While performing repairs on screens, maintenance employee cut themselves with a utility knife sustaining a thigh laceration. Emergency personnel dispatched to facility where the employee was transported via ambulance for further medical treatment. Employee received sutures and was discharged later that day.

On 6/15/23 an EE was replacing a ripper tooth on a CAT dozer. The EE used a sledgehammer to dislodge the ripper tooth and upon impact a roughly 1/4-1/2" piece of the tooth broke off and struck the EE in the left calf. On 6/16/23 EE was taken to the clinic where an x-ray confirmed the object was lodged in their calf. EE was prescribed antibiotics.

A water pump in the pond was being replaced by 6 employees. One employee was holding a 48" pipe wrench holding the shaft in place. Another employee was using a 36" pipe wrench to turn the shaft to break it free. As the employee turned the shaft, the employee holding the pipe wrench lost grip and the tool swung around coming off striking an employee in the head above left ear.

Employee was replacing a transmission when the transmission slipped off the jack, and pinched employee's right little finger between the jack and the transmission. The employee received stitches and was diagnosed with a partial traumatic transphalangeal amputation of right little finger.

Employee was trimming the conveyor belt around the motor using a husky folding knife resulting in a left forearm laceration.

Employee was using a metal hammer and metal chisel to clean debris off of tooth when a metal fragment struck them and embedded in chin.

EE was performing maintenance with a pry bar and allegedly felt a pull in the stomach area. Monday, EE stated EE was lifting grease cover and felt a pop in the stomach area.

Employee was using "Dog and Wedge" to better position a clad cheek plate, striking pieces with hammer. Fragment from steel hammer broke off hammer enter eye from below safety glasses. Fragmentation caused from steel to steel strike.

Employee was cutting a small piece of belting and pulled the knife in to employee's right leg above the knee. Making a 1 inch cut

Employee was using a utility knife cutting towards self cutting thigh.

Employee was changing cutting bits on a piece of equipment. He was using an 8 lb hammer to release the retaining pin. A metal fragment chipped off the pin and struck his low abdomen. He was taken to a clinic and the fragment was partially removed. The remaining portion was left in place due to doctor's recommendation.

On 6/4/2014, the employee was taken to the emergency room where he received stitches for an eyelid laceration. While the employee was using a chisel and hammer, the chisel dislodged and struck him in the face. The employee was diagnosed with a left eyelid laceration and was released to full duty on 6/5/14.

The employee sought medical treatment and was placed on restricted duty , which is being accommodated. While pulling on a ratchet to turn a take-up bolt on a tail pulley located beneath the cone belt, the employee sprained his right shoulder.

The employee received dental treatment on 8/29/12, beyond first aid. The employee whipped a chain and hit his face resulting in a broken tooth.

Employee requested medical treatment on July 9th and was placed on restricted duty. Employee stated he was turning water valve using a wrench. During the process, the wrench was stuck and when he applied additional pressure, the wrench slipped off the valve and alleged he fell backwards. Diagnosed with lumbosacral sprain, lower and upper back contusion(s), and back abrasion.

SERVICE TECH WAS TRYING TO BREAK THE BACK BEAD ON A TIRE WITH A COMBINATION BEAD BREAKER HE SAID HE WAS STANDING ON THE SIDE OF THE TIRE BUT AFTER INVESTIGATING HE WAS UNDER THE LOADER NOT ON THE SIDE OF THE LOADER HE PUT PRESSURE TO THE BEAD BREAKER IT CAME LOOSE AND HIT HIM ON THE FOREHEAD HE WAS WEARING HIS HARD HAT AND THAT SAVE HIM FROM SERIOUS INJURY

On 1/5/11 employee was taken to the company clinic and was placed on restricted duty. The employee attempted to remove the right outside tooth from the bucket of a loader with a 4 lb. hammer when a metal fragment chipped off and entered the R-knee. The employee was diagnosed with a foreign body to the knee.

On 9/14/10 The employee was taken to the hospital and received staples for a L-knee laceration. The employee was cutting a plastic piece of guarding with a utility knife when he lost control of his arm motion and hit the inner leg with the knife. The employee was returned to regular duty on 9/14/10.

The employee went to the company clinic on 8/23/2010 and was placed on restricted duty. Employee was using two wrenches to loosen a hydraulic coupling; one wrench slipped off the fitting and employee's L-elbow hit the frame of the haul truck.

The employee went to the company clinic on 11/14/09 and was placed on restricted duty. The employee was using a putty knife to remove a decal, when the knife tore through the decal and struck L-palm, causing a laceration that required sutures.

Employee went to the company clinic on 12/3/08 and was placed on restricted duty. The employee received a Right Lateral Epicondylitis (tennis elbow) to R-arm while raking through material on the conveyor belt.

The employee went to the company clinic on 9/3/08 and was placed on restricted duty. A porta-power disconnect fitting came apart when pressurised and struck employee on his safety glasses, resulting in a corneal abrasion and contusion to R-eye.

Employee was loosening a u-bolt on the wet screen and the bolt broke causing the wrench to strike him in the mouth loosening his left tooth (#9). Employee was seen by Dentist and a root canal was performed 05-28-08.

Employee was working on a sand screw out of a course material washer which was supported by two jack stands. The log fell off crushing the right foot and causing a small laceration.

The employee went to the hospital on 3/25/08, was placed on restricted duty, and received sutures. The employee was stripping casing from fiber optic cable using a utility knife when the knife slipped and lacerated the right palm of his hand.

While removing north screw of blade mill BM-1 using come-alongs, the shaft shifted unexpectedly and employee's right leg was caught between the flights of the north and south screws. A root cause was stored energy not controlled.

Employee went to the company clinic on 12/18/07 and was placed on restricted duty. The employee states that while he used a hammer and chisel to adjust skirting on a conveyor, he missed the target and struck his left thumb resulting in a contusion and laceration requiring sutures.

The employee went to the company clinic on 11/19/07 and was placed on restricted duty. The employee states he used a sledge hammer to remove a wedge that was stuck on the crusher. As he attempted to hit the wedge, he missed the target and hit his R-thumb.

The employee went to an emergency room on 10/05/2007, received stitches, and placed on restricted duty. The employee was cutting zip ties with a box knife cutter and lacerated his left forearm.

The employee went to the company doctor on 7/10/2007 and was placed on restricted duty. While chipping a weld, using a chipping hammer, a piece of hot slag bounced into the employee's left eye, resulting in a corneal burn to L-eye.

The employee went to the company clinic on 5/29/2007 and was placed on restricted duty. While attempting to remove the inner race of a double inter lock bearing with a hammer, a piece of steel became embedded in the employee's L-forearm.

Employee was trimming rubber liners with a razor knife when it slipped and cut his left index finger

Mechanic struck end of tooth with a hammer while replacing bucket tooth on 988G loader and piece of steel broke off tooth and hit him in the left eye.

The employee went to see the company doctor on 10/17/2006 and was returned to regular duty. A piece of metal became embedded in the employees L-cheek of face while chipping a rock out of the bolt hole. Stitches were applied.

While removing bracket teeth with a hammer, piece of metal lodged in left forearm.

The employee went to see the company clinic on 5/4/2006 and was returned to full duty. While adjusting the scrubber, the employee struck L-index finger with the hammer which caused a laceration that required sutures.

The employee went to see the company doctor on 4/4/2006 and was placed on restricted duty. While cutting rubber with a utility knife, the knife slipped causing a laceration to R-hand, which required stitches.

EE STATES HE WAS CUTTING THE BOTTOM DISCHARGE CHUTE PLATE & USING A SLEDGE HAMMER TO KNOCK IT FREE WHEN THE TOP CHUTE PLATE CAME DOWN & CAUGHT THE HAMMER & THE TIP OF HIS LEFT RING FINGER CAUSING A CRUSHING INJURY.

Employee was repairing hydraulic leak on accumulator on HP500 cone crusher when pressurized hydraulic fluid was released from the accumulator causing employee to strike himself in the chest with a pipe wrench.

The employee went to see the company doctor on 9/6/2005 and was placed on restricted duty. The employee cut his R-forearm with a utility knife while cutting rubber, causing a laceration that required sutures.

Employee went to see company doctor on 6/16/2005 and was placed on restricted work; while removing a guard on the tunnel feeder, the cutting knife slipped and caused a laceration to L-palm that required stitches.

Employee went to see company doctor on 4/4/05 and was placed on restricted work. Employee was struck in the face by a piece of metal that broke off from a damaged loader bucket tooth that was being hammered.

Employee was using hammer on clamp rail of second screen deck. Hammer struck upper deck frame glancing off course and striking employee on left thumb.

EE WAS REMOVING A STEEL FRAME FROM A RAILING THAT WAS BEING INSTALLED ON A NEW PRODUCTION PLANT/THE FRAME WAS ATTACHED TO THE RAIL BY BOLTS AND TACK WELDS. AFTER REMOVING THE BOLTS THE TACK WELDS FAILED TO HOLD THE FRAME CAUSING IT TO FALL 6" ON TO THE EE FOOT.EE INCURED A MINOR FRACTURE TO HIS BIG TOE ON HIS RIGHT FOOT. THE EE WAS ABLE TO CONTINUE WORKING HIS NORMAL DUTIES NO LO

EMPLOYEE WAS ATTEMPTING TO REMOVE FEEDBOX/SPLASH PAN TO 6'X16" VIBRATING SCREEN WITH THE HELP OF TWO COME-ALONGS. THE SCREEN FEEDBOX SWUNG DOWN AND STRUCK HIS LEFT THIGH CAUSING A CONTUSION AF TER BEING UNBOLTED FROM SCREEN. EMPLOYEE WENT TO SEE COMPANY DOCTOR ON 8/27/03 AND WAS PLACED ON RESTRICTED DUTY.

EE NOTED DISCOMFORT IN HIS RIGHT EYE AFTER THE END OF HIS SHIFT. HE REPORTED THE DISCOMFORT TO HIS SUPERVISOR ON 8-27-03 AND WAS SENT TO THE CLINIC. HE SPENT THE DAY INSTALLING TAIL GUARDS AND CLAIMED HE WAS WEARING SAFETY GLASSES AT ALL TIMES. IT WAS NOT NOTICEABLY WINDY.

EMPLOYEE, WHILE CUTTING RUBBER WITH A UTILITY KNIFE, RECEIVED A LACERATION TO HIS LEFT HAND. HE WENT TO SEE COMPANY DOCTOR ON 8/6, STITCHES WERE APPLIED, AND HE WAS PLACED ON RESTRICTED DUTY.

EMPLOYEE WENT TO SEE COMPANY DOCTOR ON 3/21/03, STITCHES WERE APPLIED AND EMPLOYEE WAS PLACED ON RESTRICTED DUTY. ATTEMPTING TO REMOVE A STRAP FROM A COUNTER WEIGHT ON #10 CONVEYOR, PRYBAR SLI PPED ALLOWING HAND TO HIT A GUARD CAUSING A LACERATION TO R-WRIST.

WHILE LOOSENING THE RETURN ROLLER WITH AN OPEN-END WRENCH, THE WRENCH WAS CAUGHT IN THE CONVEYOR BELT CAUSING A LACERATION TO THE RIGHT HAND. EE WENT TO SEE COMPANY DOCTOR ON 10/10/02, BUTTE RFLY BANDAGES (STERI STRIPS) WERE APPLIED AND EE WAS RETURNED TO FULL DUTY.

EMPLOYEE WENT TO SEE COMPANY PHYSICIAN ON 10-9-02, STITCHES WERE APPLIED AND EMPLOYEE WAS RETURNED TO FULL DUTY. WHILE USING A RAZOR KNIFE TO CUT RUBBER LINERS, EE RECEIVED A LACERATION IN LEF T RING FINGER.
