Employee was using a bar to remove a rock that had become jammed in the Kodiak K400 Cone Crusher. The employee hit the rock with the bar; the bar came back, striking the employee in the face, resulting in a split upper lip on the left side of EE's face. Employee received stitches and was placed on restricted duty.
Employee was replacing 2 v-belts on a conveyor head pulley. As EE was turning the pulley to guide the 2nd belt on, the belt pinched EE's finger against the pulley.
After inspecting the hopper, the miner lowered the hopper lid and received a cut while removing his hand

DOCTOR SENT MINER TO SURGERY FOR CARPAL TUNNEL ILLNESS. THE SAFETY OFFICE WAS NOT NOTIFIED SINCE THE MINER WENT TO HIS PERSONAL DOCTOR.
