Fall to the walkway or working surface

Employee was removing roof bolts from pushed up scaling pile, after EE placed a bolt into the bucket of the loader EE was operating, employee tripped or lost EE's balance and fell to the ground at the base of the pile striking EE's head on a rock, employee suffered laceration to eyebrow and a broken nose.

Struck by falling object

Employee had bolted the face up tight then backed out roughly 50', was reloading bolt supplies when a piece of material fell from in between the roof bolts. It struck the cab and broke over the side striking the employee who was on foot next to the cab/engine bay checking the water level. Employee received laceration to their head and leg which were closed at the hospital ER.

Accident type, without injuries

The emergency escape hoist basket fell to the bottom of the shaft after the hoist rope broke. The basket was damaged beyond usage, the main hoist rope was broken and had to be replaced as well. It was found during the investigation that the rope had jumped the sheave wheel and was rubbing on the bearing housing causing wear/damage to the rope.

Underground blaster was clearing a lower hole of cuttings. Before EE started loading the hole, the employee was using right hand to clear the hole and using left hand to support body weight. A rock of unknown size fell from the face making contact with EE's left thumb smashing it between the rock and the wall, causing an open fracture to EE's left thumb.

Accident type, without injuries

The Roof Fall happened in an area that was worked out, and was being set up for benching floors, the fall happened at the 9.5 feet height in the back/roof which is above anchorage of our roof support. Through investigation with MSHA ADM, Roof Specialist, and Field Office Supervisor it is not immediately known what caused the fall. The fall was reported at 2:40pm.

Employee walking down slight grade looking at beltline when they dragged their toe as the grade transitioned to a flatter surface, causing them to trip. Employee caught themselves on the ground with their left hand causing a laceration that required stitches to close.

Installing a saddle in conveyor and the boom truck block hook struck the employees foot causing crushing injury without fracture.

Absorption of radiations, caustics, toxic and noxious substances

Employee received an eye injury when a hydraulic hose fitting came loose/broke and sprayed hydraulic fluid into the eye. EE was wearing safety glasses at the time but they were not preventive. Prescription eye medication was administered.

Using grinder with cutting disc. Disc broke and grinder kicked back cutting left bicep.

Making dead box on plant. Welded 1 plate in & slide another plate on top of 1st plate. Did not fit/trimed & slide back in place. Plate slide off causing pinched left index finger requiring medical attention.

Struck by falling object

Repairing rip rap deflector plate on scalper screen. Angle iron was laid on roll away cover which is at a slant. He was bending over to pick up another piece of metal for repairs then piece slid down chute & smashed index finger against the cover he was using as a hand hold.

Customer truck was overloaded & had dumped material. A rock was stuck in tailgate loader operator pulled back the tailgate to release rock. Tailgate slammed closed pinning his finger.

Bearing housing bolts broke in scalping screen. EE replaced broken bolts. Nylon strap was attached to shiv which was standing vertical. Employee let go of the shiv to connect the chain come along to the strap. The shiv fell over & down a step hitting the top of employee's right foot. JSA had been done but falling hazard not identified. Medical attention not requested until 3-12-13.

Engine cooler, hydraulic cooler and exhaust blow to rear of roof bolter. When the employee went to the bolt rack at the rear of the machine to retrieve roof bolts foreign debris flew into his right eye.

A belt clean up man called off work saying his arm hurt. He was taken to the doctor and he was diagnosed having tennis elbow and put on restricted duty.

Technician was putting tools back in bed of service truck. Was sliding bed cover toward rear of truck to close, hand slipped off bed cover. Technician stumbled toward rear of truck, fell over tailgate and fell to the ground, dislocating shoulder.

Replacing blowbars in primary impact crusher. EE's were using a sledge hammer in the process, loosening blow bars prior to removal. A piece of the blow bar came loose at it was hit with hammer. The fragment cut through blue jeans into EE's upper right thigh cutting muscle & nicking the artery.

Emp. was in the process of changing screens in Tower #2 when her hand came in contact with a rough place on screen. Middle finger on left hand was cut. She was taken to emergency medical and her finger required 5 stitches.

Employee was running 988F in mine cleaning up shots when he stopped to check the air cond. fan. Without looking up he stuck his hand thru the vent hole & his fingers came in contact with the fan cutting the tips of his two middle fingers on his left hand. Required 9 stitches.

EE was going to work on Cannon Face Drill. Nose cone came loose from hammer 14' up face. EE used F450 flatbed serv truck to stand on to remove nose cone from drill steel. He was standing on the toolbox (7'L x 3'W x 4'H). Truck had been outside (36deg). Truck brought UG (68deg), caused toolbox to sweat. EE slipped on wet toolbox, lost balance, jumped 4' down to bed of truck.

TWO SURVEY CREW EMPLOYEES WERE USING A JLG MANLIFT DURING A CENTER LINE CONTROL STAKE OUT. THE JLG FELL OVER THE HIGH WALL FROM THE UPPER FLOOR TO THE NEXT LOWER FLOOR AT A LOCATION WHERE THE FACE WAS NOT SLOPED & THE CREW DID NOT KNOW THE TOP OF THE HIGH WALL WAS NOT BARRICADED. NO WARNING SIGNS WERE POSTED.

Employee was repairing a tailgate chain that was wedged between tailgate and truck bed. While prying out, tailgate dropped down and pinched employee's left hand causing a small cut that required 3 stitches.

EMPLOYEE WAS SPLICING THE #8 CONVETOR BELT. AFTER FINISHING THE SPILCE THE EE WAS USING A HAMMER TO SET THE CLAMPS AND STICK PIN. WHILE HITTING ON THE CLAMPS, SOMETHING FLEW UP AND STRUCK THE EMPLOYEE IN THE LEFT CHEEK.

THE OPERATOR WAS ATTEMPTING TO SHUT DOWN THE WASH PLANT WHEN A WASP FLEW OUT. HE JUMPED BACK AND FELL ON HIS RIGHT FOREARM. ON SEPT. 3, 2003 HE INFORMED SUPERINTENDENT AND WENT TO GET IT CHECK ED OUT. THOUGHT TO BE A SPRAIN OF THE SHOULDER, NOW A BICEP RUPTURE AFTER ANOTHER VISIT TO DOCTOR ON SEPT. 11.

EMPLOYEE SUSTAINED AN INSECT BITE PROBABLY IN THE CHANGE ROOM. BITE GOT INFECTED, LEFT FOREARM SWELLED.

EMPLOYEE WAS SHOTCRETING OFF BACK SCISSOR TRUCK. HE HAD JUST HANDED HOSE TO ANOTHER PERSON. WHILE HIS ARM WAS EXTENDED, A ROCK 6 1/2"" IN DIAMETER WEIGHING 2.5 LBS FELL FROM THE ROOF STRIKING THE BACKSIDE OF HIS RIGHT HAND CUTTING IT. EMPLOYEE WAS TAKEN TO THE EMERGENCY ROOM, THE ROCK CUT SEVERAL TENDONS AND BROKE 2 SMALL BONES IN HIS WRIST.

EE WAS PICKING UP TOOLS WHEN HE STEPPED DOWN OFF A CONCRETE PAD WHICH WAS APPROX. 12"" TALL WHEN HE STEPPED DOWN AND PUT WEIGHT ON HIS LEFT LEG HE HEARD A POP. HE HAD IMMEDIATE PAIN WHEN PRESSU RE WAS APPLIED TO THE LEG. HIS LEG WAS SPLINTED & HE WAS TAKEN TO EMERGENCY ROOM. X-RAYS REVEALED A HAIRLINE FRACTURE 4"" BELOW THE KNEE IN AN OLD INJURY. EE HAS APPOINTMENT WITH ORTHO. DR 4/6
