employee was changing pin with porta power. Pusher metal shot out, ricocheted off dozer and struck them in the face. They received 30 stitches for the wound.
Mashed left ring finger while striking a waterline with a hammer. EE received pins and stitches on left hand for the injury
Employee was removing ice and snow from switch gap with a switch broom,(broom on one end scraper on the other end). EE took a break and was standing when EE felt a pain in back and went down to one knee from the pain. EE has a muscle spasms, back strain.
Employee was cutting belt away from MRS Valve when knife slipped, cutting left ring finger. Employee was transported to ER where the employee received 5 sutures and was released back to full duty.
Employee was hammering on tail roller shuttle car and a piece of metal went into EE's hand.
Mechanic was removing the retaining bolt from the hoist cylinder pin on 522 when the wrench slipped and struck the mechanic in the face which broke a tooth and cut lip.
Mechanics had removed the old edges and cleaned the surface for the new ones. While lifting the edges a bar is used through the bolt hole so it is easier to carry. The edge slipped on the bar and pinched the mechanics finger between the bar and the edge resulting in a laceration. Employee received 3 sutures in EE's left finger work as tolerated and returned to work.
Employee had previously twisted a knee while tightening bolts but continued to work, sought medical attention on 12/18/2018 where EE underwent a procedure and received stitches to rt. knee and returned to work the next day. No time lost.
Employee was changing a broken training roller when they struck their right index finger with a hammer resulting in two stitches.
EMP was cutting the flapper from the take up on M5 Drive. As ee was cutting the belt, the knife slipped cutting left hand. EMP was treated and received 6 stitches.
Employee was cutting a skirt board and cut downward slipped and cut forearm.
Employee was bolting top in #1 entry. Insert wrench broke, small fragment struck employee above eye. Cut was about one half inch long. Took employee to ER, cleaned cut. Later went to local hospital, they removed the small fragment.
EE was helping change a speed reducer on No.8 Drive using a bar to remove the chain from sprocket. The bar slipped & caught EE's finger in chain causing a laceration to right ring finger. EE received stitches.
Employee was using a pipe wrench to remove cap from radiator, antifreeze blew on employee's hand causing burns.
EE was making a splice with knife & knife slipped & cut left hand between thumb and pointer finger.
While installing new miner conveyor, beating links together, sledge hammer came apart & metal pierced EE's leg on shin near calf.
Unstocking a cable from disconnected box, cutting ground lead loose, EE's knife slipped & cut thumb. EE received stitches and returned to job that shift on full duty.
Repairing a tire on mantrip, using a 4lb hammer, ee missed rim & pinched tip of right index finger.
Employee was taking lunch break and was cutting avocado with personal knife. The knife slipped and contacted left hand ring finger resulting in laceration. Employee was taken to the mine EMT, the wound was cleaned and bandaged. Employee was taken to hospital where employee received five sutures to laceration
Employee was cutting heel from boot with a knife. The knife slipped & cut wrist. EE received 1 stitch & returned to regular shift later that night.
EE was tightening a nut on a D3 EE was operating with pliers, both hands using full grip. EE said EE's chest popped & EE let go & drove ride outside & reported to another EE. EE did not see a M.D. until 7 days later, 11/27/17. EE missed 1 day of work, 11/28/17 and returned to EE's normal job 11/29/17.
Employee was using a utility knife to cut tape & knife slipped and cut employee's hand. Employee received 4 stitches.
EE was using a hammer on belt splice when a piece of metal came off hammer & went into their hand at the knuckle between the index and middle finger. The metal was removed & they received 3 stitches. EE has no loss time, EE returned to work.
Employee was holding pry bar with left hand and was hitting it with 8 lb hammer when they missed the bar & struck their left hand. EE finished the shift but went to physician on 07/16/2017, next day. EE was diagnosed with a bruised radial nerve. EE was taken off work for 3 days and returned to work on 07/20/2017.
Employee was cutting a piece of cord lash, when the utility knife slipped & cut EE on the inner part of the lower left thigh. EE went to ER, received 5 stitches and returned to work full duty on the following workday 04/24/2017.
Employee setting tailpiece, while turning turnbuckle w/hammer the hammer broke loose striking EE in the upper lip. This resulted in laceration requiring 2 stitches to the lip. EE went to ER @ Lonesome Pine Hospital in Big Stone Gap, VA. EE had no loss time and has full release to RTW.
Employee was cutting conduit off imer cable, holding cable in left hand, cutting away with right hand. When knife blade slipped cutting employees left palm. Employee received 3 stitches in the ER.
Employee observed a splice on lo-lo that needed trimmed before turning on the belt. While trimming the splice, the knife slipped and cut employee's left thumb leaving a 1 1/2" cut to the middle joint of the left thumb. Employee received 6 stitches.
Employee states that he was using a slate bar on 242 screen bowl when the bar twisted in his hand cutting web between thumb and finger.

Employee was assembling highwall miner-using sledgehammer & slate bar. Was prying with slate bar to align bolt holes on structure of 2nd deck, bar slipped & he fell on deck onto his behind.
