WHILE CUTTING INSULATION @ THE SHOP CRUSHER PAD, EMPLOYEE WAS BENT OVER (ON KNEES), REACHING OUT & SOMETHING POPPED IN HIS BACK.

While stepping onto the pit conveyor switchgear skid platform (6"" platform) felt left knee pop.

EE WAS LOADING BAGS INTO THE BED OF PICK UP TRUCK. HE WAS LIFTING AND TWISTING TO SET 60-70 LB. BAGS.

WHILE CLIMBING UP STOCKPILE, EE LOST HIS BALANCE WHEN MATERIAL UNDER HIS FEET GAVE WAY. THIS CAUSED HIM TO FALL AND TWIST HIS RIGHT KNEE.
