Employee was washing his loader off at the end of the shift and slip and fell on the ground, falling on his right arm and fracturing his elbow.

EE was releasing stored energy off a hdy. cylinder & it appeared to have been released. He started removing the fitting, loosening it with a wrench & then with his hand. There was stored energy on the cylinder & it blew out striking the employee on his left hand between thumb & index finger. The grease blew a hole thru his glove & made a laceration, injecting grease under the skin.

OPENING DOOR ON CRUSHER BY HITTING PIN WITH PRY BAR & 4 LB. HAMMER, PRY BAR CHIPPED & PIECE OF METAL WENT THROUGH PANTS LEG & EMBEDDED IN KNEE.

EE STEPPED IN A HOLE IN WHICH HE HAD PREVIOUSLY MOVED A MOTOR FROM, DIDN'T HURT AT THE TIME BUT ON 6-8-01 HE REPORTED HIS LEFT SIDE HURTING, WAS SENT TO THE DOCTOR WHO DIAGNOSED BRUISED RIBS, PUT ON RESTRICTED DUTY 6-11 THRU 6-15.
