Over-exertion in lifting objects

Employee was picking up a used liner off of the ground and turned and leaned forward to put it in the skid steer bucket and felt a sharp pain in chest.

During the injury the employee was performing maintenance on a wash screen. The employee slipped and fell forward smashing EE's thumb on a bolt that holds the spray bar on the wash screen.

Struck by falling object

The employee was in the process of cleaning under the primary crusher using a John Deere 318 D skidsteer. While he was performing his work, a rock fell from above and struck him in his lower right leg.

Over-exertion in lifting objects

EE was in the process of removing a tripod from the rear of his vehicle. While reaching in with his right arm to lift up,he felt a pop in his shoulder. At doctor visit the EE received a shot in the shoulder.

The EE was in the process of removing wedges from an impact crusher. He was using two sledge hammers to work them loose. While he was hitting them back and fourth to loosen the EE missed the wedge and struck his hand with head of sledge hammer. No broken bones x-rays negative, RX for pain.
