Over-exertion in lifting objects

Employee felt a pull in lower back when employee placed a steel plate, measuring approx. 16"" x 20"", onto the cutting table.

Over-exertion in lifting objects

On 7/15/19, employee was stacking screens on a pallet, stepping from left to right, when they felt their right knee pop to the inside of the leg. The employee was seen at the Company Clinic and diagnosed with a knee sprain, placed on modified duty, and was prescribed physical therapy.

On 11/22/17, employee received medical treatment and was placed on restricted duty which is being accommodated. The employee slipped, heard a pop in right knee and experienced a right patella dislocation.

On 9/9/15, the employee was taken to Company clinic, placed on restricted duty which was accommodated. Employee while walking in Company parking lot was struck by a co-worker who was operating his personal vehicle. The employee was diagnosed with L-hip buttocks contusion, lumbosacral strain, L-elbow contusion and laceration.

SERVICE TECH WAS TRYING TO BREAK THE BACK BEAD ON A TIRE WITH A COMBINATION BEAD BREAKER HE SAID HE WAS STANDING ON THE SIDE OF THE TIRE BUT AFTER INVESTIGATING HE WAS UNDER THE LOADER NOT ON THE SIDE OF THE LOADER HE PUT PRESSURE TO THE BEAD BREAKER IT CAME LOOSE AND HIT HIM ON THE FOREHEAD HE WAS WEARING HIS HARD HAT AND THAT SAVE HIM FROM SERIOUS INJURY

On 9/10/10 while the employee was loading out waste material, he stated over the radio that he felt faint. After parking the loader at the office, his manager noticed symptoms of faintness and confusion and the employee stated he had blurred vision. The employee was transported by EMT to Sharp Memorial Hospital on a non-occupational basis.

On 3/30/10 the employee was in the breakroom to start his lunch break when he called his supervisor to report discomfort in his chest and back. Management summoned EMS which responded. The employee was diagnosed with chest pain (non-specific) by a physician on a non-occupational basis.

Employee was taken to the Company clinic on 2/9/09 and was placed on restricted duty. The employee states that while placing a screen cloth, weighted at 15 lbs., into the bed of truck, he felt his right shoulder pull (Allegedly). Employee was diagnosed with a right shoulder strain.

The employee went to see the company doctor on 4/15/2005 after the injury happened on 4/11/2005 had become irritated, and was returned to full duty. The employee was removing a hose to unplug the pump, and was burned by hot water.

WHILE CHANGING SKIRT BOARD RUBBER AROUND FEEDER AND REPAIRING DIAMOND DECK PLATE, EMPLOYEE FELL THROUGHT HOLE AND STOPPED HIMSELF WITH HIS ELBOWS WHICH CAUSED LATERAL EPICONDYLITIS TO L-ELBOW.
