Placing cat walk on fuel tank to avoid falls while checking fuel levels. Cat walk slipped falling to ground before secured. Employee fell to ground with cat walk.

Miner did not report to work 9/19/2012. Called supervisor from hospital saying he broke/sprained ankle on 9/18/2012 while changing screens. Did not report injury day of occurrence. Sprained ankle was confirmed. Miner missed work on 9/19/2012. Returned to work on 9/20/2012. Suspicious as to being a work related injury.

Miner slipped off of 3rd step (from bottom) of ladder while ascending. Fell on left shoulder. Dislocated left shoulder. Ladder was not in disrepair or slippery.

While fueling Cat D9L dozer, the quick connect fuel valve on the dozer did not close when the service truck fueling hose was removed. Diesel sprayed on the employee and got in his right ear. The diesel chemically burned the employee's right ear canal. No permanent damge resulted.

Employee lifted too much weight at one time causing a hernia in right groin. Employee notified his supervisor on 10/1/05 of the injury and that he would be going to the doctor. Paperwork not received in company safety office until 10/10/05. Steve Pilling at Green River, WY MSHA office notified of incident circumstances on 10/11/05.
While refueling equipment, employee had his hand in driver's door door jam of the fuel truck. The wind blew the door shut crushing his right hand. This accident was discovered on 10/11/05 as being not reported to MSHA in the required time limit during a self-audit for the 3rd quarter Form 7000-2. Green River, WY MSHA office was notified on 10/11/05.
Miner was replacing shocks on a pickup when he felt a pain in his chest. Miner was not lifting heavy objects, did not strike an object with his body, nor did he make an awkward movement. Pain did intesify with left arm motion.
