Struck against stationary object

While walking in the plant area, miner stepped on a rock, about 4"" round, and felt a strain and pop from left knee. Ground was wet and muddy at the time.

Struck by flying object

Miner was using a cutting torch to cut a catwalk support. While making the cut, a piece of cutting debris went into miners Right eye. Miner treated at clinic, prescribed eye drops. Returned to work with no time loss.

Struck against a moving object

While being loaded by the shovel, the shovel made contact with the tip of the haul truck bed causing haul truck operator to be jolted forward in the seat resulting in head, neck and back pain.

Employee crawled into a triple deck incline screen to change the poly deck screens. Employee was using a 2lb hammer to adjust the screen when the hammer bounced back hitting EE above EE's right eye.

Strain due to jarring motion from impact of material being loaded in dump bed of 50 ton haul truck. Injury initially reported as first-aid but worsened over time and advanced to seek medical treatment.

Employee using their hands to attempt to manually adjust a wear plate being installed in a hopper when the employee felt a tear in their middle finger on the right hand.

Employee was using ratchet wrench to tighten chute lock chamber when socket slipped from nut causing the ratchet handle to hit employee in the mouth cracking a front tooth.

Employee was walking and transitioned from a paved surface to an unpaved surface and stepped into a hole injuring lower leg.

Fall from machine

Clear weather, good lighting. Employee had finished changing a tire and was putting dismounted tire onto lift gate. Mis-stepped and fell from lift gate, landing on EE's back on the ground. Received a skin laceration, which received stitches at Tripler Arony Medical Center.

During welding activities on a catwalk, the employee strained knee when repositioning self while kneeling.

Struck against stationary object

Was walking on work platform stepped around a ladder and hit back on the stair railing causing employee to jump forward out of reflex and hit knee on the side of ladder. Foot hit bottom of stairs resulting in employee twisting knee.

Absorption of radiations, caustics, toxic and noxious substances

Driver was climbing down parked haul truck at end of shift when EE was stung on the left pinky finger by unknown insect that became symptomatic the following shift and resulted in an infection.

Over-exertion in pulling or pushing objects

EE was washing debris from walkway. EE was pulled on a hose that had aggregate on it and EE injured lower back.

Shop ee was checking roller and slipped on piece of rubber. EE informed supervisor that he was not injured and refused first aid or medical treatment. Safety or management were not notified that it was a recordable and required treatment until the evening of 1-20-2014. Ee is being referred to a physical therapist for physical therapy-treatment of an injured ACL.

Employee was manually handling a spray bar when his thumb was pinched between the spray bar and a screen. This action caused his thumb to be lacerated as well as fractured.

Employee was driving a haul truck when he hit a dip in the road which caused his seat spring to compress and hit the floor in the cab causing a shock to his back.

Employee was walking on wet concrete, when he slipped, causing him to fall to the ground and landing on his buttocks, hip and back.

Struck against a moving object

Employee was operating a 988H loader to load a haul truck with aggregate materials. The ground the loader was operating on was unprepared and uneven. While driving loader on this terrain the employee reported his lower left back started to ache from the bouncing and jerking around inside his loader.

The weather on the day of the incident was hot with temperatures in the mid to high 80s with little breeze and brief periods of rain. Believe the hot humid weather contributed to the employee fainting. Injured contractor's injuries included dislocated right shoulder, hairline fracture in right shoulder and minor cuts and abrasions to contractor's head and face.

Tied a rope round a rock to lift it. The rope slipped and the rock fell on his toe.
