EE was stepping down off a flatbed trailer, slipped and fell striking his right shin on the metal frame.

While using a pry bar to loosen clay stuck in ccne crusher, bar caught on material and slammed left hand against handrailing.

When taking blocking off the frame of the equipment, EE smashed his little finger between block and angle iron resulting in a laceration on the little finger.

Two employees were switching out screens when one employee pushed up and the other pulled up. Injured's wrist got caught between the screen and the screen rail.

EE USE A PRY BAR ON CONVEYOR BELT OF THE IMPACT CRUSHER. THE PRY BAR SLIPPED OFF RESULTING IN SMASHED FINGERS AND STRAINED SHOULDER.
