Struck against a moving object

11/10/14 employee notified mgt they hurt their back 11/7/14 while driving haul truck in yard. Employee was taken to medical facility diagnosed with sprain/strain of shoulder/upper arm. Employee could still perform duties of driving truck. 1/15/15 mgt was notified employee had herniated disc and given restrictions making this a reportable injury

Mechanic used rag to check for debris on matting surface of yoke. Metal splinter entered left ring finger. Splinter pulled out mechanic said was fine. On Monday 5/19/14 mechanic reported to plant manager that his finger was hurting and requested medical treatment. Metal splinter was removed by doctor, there was abscess so prescription antibiotic was prescribed

WHILE CLOSING CRUSHER ACCESS DOOR, EMPLOYEE'S LEFT RING FINGER WAS CAUGHT BETWEEN DOOR & STRUCTURE SURROUNDING DOOR. THIS RESULTED IN A LACERATION WHICH REQUIRED 8 STITCHES.

WHILE ATTEMPTING TO LIFT SURGE BIN DOOR FOR REPAIRS, 2ND HINGE BROKE, CAUSING DOOR TO FALL. IN EE'S ATTEMPT TO CATCH DOOR (TO PREVENT IT FROM STRIKING ANOTHER EE) HIS RIGHT HAND-RINGER FINGER WAS SMASHED. FINGER WAS FRACTURE & EE RETURNED TO WORK W/RESTRICTED DUTY LIMITATIONS, THE NEXT WORK DAY.

EE WAS WELDING A CRACK ON EUCLID TRUCK IN SHOP. DURING HIS WORK A METAL SHAVING GOT INTO HIS LEFT EYE. HE WAS WEARING A FULL FACE SHIELD BUT NO SECONDARY PROTECTION. HIS EYE STARTED HURTING WH EN HE GOT OFF WORK THAT EVENING AND WENT TO DR. NEXT AFTERNOON. NO LOST TIME.

EE WAS CLOSING RAILCAR DOORS IN PREPARATION FOR LOADING WITH AGGREGATE. ONE DOOR WAS TIGHT AND DIFFICULT TO CLOSE COMPLETELY. HE WAS ATTEMPTINGTO PRY THE DOORS WITH A PRY BAR AND IT SLIPPED, STRIKING HIM JUST BELOW THE LEFT EYE. 8 SUTURES.
