While changing out gearbox on head pulley of 453 belt. While doing so the employees were adjusting the gearbox on the forks of telehandler when employees finger was smashed in between the motor base and a prybar they were using to adjust the placement of the motor base. Employees right index finger received a laceration and fractured fingertip.

3/2/2026 employee with 2 yrs of exp was dismounting from scaler u/g when they stepped into hole and fell employee tweaked their right knee 3/3 employee was seen at clinic given MRI, 3/24 dr verbally released employee with no restrictions, employee refused to go back to normal job and was terminated on 3/31, on 5/21 company was notified of WC claim with PT and rigid brace.

Employee was removing roof bolts from pushed up scaling pile, after EE placed a bolt into the bucket of the loader EE was operating, employee tripped or lost EE's balance and fell to the ground at the base of the pile striking EE's head on a rock, employee suffered laceration to eyebrow and a broken nose.

Employee had bolted the face up tight then backed out roughly 50', was reloading bolt supplies when a piece of material fell from in between the roof bolts. It struck the cab and broke over the side striking the employee who was on foot next to the cab/engine bay checking the water level. Employee received laceration to their head and leg which were closed at the hospital ER.

Employee was dismounting portable metso jaw crucher ladder to go to the other ladder on the metso and tripped. The employee scraped their left forearm which required 6 stitches.

Employee was climbing the ladder of 605-8 haul truck, while doing so the second rung of the ladder broke free from the left support. This caused the employee to drop roughly 12" to the first rung of the ladder, employee notified the mechanic and the supervisor who replaced the ladder. Employee was given days away from work by clinic.

Employee was walking on flat ground to check a fire extinguisher. EE felt EE's knee twist and notified management. First doctor said it was not work related. The company was notified on 11/23/25 a second doctor deemed it work related. Employee will have surgery 1/6/2026.

An employee lost their footing while installing an access door on a chute. While attempting to prevent a fall, the employee used their left arm to brace themselves while still holding an impact wrench. The resulting force caused a fracture to the employee's left elbow.

An employee fell while dismounting their loader. The employee landed on their back in a sand pile. The employee's initial pain was a 3/10. After a week of self-care treatment, the employee was not improving, so EE was seen at a clinic on 7/21. X-rays indicate that the employee has a compression fracture on their L2 vertebrae.

A pit loader operator sprained their left ankle while dismounting their equipment. The employee was initially evaluated at the clinic on 7/7 and released to continue full duty with no restrictions. The employee was still experiencing pain on 7/21 and went back to the clinic, where they received restrictions and were prescribed physical therapy.

Right pinky finger got pinched between the spacer and the housing of the bearing unit. While trying to remove the lower shaft and bearing on a double screw material washer. Received 1 stitch at ER to close up laceration.

A set up crew employee was attempting to remove liners from a cone crusher, by striking the liners with a 10 lb sledgehammer. During this process, a shard of the manganese liner chipped off and went into the employee's left forearm. The employee was taken to the emergency room where the shard was extracted, wound was cleaned, and 5 stitches were administered.

Employee was assisting another employee in cleaning drink coolers; the employees were using a pressure washer to do so. The injured employee reached to catch the cooler as it was falling out of reaction and was struck on the top of the hand by the water stream causing laceration to their left thumb. Employee was taken to ER where they received 6 stitches.

EE stated they fell injuring a shoulder but when asked could not state how they fell, over a chalk ball or their own feet. There where employees within 100 feet of them at that time that did not see them fall. Once taken to the clinic the clinic informed them the issues with their shoulder from birth and not work related.

The employee fell 32 inches from the tracks of a portable crusher, landing on their left side, fracturing their left distal fibula.

EE was walking a man lift down a pit road when the tires hit a 3-4 inch deep washed out rut caused by rain while traveling 2-3 feet off the ground. The rut caused the basket to bounce causing EE to be lifted into the air, when EE came down EE's right foot landed on the throttle assembly causing EE's right knee to twist.

Pliers slipped and hit employee in teeth breaking the two front teeth.

The screen needed more oil and to be greased. The injured employees added oil. The employee that got injured decided to put their left hand between the timing belt and sheave to check if the bearing was "locked up," the counter weight fell freely pulling their glove and left hand between the timing belt and sheave. Injured employee had 3 fractured fingers.

An employee arrived early to work and waited outside the closed gate. Around 5:30 AM the foreman arrived with the gate key. The employee got out and assisted the foreman on opening the gate. On the way back to their personal vehicle, the employee slipped on the gravel and fell onto the asphalt, breaking their right hip. The employee was not clocked in and not wearing work boots.

Employee was splicing a belt using a battery operated drill, while tightening one of the nuts down it bottomed out. The torque from the drill wrenched from their hands, the drill spun around and the battery struck the employee in the jaw chipping a molar. There was no bruising or laceration. They went to the dentist where a crown was installed on the tooth.

Employee was mounting an excavator to fuel it and as they stepped onto the tracks, they tore their left meniscus in their knee.

Employee was dismounting haul truck when employee lost footing. The employee caught self causing a labrum tear in left shoulder.

Worker was removing a high-pressure filter cap from a machine using a breaker bar. It slipped and crushed two fingers on worker's left hand between the breaker bar and hydraulic tank on the machine.

Employee was adjusting the primary jaw crusher. The pitman got hung on the cheek plate and did not come back with the toggle plate like it should have. The employee placed their hand on the spring bolt to observe what was catching the pitman and the toggle plate fell on the employee's right hand, causing a laceration that needed stitches.

Employee was working in the plant whenever EE stated EE felt that EE was having chest pains, EE contacted the foreman who then contacted the plant manager. Managment then called an ambulance, and the employee was taken to the hospital. The employee stated that EE had suffered a stroke about 3 years prior and was on medication, which EE had not been following the recommended use of.

Operator was making a hard left turn up a ramp when the right front tire caught traction and pulled the cab over on the right side of machine.

The employee was tied off, standing on angle iron, cutting bolts out of a wash screen with a torch. As they turned to step down, employee felt a pop in their right knee. Diagnosed with a right knee strain.

Employee lifted a chute door to put it back into place (estimated 40lbs) and tore left bicep.

The emergency escape hoist basket fell to the bottom of the shaft after the hoist rope broke. The basket was damaged beyond usage, the main hoist rope was broken and had to be replaced as well. It was found during the investigation that the rope had jumped the sheave wheel and was rubbing on the bearing housing causing wear/damage to the rope.

Bolts on weather head of motor broke. While repairing the motor an employee's finger was caught by a wire while tightening a bolt with an impact wrench causing the finger to be degloved just below the fingernail after the employee jerked the finger back.

Worker was transferring an oxygen cylinder from one service truck to another. When lifting on the cylinder, EE felt pain in EE's back. A muscle was strained on the right side of EE's back.

Employees were replacing a tail pulley. It dropped into place unexpectedly and the shaft fell onto the tip of the employee's ring finger on their right hand severely mashing it. The fingertip was later amputated at the first joint behind the fingernail, at the ER.

Changing out cylinder in JD 824 employee got tips of fingers between the mount and the cylinder cutting off tips of ring finger and middle finger.

Underground blaster was clearing a lower hole of cuttings. Before EE started loading the hole, the employee was using right hand to clear the hole and using left hand to support body weight. A rock of unknown size fell from the face making contact with EE's left thumb smashing it between the rock and the wall, causing an open fracture to EE's left thumb.

An employee was using a handheld die grinder to widen the center of a spool, when it kicked out of the hole and caught the employee's gloved left ring finger. The left ring finger was amputated down to the highest knuckle.

Employee was mounting the yard loader and in the process of pulling body weight to the first step of the ladder employee's right hand slipped causing the employee to fall to the ground from approximately 24" high landing on employee's rear and injuring lower back and wrist.

Loading a shot, was struck in the head by the moving Auger Tube. (recordable after the appointment on 2/13/24)

At approximately 3:05 PM EE was walking from the plant to the C-9 Belt area when EE passed the Oversize chute a 1 1/2 -2 inch rock bounced out of the screen 2 chute and hit EE on the left thumb. A splint was placed on EE's left thumb to minimize movement. Restrictions given from clinic were no typing, pushing, pulling and to wear splint.

Splicing a conveyor belt. Box Knife slipped and lacerated EE's thumb. Taken for medical care where EE received 4 stitches.

An employee placed the handheld battery powered grinder they were using to change the bearings on a tail pulley between their body and left arm, when it turned on and lacerated their upper arm.

Employee hit left thumb with a ballpein hammer, resulting in smashed tendons in the thumb.

An underground blaster was cutting an ANFO hose with a utility knife when the knife slipped, causing a laceration on the employee's left index finger.

Employee was reversing down from the 1/4' pea gravel stockpile ramp in the JD844L loader with an empty bucket. The bucket was about midway up. When the rear tires contacted the grade change at the bottom of the ramp it jolted the loader causing the air ride seat to bottom out causing pain in the left hip area.

The Roof Fall happened in an area that was worked out, and was being set up for benching floors, the fall happened at the 9.5 feet height in the back/roof which is above anchorage of our roof support. Through investigation with MSHA ADM, Roof Specialist, and Field Office Supervisor it is not immediately known what caused the fall. The fall was reported at 2:40pm.

While hauling stripping equipment, the haul truck driver contacted a dip in the road which caused the seat to bottom out. The driver experienced back pain, and was diagnosed with back spasms.

Employee parked truck part way on ice. Got out and saw the ice but still walked behind the truck on ice and fell getting a wrist sprain. There were areas without ice.

While attempting to clear a blockage in an ANFO hose, a small amount of ANFO and compressed air hit the employee in the left wrist causing a burn/abrasion. After the employee's second follow-up appointment on 12/15, EE was given days off, making the injury reportable.

Employee was attempting to climb up onto the bumper/landing of the back of the service truck when their foot slipped, and they fell to the ground. The fall resulted in a compression fracture in the lower back and a break to the foot.

Employee walking down slight grade looking at beltline when they dragged their toe as the grade transitioned to a flatter surface, causing them to trip. Employee caught themselves on the ground with their left hand causing a laceration that required stitches to close.
