Employee was using utility knife to cut and remove the nylon strap around a box; as employee is cutting the strap, EE's hand slipped causing the knife to cut into EE's right forearm.

Struck by flying object

Employee setting up backhoe to begin clearing area near tunnel when EE felt discomfort in left eye. Employee continued to work for some time before attempting to remove object with eye wash. When object did not come out, employee was taken to clinic where metal shard was removed.

After taking break miner was walking to gather tools to complete a housekeeping project. Miner slid left hand into pocket (while wearing gloves), where open pocket knife cut the top of miner's hand just above the thumb.

Struck against a moving object

A scraper operator was headed eastbound through the bottom of the pit when the back end of the scraper lost traction. The scraper spun around and eventually grabbed traction as it slowed down. Momentum caused the scraper to tip onto it's side.

While replacing bin pulley assembly, employee was removing a bolt with a 1/2 impact and a 15\16 ratcheted box end wrench. The wrench spun, trapping finger between the wrench and the bracing resulting in the loss of the tip of left ring finger.

Struck by falling object

While removing the retarder on a 633 mega scraper the retarder valve fell and struck the employee in the eye area. The employee was wearing safety glasses at the time of the incident. The angle of the falling valve knocked the glasses off and the employee got cut by the sharp metal of the valve. Employee sustained two lacerations on outside of right eyelid.

Employee was exiting skid steer and employee was standing on the right tire. When employee stepped down with right foot, employee's left foot got hung up in the tire tread causing pain in the left knee.

The employee's job is to grease and lube moving parts at the rock plant on a daily basis. This is a repetitive motion type of injury with a cumulative affect.

Fall from machine

Employee's loader wouldn't start so he climbed down to check the battery box and move the cables. He then decided to climb back up on the loader using only the hand rail next to the engine compartment. He lost his grip and fell on his back. The operator didn't report the incident to management until June 17th, 2011.

After driving a pry bar into the packing gland of a water pump to loosen the packing, the employee tapped the pry bar to remove it and the bar sprang out and hit him in the forehead.

Employee was cleaning inside of driveover feeder at Thoming pit using a demolition hammer. While he was cleaning the side walls he strained his right elbow and arm. His fingers began to go numb. The employee reported it to his supervisor but said that he did not require medical treatment.

Employee was removing liners with a pry bar in the rodmill. The liner was in the 10 O'clock position. When it broke loose it fell to the bottom of the barrel at approximately the 4 O'clock position and hit employee on the foot above the protective toe of his boot.

While moving a piece of steel by hand in the steel yard, employee twisted his back.
