Employee suffered a laceration to EE's left forearm by a moving machine part.

Employee was recycling an iron plate out of work vehicle. The plate snagged left glove as they were tossing the plate into the bin, and came down on the tip of left middle finger, fracturing it.

The employee was leaning to pick up a water bottle and hit a bump while operating the scraper. After hitting the bump the seat bounced causing EE to bounce up and cut EE's ear on a GPS bracket.

Accident type, without injuries

A SMUD transformer exploded causing a grass fire near the mine site in the Aerojet easement. The fire was never actually on the mine site but called in for reporting purposes.

Cut hand on roller that wasn't machined smooth, had a razor sharp edging. Cut required 6 stitches.

Struck by falling object

Conditions were sunny and clear. Employee was doing repairs to West ""P2"" Conveyor when he reached on to the frame and at same time a large cobble fell off the belt onto his right ring finger. Employee heard a popping noise and right ring finger began to bleed.

Employee was doing repairs in outside in the Plant during hot weather conditions.

Weather and surrounding conditions were clear. The employee at the time of injury was changing out roller on conveyor #16 and #18 which required bending and lifting.
