While removing a bearing, employee struck self in face with hammer

Smashed finger between steel frame and prybar while trying to remove built up material from a roller.

Employee was cutting flashing with a knife, knife slipped and cut EE's leg. Took several stiches to close the laceration.

Employee was striking a steel pin when a metal shard broke off and embedded in EE's leg. After metal shard was removed, a stitch was required to close the laceration.

Employees were performing maintenance on a crusher screen. One employee inadvertently struck partners finger with a hammer while installing screen clamp bolts. The injured employee has an open fracture of the left middle distal phalange.

Finger was pinched when wrench slipped resulting in a laceration to the left index finger tip. Took 6 stitches to close wound.

Employee was standing on a step ladder shoveling off a conveyor, ladder shifted and employee fell backwards to the ground.

Pounding in silt fence and felt tightness in neck afterwards.

EE was hammering screen deck rail wedge pin when the wedge pin broke and one part flew up and hit EE in the lip resulting in the injury.

EE cleaning return roller with a bar, it got caught resulting in laceration to the right ring finger.

Using a pry bar to move tail pulley of cross conveyor to unload the belt of rock when the pry bar slipped he landed on it against his chest.

A fragment from a steel pin when struck by a mall flew toward an EE striking him on the wrist.

Employee reported that he was attempting to use a pry bar to dislodge frozen chunk material from the chute of the bin. This activity apparently resultng in shoulder pain.

While tightening rail bolts on screen plant employee pulled hard causing a pulled muscle in Rt. shoulder.

Employee was replacing teeth on a 330 Volvo front-end loader bucket. Kyle attempted to drive the pin out of the tooth with a mallet and struck his thumb as a result. Kyle began to loose time on 6/29/04.

WHILE USING A HAMMER TO POUND ON BLOCK OF EQUIPMENT, EE CAUGHT THE TIP OF HIS THUMB. THE RESULT WAS THE THUMB NAIL WAS SEPARATED AND THUMB BONE FRACTURED.

EE WAS USING A CHISEL TO CLEAN DIRT FROM THE JAW, THE CHISEL SLIPPED RESULTING IN A CUT INJURY.

AS EE RAISED HIS ARMS ABOVE HIS HEAD TO PULL ON THE RATCHET WRENCH TO ADJUST THE JAW, HE FELT PAIN IN HIS RIGHT SHOULDER.

EE WAS HELPING TO PUT A BEARING ON A SHAFT. WHEN THEY BEGAN TO STRIKE A PUNCH WITH A HAMMER A FRAGMENT FLEW OFF THE PUNCH. THE FRAGMENT STRUCK EE'S BICEP AND EMBEDDED.

EE USE A PRY BAR ON CONVEYOR BELT OF THE IMPACT CRUSHER. THE PRY BAR SLIPPED OFF RESULTING IN SMASHED FINGERS AND STRAINED SHOULDER.

EE LACERATED HIS LEFT THUMB WHILE CUTTING WATER HOSE.

WHILE PULLING ON A PIPE WRENCH WITH BOTH HANDS, WHEN EVERYTHING CAME LOOSE THERE WAS PAIN IN HIS WRIST.

MINER WAS WORKING ON THE JAW CRUSHER TIGHTENING THE TOGGLE PLATE. AFTERWARD HE STARTED TO HAVE PAIN IN HIS SHOULDER. THE MINER HAS SOUGHT MED TREATMENT.

WHILE CUTTING BELTING FOR A CONVEYOR-UTILITY KNIFE SLIPPED AND CUT EE'S LEG REQUIRING MEDICAL SERVICES.

WHILE CHANGING THE JAW ON THE PRIMARY, EE CRAWLED INSIDE TO TIGHTEN SOME BOLTS. HE WAS PUSHING WITH HIS LEGS WHEN HE FELT SOMETHING POP. HE IS TREATING CURRENTLY.

EE WAS TRYING TO SEPARATE A PIECE OF STEEL FROM THE DRILL STEEL WITH A PIPE WRENCH. IT SLIPPED OFF AND CAUGHT HIS LT RING FINGER BETWEEN THE WRENCH AND FRAME. THE LACERATION REQUIRED STITCHES.

WHEN TAKING MACHINE APART, PULLING ON A WRENCH EE FELT SOMETHING SNAP IN HIS RT WRIST. SOUGHT MEDICAL ATTENTION.

WHEN CHANGING THE IMPACT BARS ONTHE MILL. EE STRUCK HIS HAND W/HAMMER. HE ATTEMPTED TO REMOVE A BOLT W/HAMMER AND PUNCH.

EE WAS CHANGING THE BELT LACING ON DISCHARGE CONVEYOR WHILE PRYING THE OLD LACE LOOSE HIS HAND SLIPPED RESULTING IN LACERATION ON MIDDLE FINGER

WHILE SHOVELING EE STRUCK HIS FINGER AGAINST A PIECE OF METAL AT THE ROLL PLANT.

WHILE CUTTING A PIECE OF CONVEYOR BELT WITH A UTILITY KNIFE EE STABBED HIMSELF ABOVE THE KNEE

EE SHOULDER BECAME PAINFUL WHILE ADJUSTING THE JAW OF THE BREAKER, PUSHING UP WITH A WRENCH.

WHILE KNOCKING FROZEN MATERIAL OFF A CONVEYOR WITH A HAMMER HE BEGAN HAVING PAIN IN HIS RIGHT ELBOW.
