Employee was working on a belt, was holding on to a piece of Angle iron, the angle iron came loose, and the employee fell approx. 6 feet, employee suffered a small cut on EE's right forearm and was taken to the Urgent care. Employee was returned to duty same morning

Employee was recycling an iron plate out of work vehicle. The plate snagged left glove as they were tossing the plate into the bin, and came down on the tip of left middle finger, fracturing it.
Left foot injury.
Employee was cutting a 2 inch hose and punctured their leg with a utility knife.
Miner used a front-end loader to carve a path into stacked material, then exited the loader and entered the newly-carved path on foot. The newly-created unstable wall of the material collapsed and covered the miner, causing a fatality.
Tripping incident
Employee was using utility knife to cut and remove the nylon strap around a box; as employee is cutting the strap, EE's hand slipped causing the knife to cut into EE's right forearm.

Employee was stung by a bee while traveling in an open cab utility vehicle. Employee is allergic and administered personal epi pen. Employee follow up with doctor and was required to be off work.
Slag fell on employee's left pant leg that caused burn to left shin
The employee was leaning to pick up a water bottle and hit a bump while operating the scraper. After hitting the bump the seat bounced causing EE to bounce up and cut EE's ear on a GPS bracket.
Employee setting up backhoe to begin clearing area near tunnel when EE felt discomfort in left eye. Employee continued to work for some time before attempting to remove object with eye wash. When object did not come out, employee was taken to clinic where metal shard was removed.
The miner was securing a belt clamp when it slipped and hit the miner in the chin.
After taking break miner was walking to gather tools to complete a housekeeping project. Miner slid left hand into pocket (while wearing gloves), where open pocket knife cut the top of miner's hand just above the thumb.
A scraper operator was headed eastbound through the bottom of the pit when the back end of the scraper lost traction. The scraper spun around and eventually grabbed traction as it slowed down. Momentum caused the scraper to tip onto it's side.
While a 385 Excavator was loading a 745 CAT Haul truck, a rock est. 500-1000 pounds rolled over the truck bed. The passenger side hand rail, a stair tread, and a hose bracket were bent. No damage to the operator cab. Employee refused post accident drug and alcohol screen, left the site without notice prior to end of shift, and later resigned via text msg.
While replacing bin pulley assembly, employee was removing a bolt with a 1/2 impact and a 15\16 ratcheted box end wrench. The wrench spun, trapping finger between the wrench and the bracing resulting in the loss of the tip of left ring finger.
Removing 2X2 ceramic liners from the discharge box of #12 screen. I grabbed the heavy liner and set it outside the box on the catwalk. I realized it had serrated edge. Cut the palm on left hand.
Employee injured EE's heel while trying to get a crusher part.
A SMUD transformer exploded causing a grass fire near the mine site in the Aerojet easement. The fire was never actually on the mine site but called in for reporting purposes.
While removing the retarder on a 633 mega scraper the retarder valve fell and struck the employee in the eye area. The employee was wearing safety glasses at the time of the incident. The angle of the falling valve knocked the glasses off and the employee got cut by the sharp metal of the valve. Employee sustained two lacerations on outside of right eyelid.
EE was laying on the screening media when working. Upon sitting up, ee felt a sharp stabbing pain in lower right abdomen. EE was replacing both the upper and lower motors on pre wash.

While operating the Portable Powerscreen Jaw crusher, the employee had a rock fall from above that struck the operators control panel. The control panel door stuck the employee in the back. The extent of the injury to the employee was unknown so the emergency response team called an ambulance immediately. Employee was taken to the emergency room.
Cut hand on roller that wasn't machined smooth, had a razor sharp edging. Cut required 6 stitches.
Trommel caught on fire after welding operations where being performed on the outside of it. Employees left for lunch at 6:00PM and returned at 6:30 PM and trammel was on fire. Fire department was called and fire extinguished around 10:30PM.
Hit with sharp point of rock hammer on the left hand.
Trying to put tail pulley on truck that was hanging from a truck mounted crane. Cut left pinky finger on a jagged edge of tail pulley.
Approaching switch gear to lock out conveyor, wasp flew under safety glasses and stung the left eye.
Employee jumped from an excavator to a haul truck. Fell approx. 7' sustaining head and back pain. Employee refused assistance and medical attention.
Employee experienced lower back pain while installing screen media panels on a new trommel.
equipment operator slipped on the steps of a 980H loader. Fell back tried to catch himself, resulting in a broken left wrist.
While drilling on a sheet of steel victim got small sliver of metal embedded in his right eye. The victim was wearing safety glasses at the time , however it was breezy in the area he was working. Once he felt the sliver he irrigated his eyes at the eye wash station, and he thought he had removed the problem. Several days later his right eye was red, itchy and very watery.

Replacement of media screen. J bolt in middle of screen broke loose snapping back striking him in the mouth.
While lifting the forklift blade to adjust the horizontal location, the employee got her right pinky finger smashed between the blade and the carriage. This resulted in a hairline fracture and sutures to close the wound.
Employee was exiting skid steer and employee was standing on the right tire. When employee stepped down with right foot, employee's left foot got hung up in the tire tread causing pain in the left knee.
Employee twisted his knee on 12/9/2013 while working in the plant but thought it was fine (First Aid only). He reported it to the Manager and returned to work that day. On 2/19/14 employee informed the Superintendent that his knee was not healing. He was transported to the clinic at that time where the physician recommended physical therapy.
Hearing loss
EE was pulling on an 1 1/8" wrench & he strained his right shoulder. ** This did not become reportable until 10/12/2012 when information was received about physical therapy **
Employee reported stepping off of a conveyor frame losing balance and striking his lower back against a hose hanger bracket mounted on the wall of # 6 tunnel. Employee was diagnosed with a low back contusion/sciatica.
Running a scraper when the seat bounced going through cut. Felt the seat cushion slam down.
Small piece of rock hammer chipped off and struck employee in right abdomen.
EE was giving tickets to drivers at the time. He reports being struck by a rock in thigh and knocked to the ground.
