WHILE PERFORMING MAINTENANCE ACTIVITIES, PNEUMATIC HOSE ATTACHED TO IMPACT GUN SEPARATED AT UNION. COUPLER AT END OF HOSE STRUCK TM IN RIGHT CHEEK AND MOVE BETWEEN EYE AND EYE PROTECTION. TM WAS EVALUATED AND PRESCIRBED MEDICATION

Employee was stockpiling in same pile with another loader. As employee rolled into the pile to dump the bucket there was an uneven layer of material the employee did not notice which caused the loader to bounce resulting in an acute cervical myofascial strain to the neck.

During the lateral adjustment of a set of forks on a Cat TL1255 Telehandler, the worker cut his left pointer finger while he was pulling and another worker was pushing one of the forks. The fork suddenly slid quicker than was expected, catching his finger between one of the hooks and the bar. He was wearing gloves at the time. his finger received nine (9) stitches.
