE SLIPPED CUTTING EMPLOYEE'S LEFT HAND, IN WEB OF THUMB AND FOREFI NGER.
