Removing 2X2 ceramic liners from the discharge box of #12 screen. I grabbed the heavy liner and set it outside the box on the catwalk. I realized it had serrated edge. Cut the palm on left hand.

EE was laying on the screening media when working. Upon sitting up, ee felt a sharp stabbing pain in lower right abdomen. EE was replacing both the upper and lower motors on pre wash.

Accident type, without injuries

Trommel caught on fire after welding operations where being performed on the outside of it. Employees left for lunch at 6:00PM and returned at 6:30 PM and trammel was on fire. Fire department was called and fire extinguished around 10:30PM.

Employee was dislodging a broken return roller from a pulley on a de-energized conveyor. Pre-job plan, lock out/tag, and release of the hydraulic take up were conducted prior to start. When the broken return roller was cut free from the pulley the unanticipated stored energy wedged his right leg in the conveyor pulley.

EE and another EE were working on the Bizmark, replacing a canoe liner at the head pulley of the feeder belt. Injured EE was on his knees on the belt, when he leaned forward to pick up a socket from his tool bag and felt a sharp pain in his back.

EE was cleaning mud off the skid steer tractor. He was standing on the step and was cleaning the top of machine. He slipped off and tried to catch himself; he reached and grabbed the frame. He fell between the cab and bucket of the machine. The fingers of his right hand were cut which required stitches. He also sustained an abrasion on the back of the right leg.

Employee and co-worker were working together on the Bizmark. Repairing a damaged apron on the drive side of the feeder. EE was on the ground below his co-worker who was removing the last 4 bolts. As the angle iron brace fell it deflected out towards EE striking him in the face. He was wearing safety glasses and required 10 stitches.

Employee suffered neck strain while checking a motor on the hydraulic system of the # 3 shorthead crusher. He was moving the cover built over the hydraulic system to protect against falling rocks. After completing the job he felt neck strain.

Employee was walking from shop area on his way to the plant when he stepped on uneven ground at the base of the scalper stairway. He felt his right knee pop. He did not fall and he was not carrying anything at the time of injury.

EE was working inside main frame of crusher to remove build up, He slipped and as he lunged forward his left hand made contact with the sharp edge of the bronze socket liner cutting his left hand requiring 9 stitches.

Employee strained his right Achilles tendon while walking up a 6' embankment to check a pond. He slipped back slightly and felt pull in his right heel. He was evaluated May 21, 2007 and the sprain to his right foot had not improved. The doctor prescribed physical therapy. No restrictions.

EE was repairing wiper on the discharge end of #4 conveyor. He was positioned in the discharge chute. He stood from crouched position and struck the center of his back on the diverter chute. He reported to foreman he was sore but did not need medical attention. Foreman noted incident.

EE came into the office on Dec 11, 2006 and reported a sore back and that he would not be able to work. He stated he had pain since 11/11/06 and thought it was due to a cough from being sick. He also stated he had experienced some jolting while drivng in the yard.

Employee bumped his elbow and felt pain. He noticed a small red bump and squeezed it and a piece of metal came out. He went to the hospital and was released. He returned due to pain and the doctor admitted him to the hospital. He remembered he had bumped it two weeks prior. It was not known if the infection was from a work related injury, but we are treating it as a work comp.

Employee was holding pipe to free bunker. As he was prying gate, the pipe slipped from its hold and employee's finger was caught between pipe and catwalk lacerating his finger.

Employee was installing catwalk to pond pump. He was using 4"" grinder with 7"" cutting wheel modify handrails and toe boards to fit. As he was cutting, the cutting wheel bound up and grinder kicked back causing 1"" cut.

Employee was cutting 48"" belt out from under the pit feeder in aspen 3 south. EE was in a tight area with limited mobility. He was using razor knife when it slipped and cut his right forefinger.

Laborer on the graveyard shift injured his right pinky finger when it was caught between two 55 gallon drums. He was helping fellow employee change oil in the crusher when he attempted to manually move the full 55 gallon drum next to another drum, as he was setting the drum into place his pinky finger was caught between them.

Pulled a muscle in his lower back wile shoveling. He was shoveling cobbles from the #4 tunnel ramp. He was standing on the uphill side of the ramp facing down the incline shoveling rock from the walkway. He felt a pop in his back and dropped to his knees.

AS THE EMPLOYEE PICKED UP A STEPLADDER THAT WAS LAYING ON A STACK OF 8"" X 8"" TIMBERS TO USE ON A CRUSHER, THE TIMBER ROLLED OFF A CONCRETE STEP AND LANDED ON HIS LEFT FOOT.
