Employee was cutting a 2 inch hose and punctured their leg with a utility knife.

Employee was using utility knife to cut and remove the nylon strap around a box; as employee is cutting the strap, EE's hand slipped causing the knife to cut into EE's right forearm.

Hit with sharp point of rock hammer on the left hand.

EE was pulling on an 1 1/8" wrench & he strained his right shoulder. ** This did not become reportable until 10/12/2012 when information was received about physical therapy **

Small piece of rock hammer chipped off and struck employee in right abdomen.

After driving a pry bar into the packing gland of a water pump to loosen the packing, the employee tapped the pry bar to remove it and the bar sprang out and hit him in the forehead.

Employee was dislodging a broken return roller from a pulley on a de-energized conveyor. Pre-job plan, lock out/tag, and release of the hydraulic take up were conducted prior to start. When the broken return roller was cut free from the pulley the unanticipated stored energy wedged his right leg in the conveyor pulley.

EE and another EE were working on the Bizmark, replacing a canoe liner at the head pulley of the feeder belt. Injured EE was on his knees on the belt, when he leaned forward to pick up a socket from his tool bag and felt a sharp pain in his back.

Employee was removing liners with a pry bar in the rodmill. The liner was in the 10 O'clock position. When it broke loose it fell to the bottom of the barrel at approximately the 4 O'clock position and hit employee on the foot above the protective toe of his boot.

Employee was cleaning out a screen and hit his finger with a hammer

Employee was cutting 48" belt out from under the pit feeder in aspen 3 south. EE was in a tight area with limited mobility. He was using razor knife when it slipped and cut his right forefinger.

Employee was working on the dust control sandvik crusher deck. He attempted to pull on belt to line up bolt. While tightening up the fitting with a wrench, he strained his lower back. He returned to work on 3/16/06 and has worked every day since until 6/16/06. He reported on 6/15/06 to the office, that he has been going to physical therapy since 6/7/06.

Using a sledge hammer to remove liner from crusher employee got piece of metal in left eye. Employee didn't feel anything in eye that night but reported injury next day. Foreman took employee to Sutter Health. Then referred to specialist who removed object. Employee released back to work with no restrictions.

WHILE CUTTING INSULATION @ THE SHOP CRUSHER PAD, EMPLOYEE WAS BENT OVER (ON KNEES), REACHING OUT & SOMETHING POPPED IN HIS BACK.

The ee was cutting down a urathane rubber screen when the knife slipped cutting his right hand below his thumb.

EE WAS CUTTING SKIRT RUBBER. THE BLADE SLIPPED, CUTTING EE'S UPPER RIGHT THIGH.

EMPLOYEE WAS CUTTING A HOLE IN A RUBBER BELT, HIS HAND SLIPPED CAUSING HIM TO STAB HIMSELF IN THE LEFT THIGH WITH A BELT KNIFE.

THE EE WAS USING A COM-A-LONG TO PULL A MOTOR FORWARD, IT BROKE OFF THE LIFTING EYE CAUSING HIM TO FALL INTO A SCREEN.

WHILE CUTTING A PIECE OF RUBBER W/ A RAZOR KNIFE, EE CUT ACROSS THE PALM OF HIS RT. HAND & WAS NOT WEARING GLOVES.

THE EMPLOYEE WAS USING A HAMMER TO BEND THE RETURN ROLL BRACKET WHEN HE HIT HIS FINGER WITH THE HAMMER.

EE WAS SHOVELING UNDER TAIL PULLEY ON CONV. #1. HE WAS DISTRACTED, THE SHOVEL CAUGHT THE TAIL PULLEY, HIS HAND WAS SLAPPED BETWEEN HANDLE AND CONCRETE.

WHILE INSTALLING A GUARD, EE HIT HIS LEFT ELBOW ON A UNISTRUT THAT WAS HANGING DOWN.

AS THE EE WAS CHIPPING ASPHALT OFF THE MIXER BARA PIECE OF METAL FLEW INTO HIS RIGHT CHEEK.

EMPLOYEE WAS CHIPPING MUD OUT OF TRANSFER BOX WITH A ROCK HAMMER WHEN A PIECE OF DIRT GOT BEHIND HIS SAFETY GLASSES AND INTO HIS LEFT EYE.

EE WAS DOING MAINTENANCE AND REPAIRS ON THE DRAGLINE DRL0001. HE WAS PULLING ON A WRENCH WHICK SLIPPED OFF AND HIT HIS LIP.

WHILE SITTING ON A 16" PIPE EMPLOYEE WAS TRIMMING BELTING TO FORM A SEAL AROUND PIPE. AS HE WAS MAKING HIS LAST CUT THE UTILITY KNIFE CAME THROUGH THE BELT EASIER AND FASTER THAN HE WAS EXPECT ING AND WENT ACROSS HIS RIGHT LEG CUTTING IT OPEN ABOUT 3".

AS EMPLOYEE WAS LOWERING SCREEN FROM DECK TO GROUND, THE SCREEN SWAYED, PINNING THE RIGHT INDEX FINGER BETWEEN THE SCREEN FRAME AND THE HAND RAIL. THE FINGER WAS SMASHED AND THE FINGERNAIL WAS TORN OFF.

BARRING DOWN ON BUILD-UP ON THE INSIDE OF A CONE CRUSHER MAIN FRAME. LARGE PIECE DISLODGED LEVERAGING BAR INTO THE RIB CAGE.

WHILE PRYING OPEN FEEDER DOOR, A BIG ROCK CAME OFF OF THE TOP AND STRUCK EE'S RIGHT WRIST.

EMPLOYEE WAS USING A HAMMER AND PUNCH TO REMOVE A PIN FROM CONVEYER ROLLER. HE MISSED THE PUNCH AND HIT HIS RIGHT THUMB CAUSING A FRACTURE AND LACERATION.

ATTEMPTING TO LOOSEN BOLT A V TRAP STRAINER, BOLT RELEASED, SOCKET SLIPPED OFF BOLT HEAD. RIGHT ELBOW STRUCK SCREEN CHUTE. INFECTION SET IN WITH NO EVIDENCE OF SKIN RUPTURE.

EE WAS USING A 5' LONG DIGGING BAR WHILE CHANGING A CONVEYOR RETURN IDLER BRACKET. THE BAR SLIPPED AND PUNCTURED HIS LEFT HAND AT THE BAS OF THUMB.

THE EE WAS CUTTING A BELT WITH A UTILITY KNIFE WHEN IT SLIPPED AND CUT INTO RIGHT LEG.
