Employee as working on replacing the #1 drive motor for the screen. While torquing the bolt, the motor pivoted, causing EE's left hand to be caught between motor mount and pivot point; resulting in a fracture to EE's left thumb. EE went to clinic and EE's thumb was splinted.

Showing all 25Employee was holding a bearing still while another employee was hammering a chisel to remove race from the housing. During one of the hammer strikes to the chisel something broke off and the object went through sweatshirt of the employee holding the bearing and into EE's left forearm. Employee was taken to clinic where a piece of metal was removed; employee required three stitches.
Employee straightened after stemming a hole and felt a sharp pain in EE's right pectoral muscle that didn't go away for the rest of the day. Commented on it to 2 other EE's. Dull ache with certain movements triggering more intense pain. Worsened over the weekend.
Employee was using carpenters knife to cut grease line when the knife slipped off the hose and cut left thumb knuckle. Did not require stitches, was placed in a splint to immobilize due to the location of laceration.
Employee was trying to tighten the tension adjuster for the tail pulley on the feeder. EE was using a 3ft pipe wrench to tighten the nut and applying pressure down when the wrench slipped causing EE to strike chin on the tail pulley guard. Employee received a laceration to their chin that required three stitches.
On May 9, 2024 employee was ratcheting a cable come along to move a head pulley over and felt a pain in EE's left bicep/shoulder. This incident became reportable on 09/03/2024 when employee had outpatient surgery.
Employee was holding pin with a pipe wrench. While attempting to hit pin with a hammer, the employee missed and hit pinky finger causing fracture to end of finger and four stitches.
The employee was performing maintenance on a sand dryer. During maintenance the employee was cutting zip ties off of electrical wires with a collapsible knife. While doing so the employee cut the top of right hand resulting in five stitches.

Employee was installing a new belt, belt was spliced and employee broke off excess bolts. Employee was using a foldable knife to cut the corners off the belt when the knife slipped and the employee's left forearm went forward scrapping the broken bolts. Employee received a laceration to the left forearm requiring eight stitches.
Working on stripping wires - it was cold and snowy making it difficult to strip the wires. Cut deep into right thumb. Had to have surgery to repair the tendon in that thumb and had to get stitches. Wore hand brace.

Employee was using a bar to remove a rock that had become jammed in the Kodiak K400 Cone Crusher. The employee hit the rock with the bar; the bar came back, striking the employee in the face, resulting in a split upper lip on the left side of EE's face. Employee received stitches and was placed on restricted duty.
Employees was breaking up chunks of material with a rock bar, employee reached in to grab and roll chunk that broke up while other employee was still trying to bust it up...this resulted in employee getting finger struck with the bar resulting to laceration and break of ring finger.

The EE was in the process of removing wedges from an impact crusher. He was using two sledge hammers to work them loose. While he was hitting them back and fourth to loosen the EE missed the wedge and struck his hand with head of sledge hammer. No broken bones x-rays negative, RX for pain.
