While performing repairs on screens, maintenance employee cut themselves with a utility knife sustaining a thigh laceration. Emergency personnel dispatched to facility where the employee was transported via ambulance for further medical treatment. Employee received sutures and was discharged later that day.

On 6/15/23 an EE was replacing a ripper tooth on a CAT dozer. The EE used a sledgehammer to dislodge the ripper tooth and upon impact a roughly 1/4-1/2" piece of the tooth broke off and struck the EE in the left calf. On 6/16/23 EE was taken to the clinic where an x-ray confirmed the object was lodged in their calf. EE was prescribed antibiotics.

A water pump in the pond was being replaced by 6 employees. One employee was holding a 48" pipe wrench holding the shaft in place. Another employee was using a 36" pipe wrench to turn the shaft to break it free. As the employee turned the shaft, the employee holding the pipe wrench lost grip and the tool swung around coming off striking an employee in the head above left ear.

Employee was replacing a transmission when the transmission slipped off the jack, and pinched employee's right little finger between the jack and the transmission. The employee received stitches and was diagnosed with a partial traumatic transphalangeal amputation of right little finger.

Employee was changing cutting bits on a piece of equipment. He was using an 8 lb hammer to release the retaining pin. A metal fragment chipped off the pin and struck his low abdomen. He was taken to a clinic and the fragment was partially removed. The remaining portion was left in place due to doctor's recommendation.

On 6/4/2014, the employee was taken to the emergency room where he received stitches for an eyelid laceration. While the employee was using a chisel and hammer, the chisel dislodged and struck him in the face. The employee was diagnosed with a left eyelid laceration and was released to full duty on 6/5/14.

Employee requested medical treatment on July 9th and was placed on restricted duty. Employee stated he was turning water valve using a wrench. During the process, the wrench was stuck and when he applied additional pressure, the wrench slipped off the valve and alleged he fell backwards. Diagnosed with lumbosacral sprain, lower and upper back contusion(s), and back abrasion.

On 1/5/11 employee was taken to the company clinic and was placed on restricted duty. The employee attempted to remove the right outside tooth from the bucket of a loader with a 4 lb. hammer when a metal fragment chipped off and entered the R-knee. The employee was diagnosed with a foreign body to the knee.

On 9/14/10 The employee was taken to the hospital and received staples for a L-knee laceration. The employee was cutting a plastic piece of guarding with a utility knife when he lost control of his arm motion and hit the inner leg with the knife. The employee was returned to regular duty on 9/14/10.

EE WAS REMOVING A STEEL FRAME FROM A RAILING THAT WAS BEING INSTALLED ON A NEW PRODUCTION PLANT/THE FRAME WAS ATTACHED TO THE RAIL BY BOLTS AND TACK WELDS. AFTER REMOVING THE BOLTS THE TACK WELDS FAILED TO HOLD THE FRAME CAUSING IT TO FALL 6" ON TO THE EE FOOT.EE INCURED A MINOR FRACTURE TO HIS BIG TOE ON HIS RIGHT FOOT. EE NOTED DISCOMFORT IN HIS RIGHT EYE AFTER THE END OF HIS SHIFT. HE REPORTED THE DISCOMFORT TO HIS SUPERVISOR ON 8-27-03 AND WAS SENT TO THE CLINIC. HE SPENT THE DAY INSTALLING TAIL GUARDS AND CLAIMED HE WAS WEARING SAFETY GLASSES AT ALL TIMES. IT WAS NOT NOTICEABLY WINDY.
