Employee found debris on the under belt scraper of the head pulley coming out of jaw crusher (7158) and attempted to clean it without de-energizing the piece of equipment first. This resulted in the belt grabbing the 3 foot pry bar and slamming EE's hand into the scraper and framework of the under belt. This resulted in 2 broken bones in the Right arm and hand.

Showing all 50Miner was using a cutting torch to cut a catwalk support. While making the cut, a piece of cutting debris went into miners Right eye. Miner treated at clinic, prescribed eye drops. Returned to work with no time loss.
DUMP BOX OF HAUL TRUCK BEING OPERATED BY MINER WAS JOSTLED BY EXCIVATOR BUCKET WHILE LOADING MATERIAL. MINER REPORTS NECK AND BACK PAIN.
While being loaded by the shovel, the shovel made contact with the tip of the haul truck bed causing haul truck operator to be jolted forward in the seat resulting in head, neck and back pain.
Team members were picking up a sand screw equipment off a trailer with a crane. One of the rear rigging legs was hung up on a bolt/stud. Team member went to correct the rigging. As EE's hand got near the chain it released tension, taking EE's hand with it towards the sand screw frame rail, pinning it between the chain and the frame. Tip of right index finger was fractured.
EMPLOYEE INSPECTING BLOW BARS ON IMPACT,ELBOW STRUCK THE BREAKER PLATES AND ROTOR COTINUED TO ROLL. THIS CAUSED BREAK IN RIGHT WRIST.
WHILE PERFORMING MAINTENANCE ACTIVITIES, PNEUMATIC HOSE ATTACHED TO IMPACT GUN SEPARATED AT UNION. COUPLER AT END OF HOSE STRUCK TM IN RIGHT CHEEK AND MOVE BETWEEN EYE AND EYE PROTECTION. TM WAS EVALUATED AND PRESCIRBED MEDICATION
Laceration to the non-dominant hand, Right ring finger approx 1/2 inch in length. 7 stitches. Employee was unbolting a screen deck with a leather glove and battery operated impact tool. As EE was removing the bolt, the bolt spun and it had a small metal bur on the end. Which spun and cut the glove and the employee's finger on the distal digit (tip) of finger.
Employee was working on Impact Crusher when rotor turned pinching right hand between blow bar and side of crusher.
Miner was operating long stick excavator over a period of time and felt weakness, muscle burning and loss of feeling in right shoulder and arm. Miner lists the date of injury as early September but did not report it to employer until 12/05/2018 and first sought medical treatment on 12/15/2018. Attending physician has listed injury as right arm pain and hand numbness.
ON 8/16/18, MINER REPORTED EXPERIENCING SEVERE BACK PAIN AFTER SITTING IN A CHAIR WHILE OPERATING THE DREDGE ON 8/14/18, FOLLOWED BY A 12 HR SHIFT PERFORMING MAINTENANCE ON 8/15/18. MINER INITIALLY REPORTED TO SUPERVISOR AND A GROUP OF MORE THAN FIVE OTHER MINERS THAT BACK PAIN WAS NOT RELATED TO WORK CONDITIONS.
The employee attempted to stop a pulley and belt system with employee's left hand.
EE was retrieving a rock splitter tool from a rock when the rock separated, causing the tool to move EE's hand against the side of the man basket and when the rock separated, the man basket moved causing EE to hit right knee on the side of the man basket. EE declined medical attention on 6/14/16, EE called in from a scheduled day off on 6/21/16 requesting to see a doctor.
Injured employee was removing bolts from a belt frame on a crusher screen. He had his arm underneath the frame piece when the final bolt was removed the frame piece fell pinching his arm between the piece and the box. He went to the emergency room and was given a prescription and released back to full duty.
Running a half inch Drill Motor the bit caught and the drill twisted around and fractured the employees right hand,
Was using hand to try and hold a bolt on a cutting edge to keep into place. The air gun spun the bolt and lacerated his hand. The employee was wearing gloves.
dust and metal grindings blew into eye of miner while repairing conveyor belt. Miner was wearing safety glasses and face shield, but should have worn goggles due to windy conditions, and postioned himself upwind of the work area.

Mine operator sitting in seat of excavator turned to see what other miner wanted as he had yelled. When he turned in the seat coat sleeve caught swing lever and swung excavator into other operator's foot.
Employee was manually handling a spray bar when his thumb was pinched between the spray bar and a screen. This action caused his thumb to be lacerated as well as fractured.
Employee was adding a drill rod onto the rock drill and it fell off and smashed his foot breaking the toe.

While grinding on a material screen plant a foreign object got into his right eye. He didn't notice it until 5:00 in the evening when showering. He could feel something in his eye and reported it the following day at work and went to the doctor and a piece or rust was removed, he has a corneal abraison.

Employee was using a hand grinder. He was holding a brass fitting with his left hand. The grinder slipped off the fitting striking him on the left ring finger. Three stitches; updated tetnus.
EE WAS WELDING AND A PIECE OF SLAG GOT INTO HIS EYE
Holding a carrage bolt, using an air gun to remove the nut, the bolt spun around and cut his finger.
EE WAS UNDERNEATH CONVEYOR, GRINDING OFF RUST ON CONVEYOR FRAME. EE GOT A PARTICLE IN HIS LEFT EYE. (PARTICLE OFF CONVEYOR FRAME WAS EXTRACTED WITH FORCEPS.)
Laborer prepared a piece of angle iron to cut with cutting torch. As soon as he put heat to it a foreign object popped and hit his cheek under his safety glasses and the object fluid p in his right eye. shoulder had a face shield over his safety glasses or welding goggles.
EE HAD DUST ON HIS COVERALLS AND HARD HAT. AS HE WALKED BY THE BACK OF THE GEN SET, THE DUST BLEW INTO HIS FACE FROM THE FAN OF THE ENGINE.

EMPLOYEE WAS PLACING A BIT IN A DRILL WHILE PLUGGED. THE TRIGGER STRUCK THE COUNTER TOP AND THE DRILL SPUN WHILE THE CHUCK WAS STILL IN PLACE. THE CHUCK SPUN UP AND CUT THE EMPLOYEE'S FINGER.
HAVING TO CUT OVERHEAD IN A CONFINED AREA.
EE WAS CUTTING SOME PLASTIC WHEN THE SAW HE WAS USING KICKED BACK, CUTTING THE SKIN ON HIS STOMACH.
WHILE GRINDING A PIECE OF METAL WITH A HAND HELD AIRDISC GRINDER, DISC CAME APART WHEN IT CAME IN CONTACT WITH METAL CUTTING LEFT ARM.
INDIVIDUAL WAS USING A DOZER TO RIP FROST CHUNKSFOR THE SCRAPER OPERATORS. WHEN GOING ACROSS THECHUNKS THE DOZER FELL TO ONE SIDE SHIFTING THE OPERATOR INTO AN UNUSUAL POSITION CAUSING DAMAGE TO HIS BACK EVEN THOUGH HE HAD HIS SEAT BELT ON.
THE EE WAS USING A TORCH TO MELT SNOW FROM THE HEAD PULLEY ON THE NEAR PIT CRUSHER WHEN THE PULLEY CAUGHT THE TORCH AND FORCED HIS HAND TO SLAM TO THE SIDE. HITTING A PIECE OF ANGLE IRON.
THE WELDERS WERE IN A VERY CONFINED AREA GRINDING OUT A WELD THAT HAD JUST BEEN LAYED ON A 1570 DRAGLINE. THE FACE SHIELD BEING USED DID NOT EXTEND BACK FAR ENOUGH AND THE SHARDS FROM THE GRIN DING FLEW IN BEHIND THE GUARD AND LODGED IN THE CORNER OF HIS EYE. THE METAL HAD TO BE EXTRACTED BY AN OPTOMITRIST.
SPROCKET & TENSIONER CAME LOOSE & FELL UPON WORKER (TWO METAL PARTS FORM DRILLING MACHINE).
SUDDEN JOLT BY HITTING CONCRETE SLAB ON ROAD WITH GRADER BLADE. ACCIDENT OCCURRED ON 5/5/00 AND REPORTED TO SUPERVISOR ON 5/12/00.
EE POSITIONED A STEEL BEAM TO WELD IT AND IT SPRANG AWAY CUTTING THE TOP OF HIS HAND. HE WAS WELDING A GUARD RAIL AT THE HOPPER.
