Showing all 23Miner was working to repair a crusher using parts cleaner. Parts cleaner and oil from the crusher got onto EE's pants. When EE used a grinder to cut steel sleeve, sparks ignited EE's pants and EE's knees and calves were burned.
Miner was stacking stone coming out of the hydraulic splitter. As employee was picking up a piece of stone from the conveyor belt, the belt reversed and pinched a finger between the stone and part of the machine.
Miner was operating a backhoe equipped with a rock hammer. After several hours of operation, ee began to feel numbness in right arm and right hand.

This incident is under investigation for possible workers compensation fraud.
Worker was removing nuts using and air impact wrench and his right wrist became sore. He did not report the injury to management until 05/26/2009. Went to a clinic and was placed on restricted duty. No lost time.
Operator was cutting stone using a guillitone cutter. When he cut the stone, his L hand was under the back of the stone. The stone fell backwards on his hand, causing a fracture to the middle finger of his L hand. No Lost time.
Worker was repairing a air-powerd greaser; the piston became stuck. When he tried to free the piston the fingers of his left hand became trapped by the piston causing a laceration to the middle finger of his left hand requiring stitches to close the wound.

Employee was cutting the stone and I turned around and it cut his finger in the middle of the left finger of the left hand.
Stone fell on right forearm while employee was using a splitter, causing a laceration to his forearm above the wrist (underside).
EE WAS SANDBLASTING AND ANOTHER EMPLOYEE WAS MOVING A TUB OF STONE AND HIT HIS IN THE HIP AND SHOULDER. DR DIAGNOSIS STRAIN AND ABRASION.
Employee was cutting some steps and half of the stone fell onto the top of his left thigh.
HE WAS ON HIS MACHINE AND IT MALFUNCTIONED...THE BLADE CAME DOWN TOO QUICKLY. WHEN HE GOT HIS HAND AWAY FROM THE BLADE HIS LEFT INDEX FINGER GOT TRAPPED BETWEEN THE STONE AND THE HEAD OF THE M ACHINE. THAT TOOK THE TIP OF HIS FINGER OFF.
HE TOLD HIS SUPERVISOR THAT HIS BACK JUST STARTED HURTING.
EE STARTED THE AIR COMPRESSOR AND A PARTICLE CAME OFF THE EXHAUST AND FLEW IN HIS RIGHT EYE. THE CENTER OF HIS RIGHT EYE WAS SCRATCHED BY THIS MATTER.
WAS WELDING AND TWISTED WRONG AND HIS BACK PULLED. HURT.
HE WAS WORKING WITH A GRINDER ON A PEDESTAL & HECUT HIS LT. PINKY (TOP). WE SUGGESTED HE GO & GET IT CHECKED OUT BUT HE REFUSED TREATMENT.
WAS CUTTING ON MACHINE, WHILE MOVING A STONE, HE CAUGHT HIS FINGER BETWEEN THE STONE AND THE MACHINE.
LACERATION AND FRACTURE TO LEFT HAND FINGER. PINCHED FINGER BETWEEN TWO STONES WHILE WORKING ON A GUILLOTINE.
EE WAS GRINDING IN A REAL TIGHT SPOT AND THE GRINDER (9" HAND HELD GRINDER) KICKED BACK AND HIT MY LEFT KNEE. SOME SWELLING AND HURTS WHEN HE BENDS THE KNEE BUT IF HE STANDS UP STRAIGHT DOES N OT HURT.
EE AND OTHER WERE WORKING ON A MACHINE AND HE GOT HIS LEFT PINCKY FINGER CAUGHT HE CUT THE TIP OFF.
