The miner was securing a belt clamp when it slipped and hit the miner in the chin.

Removing 2X2 ceramic liners from the discharge box of #12 screen. I grabbed the heavy liner and set it outside the box on the catwalk. I realized it had serrated edge. Cut the palm on left hand.

EE was laying on the screening media when working. Upon sitting up, ee felt a sharp stabbing pain in lower right abdomen. EE was replacing both the upper and lower motors on pre wash.

Finger in between lifting cable and crusher.

Trommel caught on fire after welding operations where being performed on the outside of it. Employees left for lunch at 6:00PM and returned at 6:30 PM and trammel was on fire. Fire department was called and fire extinguished around 10:30PM.

Hit with sharp point of rock hammer on the left hand.

Approaching switch gear to lock out conveyor, wasp flew under safety glasses and stung the left eye.

Employee experienced lower back pain while installing screen media panels on a new trommel.

Removing and installing roller on beltline. Hit elbow on frame.

Replacement of media screen. J bolt in middle of screen broke loose snapping back striking him in the mouth.

Hearing loss

Employee reported stepping off of a conveyor frame losing balance and striking his lower back against a hose hanger bracket mounted on the wall of # 6 tunnel. Employee was diagnosed with a low back contusion/sciatica.

Running a scraper when the seat bounced going through cut. Felt the seat cushion slam down.

Small piece of rock hammer chipped off and struck employee in right abdomen.

EE was giving tickets to drivers at the time. He reports being struck by a rock in thigh and knocked to the ground.

Employee reported cumulative trama to back from operating equipment for many years.

Dark outside and shadows walking to #2 conv. Twisted left ankle.

Eye problems

His right eye starting feeling itchy. He went to wipe it and something went into his eye.

EE was walking from breakroon, tripped over box, hit head on support column.

While switching a valve, strained lower back.

Employee was dislodging a broken return roller from a pulley on a de-energized conveyor. Pre-job plan, lock out/tag, and release of the hydraulic take up were conducted prior to start. When the broken return roller was cut free from the pulley the unanticipated stored energy wedged his right leg in the conveyor pulley.

EE and another EE were working on the Bizmark, replacing a canoe liner at the head pulley of the feeder belt. Injured EE was on his knees on the belt, when he leaned forward to pick up a socket from his tool bag and felt a sharp pain in his back.

EE was cleaning mud off the skid steer tractor. He was standing on the step and was cleaning the top of machine. He slipped off and tried to catch himself; he reached and grabbed the frame. He fell between the cab and bucket of the machine. The fingers of his right hand were cut which required stitches. He also sustained an abrasion on the back of the right leg.

Employee and co-worker were working together on the Bizmark. Repairing a damaged apron on the drive side of the feeder. EE was on the ground below his co-worker who was removing the last 4 bolts. As the angle iron brace fell it deflected out towards EE striking him in the face. He was wearing safety glasses and required 10 stitches.

Employee suffered neck strain while checking a motor on the hydraulic system of the # 3 shorthead crusher. He was moving the cover built over the hydraulic system to protect against falling rocks. After completing the job he felt neck strain.

Employee was walking from shop area on his way to the plant when he stepped on uneven ground at the base of the scalper stairway. He felt his right knee pop. He did not fall and he was not carrying anything at the time of injury.

EE was working inside main frame of crusher to remove build up, He slipped and as he lunged forward his left hand made contact with the sharp edge of the bronze socket liner cutting his left hand requiring 9 stitches.

Employee strained his right Achilles tendon while walking up a 6' embankment to check a pond. He slipped back slightly and felt pull in his right heel. He was evaluated May 21, 2007 and the sprain to his right foot had not improved. The doctor prescribed physical therapy. No restrictions.

EE was repairing wiper on the discharge end of #4 conveyor. He was positioned in the discharge chute. He stood from crouched position and struck the center of his back on the diverter chute. He reported to foreman he was sore but did not need medical attention. Foreman noted incident.

EE came into the office on Dec 11, 2006 and reported a sore back and that he would not be able to work. He stated he had pain since 11/11/06 and thought it was due to a cough from being sick. He also stated he had experienced some jolting while drivng in the yard.

Employee bumped his elbow and felt pain. He noticed a small red bump and squeezed it and a piece of metal came out. He went to the hospital and was released. He returned due to pain and the doctor admitted him to the hospital. He remembered he had bumped it two weeks prior. It was not known if the infection was from a work related injury, but we are treating it as a work comp.

Employee was holding pipe to free bunker. As he was prying gate, the pipe slipped from its hold and employee's finger was caught between pipe and catwalk lacerating his finger.

Employee was installing catwalk to pond pump. He was using 4"" grinder with 7"" cutting wheel modify handrails and toe boards to fit. As he was cutting, the cutting wheel bound up and grinder kicked back causing 1"" cut.

Employee was cutting 48"" belt out from under the pit feeder in aspen 3 south. EE was in a tight area with limited mobility. He was using razor knife when it slipped and cut his right forefinger.

Laborer on the graveyard shift injured his right pinky finger when it was caught between two 55 gallon drums. He was helping fellow employee change oil in the crusher when he attempted to manually move the full 55 gallon drum next to another drum, as he was setting the drum into place his pinky finger was caught between them.

Pulled a muscle in his lower back wile shoveling. He was shoveling cobbles from the #4 tunnel ramp. He was standing on the uphill side of the ramp facing down the incline shoveling rock from the walkway. He felt a pop in his back and dropped to his knees.

He was walking on the crusher deck when he slipped on a water hose and fell, hitting his chest on the feed tube.

Aggravation of an old injury that occuried on 3/24/00. Faulty rung on ladder gave way under employee's weight, causing the employee to fall through the ladder. He received a bruise to this lower left leg.

He stepped wrong and fell hitting conveyor frame work

HE WAS DRIVING WITH HIS HAND ON THE SUICIDE KNOB & AS HE WAS TURNING HE HIT A BUMP AND HIS SHOULDER POPPED.

EE WAS OPERATING A 637 SCRAPER. A DOZER WAS PUSHING THE SCRAPER THROUGH THE SLICK. THE DOZER HIT THE SCRAPER WITH MODERATE FORCE CAUSING A JOLT TO THE SCRAPER OPERATOR.

EMPLOYEE WAS OPERATING A 637 SCRAPER AT THE FRONT OF A 2 SCRAPER TANDEM. THE REAR SCRAPER STRUCK EE'S AT A MODERATE RATE OF SPEED. EE WAS FACING FORWARD & LOOKING DOWN AT THE TIME OF IMPACT, CAUSING HIS HEAD TO WHIP BACK.

AS THE EMPLOYEE PICKED UP A STEPLADDER THAT WAS LAYING ON A STACK OF 8"" X 8"" TIMBERS TO USE ON A CRUSHER, THE TIMBER ROLLED OFF A CONCRETE STEP AND LANDED ON HIS LEFT FOOT.

THE EMPLOYEE WAS STEPPING OUT OF THE WATER TRUCK TO FILL UP THE TANK. AS HE WAS STEPPING OFF THE BOTTOM STEP, HIS FOOT SLIPPED AND HE FELL TO THE GROUND.

He strained his lower back while attempting to remove a large cobble from the 5 1/2' standard crusher. It was an aggravation of an old injury that he sustained prior to his employment with Teichert.

Employee pulled a muscle in his lower back while shoveling in a tunnel.

THE EMPLOYEE WAS INSTALLING GRATING UNDER THE #1 SHORTHEAD CRUSHER. WHILE ATTEMPTING TO MANUEVER THE GRATING INTO PLACE, THE GRATING PINNED HIS HAND BETWEEN THE DECKING & THE GRATING CAUSING A CUT TO HIS RIGHT PALM APPROX. 1 & 1/2""LONG.

AS THE EMPLOYEE WAS BLOWING OUT BOLT HOLES WITH AN AIR WAND A PIECE OF DIRT OR ROCK FLEW INTO HIS RIGHT EYE.

The ee was cutting down a urathane rubber screen when the knife slipped cutting his right hand below his thumb.

THE EMPLOYEE PULLED A MUSCLE IN HIS LOWER BACK WHILE SHOVELING IN A TUNNEL.

THE EE WAS USING A COM-A-LONG TO PULL A MOTOR FORWARD, IT BROKE OFF THE LIFTING EYE CAUSING HIM TO FALL INTO A SCREEN.

INJURY OCCURRED WHILE DRIVING EUCLID STOCKPILE TRUCK AT PLANT.

THE EMPLOYEE WAS USING A HAMMER TO BEND THE RETURN ROLL BRACKET WHEN HE HIT HIS FINGER WITH THE HAMMER.

A TOOL BAG FELL OFF THE CATWALK AND HIT THE EE IN THE SIDE.

AS THE EE WAS CHIPPING ASPHALT OFF THE MIXER BARA PIECE OF METAL FLEW INTO HIS RIGHT CHEEK.

AS THE EMPLOYEE WAS LIFTING TOOLS UP TO THE CONVEYOR BELT TO DO A CUT & SPLICE HE PULLED A MUSCLE IN HIS BACK.

AS THE LOADER LOADED MATERIAL INTO THE TRUCK, THE LOADER BUCKET DROPPED STRIKING THE TRUCK.

EMPLOYEE WAS CHIPPING MUD OUT OF TRANSFER BOX WITH A ROCK HAMMER WHEN A PIECE OF DIRT GOT BEHIND HIS SAFETY GLASSES AND INTO HIS LEFT EYE.

EE WAS RAISING A BOTTLE OF OXYGEN THAT WAS IN A SAFETY CAGE TO HIS WORK DECK. HE WENT TO LET THE CAGE DOWN & THE CAGE HIT THE HANDRAIL, FLIPPED OFF THE HOOK & HIT HIM IN THE HEAD, KNOCKING HIM BACKWARDS.

WAS SHOVELING OUT CONVEYOR #30 TUNNEL. KNEE BECAME SORE BUT DID NOT REPORT UNTIL JANUARY 16, 2002.

AS EE PULLED TONGUE OF PORTABLE WELDER DOWN TO TRAILER HITCH, THE WEIGHT SHIFTED, BRINGING THE TONGUE DOWN TO THE HITCH FORCEFULLY, CRUSHING EE S FINGER.

AS EE STEPPED BACK AND DOWN FROM PULLEY HOUSING, HE LANDED HEAVILY WITH LEFT FOOT ON 1/4"" BY 3"" BOLT THAT WAS WELDED TO THE DECK. THE BOLT PENETRATED HIS BOOT AND PUNCHTURED HIS FOOT.

BREEZY DAY.

AS THE LOADER OPERATOR LOADED MATERIAL INTO THE TRUCK, THE LOADER BUCKET HIT THE CAB SHIELD, JOLTING THE TRUCK. THE DRIVER OF THE TRUCK WAS JOLTED INSIDE THE CAB, RESULTING IN NECK AND BACK ST RAINS.

AS EMPLOYEE WAS LOWERING SCREEN FROM DECK TO GROUND, THE SCREEN SWAYED, PINNING THE RIGHT INDEX FINGER BETWEEN THE SCREEN FRAME AND THE HAND RAIL. THE FINGER WAS SMASHED AND THE FINGERNAIL WAS TORN OFF.

EMPLOYEE WAS USING A HAMMER AND PUNCH TO REMOVE A PIN FROM CONVEYER ROLLER. HE MISSED THE PUNCH AND HIT HIS RIGHT THUMB CAUSING A FRACTURE AND LACERATION.

EE WAS USING A 5' LONG DIGGING BAR WHILE CHANGING A CONVEYOR RETURN IDLER BRACKET. THE BAR SLIPPED AND PUNCTURED HIS LEFT HAND AT THE BAS OF THUMB.

EE WAS USING AN OVERHEAD HOIST TO TURN OVER A SCREEN DECK FRAME. THE ROD HOLDING EYE BROKE ON THE SCREEN FRAM, DROPPING THE FRAME ON THE EE'S LEFT FOOT BEHIND THE STEEL TOE AREA.

A EE WAS REMOVING A LARGE ROCK FROM INSIDE OF BUNKER, HE SLIPPED, CAUSING THE ROCK TO SMASH HIS THUMB AGAINST THE BUNKER WALL, RESULTING IN A BRUISED RIGHT THUMB.\\

FAULTY RUNG ON LADDER GAVE WAY UNDER EE'S WEIGHT, CAUSING THE EE TO FALL THROUGH THE LADDER. HE RECEIVED A BRUISE TO HIS LOWER LEFT LEG.

ONE TRUCK WAS LEAVING STOCKPILE AREA, EMERGED FROM BEHIND A ROCKPILE AS A LOADED TRUCK APPROACHED AT DUSK. EMPLOYEE RECEIVED MINOR SCRATCHES AND BRUISES AND WAS SHAKEN UP WHEN TRUCKS COLLID ED.
