Employee was climbing the ladder of 605-8 haul truck, while doing so the second rung of the ladder broke free from the left support. This caused the employee to drop roughly 12" to the first rung of the ladder, employee notified the mechanic and the supervisor who replaced the ladder. Employee was given days away from work by clinic.

Employee was assisting another employee in cleaning drink coolers; the employees were using a pressure washer to do so. The injured employee reached to catch the cooler as it was falling out of reaction and was struck on the top of the hand by the water stream causing laceration to their left thumb. Employee was taken to ER where they received 6 stitches.

Employee was splicing a belt using a battery operated drill, while tightening one of the nuts down it bottomed out. The torque from the drill wrenched from their hands, the drill spun around and the battery struck the employee in the jaw chipping a molar. There was no bruising or laceration. They went to the dentist where a crown was installed on the tooth.

Bolts on weather head of motor broke. While repairing the motor an employee's finger was caught by a wire while tightening a bolt with an impact wrench causing the finger to be degloved just below the fingernail after the employee jerked the finger back.

An employee was using a handheld die grinder to widen the center of a spool, when it kicked out of the hole and caught the employee's gloved left ring finger. The left ring finger was amputated down to the highest knuckle.

An employee placed the handheld battery powered grinder they were using to change the bearings on a tail pulley between their body and left arm, when it turned on and lacerated their upper arm.

While attempting to clear a blockage in an ANFO hose, a small amount of ANFO and compressed air hit the employee in the left wrist causing a burn/abrasion. After the employee's second follow-up appointment on 12/15, EE was given days off, making the injury reportable.

Employee was changing blow bars in a horizontal crusher when the jig that is used to lift blow bars fell over and smashed the employees finger causing a fracture.

Using jib crane to reposition pump clean-out/door assembly and part spun towards employee striking left leg and breaking it

Injured employee was putting air in the tire of the drill truck and another employee was letting the drill down so they could tell if it had enough air in it. The injured employee had their right foot under the tire and did not know the other employee was going to let it down. The foot got smashed.

Employee was assisting with flipping blow bars in a Stedman Grand Slam 6460 impact crusher. Employee got hand pinched between the rotor/blow bar and the sidewall of the crusher.

Using grinder with cutting disc. Disc broke and grinder kicked back cutting left bicep.

Employee put hand in pinch point to remove drive gear on grizzly.

On 3/15/19 an employee was cutting liners from a grizzly feeder with a torch. A piece of slag fell on the employee's pants and into their open top boot burning the top of their foot. The employee self treated over the weekend and went to the doctor the following Monday. The employee was diagnosed with 2nd degree burns and treated with antibiotic cream.

Using 10 1/2' burning bar cutting loader bucket tooth out of crusher. When tooth released the pressure pushed the burn bar holder into face causing small laceration on cheek close to right eye

The rock breaker was breaking oversize down small enough to run to primary crusher. A rock came back and hit the cylinder and then ricocheted back toward the cab entering the cab hitting employee in the right side of the head.

Employee got foreign object in eye when welding/chipping slag. Employee was wearing welding helmet at time of injury.

D8R Bulldozer was dressing floor of overburden pile. D8R was north and 30 offset to the west of 374F, loading material. 773D dumptruck lined up and responded on signal from excavator by backing up towards loading position. D8R backed into path of dumptruck. 773D struck ROPS of D8R. Collision caused D8R operator to strike self in the mouth with fist, fracturing teeth.

Employee was using a hand held electric grinder with a wire brush to clean parts for a crusher rebuild. The grinder kicked to the side catching glove and wrist. The brush came into contact with the employee's wrist causing a cut that required 5 stitches

Using a circular grinder to cut off the top of a metal handrail. EE had one hand on the rail and was operating the grinder with the other hand. The grinder requires two handed operation. (EE was wearing gloves for protection.)

Employee operating grader in spoil area and blade hit a hidden bolder throwing the employee forward. Employee's mouth struck the steering wheel knob and chipped a tooth.

The lifting chain came unhooked from the crane striking employee standing nearby.

Maintenance worker was using a cordless impact wrench to drill holes in conveyor belt spice. When pulling trigger to drill 3rd hole debris flew out of wrench vent holes and went under employees safety glasses and into right eye. Antibiotic eye drops prescribed.

RGI was injured while working on an excavator engine hood. The employee had the engine hood in the open position with the hinges clamped into place and was working from a ladder. The employee was working overhead, welding and repairing the hinges and was wearing safety glasses, welding hood, and helmet during this operation and had performed a verbal JSA on this task.

After completing some grinding activities the employee states they felt some dust or material fall on their face when they raised up the face shield. Employee also wiped their forehead off to clear the dust. Eye wash was used to flush the eye but still felt like something was in their eye. Dr. observed a scratch but nothing in the eye and prescribed antibiotic.

contract employee severed right index finger while performing task of emptying used oil filters into his truck. He also broke his right ring finger as a result of this pinch point injury.

Employee working in awkward position. No fixture to hold wedges at waist height. Employee failed to stop grinding wheel before releasing the handle. JSA did not cover task. Employee did not SLAM the hazard.

Miner was removing cutting edges, used impact to loosen bolts, bolt spun, he stuck his hand under the edge to hold up on the bolt, it spun again, cutting through a two layer mechanic's glove, and made a laceration on his hand. Took him to the clinic, where a doctor put in 7 stitches to repair. He was released back to work.

Engine cooler, hydraulic cooler and exhaust blow to rear of roof bolter. When the employee went to the bolt rack at the rear of the machine to retrieve roof bolts foreign debris flew into his right eye.

After grinding and welding felt eye irritation. Washed out eye. Next day (holiday) it was still bothering and he went to optometrist who removed tiny sliver of metal. Was wearing face shield and safety glasses when grinding.

Loader operator was holding the bottom of a bolt to a cutting edge while another employee was holding the top side. The bolt was spun in his hand cutting through his leather glove and into the palm of his hand. He was immediately taken to a local hospital where he received 8 stitches to the laceration in his right palm.

2 employees were to weld a broken hand rail & replace a v-belt guard. The 2 items were 5 feet apart. They decided to weld the handrail & then replace the guard. For reasons unknown the injured employee started replacing the guard while the welder was repairing the hand rail. This resulted in a flash burn to the employees right eye. The employee did not work his next shift.

EE was rebuilding the tail pulley guards at plant using torches and grinder. At 1:30 pm realized his eye was very dry and irritated, continued to work & finished the day. Around 5:30 pm at home his eye started to water. Reported on 10/01/10, and requested medical attention. Dr. removed a rust flake from his right eye and gave prescription medication for the eye.

I tripped and my hand hit down lever on drill rig. The bit then went down and crushed my foot.

Employee claims he was cutting a large hopper and while cutting the beam, he cut completely through. He stated he did not know he had cut through and had turned to walk around and cut the other side of the beam. He states when he turned around, the beam fell and hit the right side of his shoulder.

EE operating dozer on base stone pile, complained of soreness in his lower back area. He was given X-rays and MRI which showed new injury to old condition on 11/9/07. He was medically treated on 11/29/07 when doctor administered cortisone shots. The area that the EE had surgery on years ago was reaggravated by operating dozer.

A team was setting up the crane. An employee then put his fingers on the alignment collar-bolting flange to see if the bolt had broken off in the housing. While physically checking the bolt pattern the hydraulic cylinders were actuated and the alignment collar slid back into the cylinder housing smashing the left hand index finger of the employee who was checking the bolt.

Employee was removing bolts on loader cutting edge by keeping pressure on the head of the bolt with the palm of his right hand and loosening the nut with his other hand. The head of the bolt spun and cut the palm of his hand requiring stitches.

TECHNICIAN REMOVING MOUNTING BOLT FROM TRANSMISSION MOUNT - THE NUT PLATE BEGAN SPINNING AND CAUGHT FINGER AGAINST BOLT.

EE WAS WELDING STRAP ON PRIMARY FEEDER USING THE TELEDYNE HAMMER TO HOLD METAL IN PLACE. WHILE WELDING, HAMMER SLIPPED OFF AND STRUCK EE LITTLE FINGER ON LEFT HAND REQUIRING 5 STITCHES

Employee was burning out templates in steel plate in shop. After finishing task, employee began feeling dizzy. He later began feeling nauseous. Once returning to Industrial Machine, he vomited twice before being take to the ER.

AT APPROX. 4:15PM EE WAS RESETTING SHIMS JAW CRUSHER. EE HAD LOOSENED TENSION SPRING WITH IMPACT WRENCH JACKEDSAW OPEN NEEDED TO LOOSEN SPRING SOME MORE, PUT IMPACT BACK ON NUT, WHEN PULLED TR IGGER ON, IMPACT NUT CAME OFF RELEASING SPRING CAUSING HAND TO BE PINCHED BETWEEN AIR WRENCH & MOTOR FRAME, MASHING THUMB.

EMPLOYEE WAS CUTTING 2 PIECES OF METAL FOR THE SIDES OF AN L-220 VOLO LOADER BUCKET. AFTER CUTTING METAL 1" X 29" X 9" EMPLOYEE REACHED WITH LEFT HAND TO FREE METAL FROM THE PIECE WHICH IT WAS CUT FROM. WHEN IT BENDED SLIPPED FROM HIS HAND DROPPING ON TOP OF RIGHT INDEX FINGER. THE CUT TOOK 5 STITCHES. THE EMPLOYEE WAS WEARING GLOVES.

EE RECEIVED A FLASH BURN TO HIS RIGHT EYE AS HE WAS HOLDING THE OPPOSITE END OF A 10" PIPE THAT WAS BEING WELDED. HE WAS ABOUT 8' FROM WHERE THEPIPE WAS BEING WELDED WITH HIS BACK TO THE WELDI NG. HE CLAIMS HE NEVER LOOKED AT THE WELDING ANDHAD HIS BACK TO THE WELD THE ENTIRE TIME.

MECHANIC FAILED TO DISABLEFAN WHILE GUARDS WERE OFF. WHEN PROCEEDING TO DO MAINTENANCE WORK, FANCAUGHT LEFT THUMB RESULT IN CUT THAT REQUIRED ONE STITCHE AND PRESCRIPTION FOR PAIN.

EE WAS WELDING A CRACK ON EUCLID TRUCK IN SHOP. DURING HIS WORK A METAL SHAVING GOT INTO HIS LEFT EYE. HE WAS WEARING A FULL FACE SHIELD BUT NO SECONDARY PROTECTION. HIS EYE STARTED HURTING WH EN HE GOT OFF WORK THAT EVENING AND WENT TO DR. NEXT AFTERNOON. NO LOST TIME.

EE WAS IN THE PROCESS OF SIPNONING GAS FROM A WELDER GASOLINE WHEN HE ACCIDENTALLY SWALLOWED A SMALL AMOUNT.

REMOVING PIPE ON HIGHWALL EE WAS WORKING TOO CLOSE TO THE MACHINE TO BE DIRECTING THE OPERATOR. THE EE WAS ON THE INSIDE OF THE BERM WITH THE MACHINE WHICH COULD BE A RESTRICTED WORK AREA.
