Employee was trimming excess aluminum honey comb off of panels using 10lb Sawzall. Process involves a lot of bending, twisting which caused back strain. Injury was not reported until 8/21/25 when they were treated for back strain.
Employee was on top of granite block, EE was drilling block to split in 1/2. When the block split, it shifted and EE dropped to EE's knees. EE's left shin hit the edge of the block causing abrasion to left shin. EE was released to work, modified duty- should be sitting 50% of time, no squatting, no climbing ladders. Employee returned to work 5/7/24.
Employee was starting to drill hole in the granite, was looking to see how it was started, and something flew in EE's right eye. EE went to clinic later as eye continued to be irritated, they sent EE to ER. The ER doctor said not an emergency, prescribed eyedrops. Employee returned to work, no restrictions, no lost time, no lost wages.
Employee was holding pendant with left hand, using right hand to wedge head of the boom on the rock buggy so it wouldn't damage unit over the weekend. Head of boom smashed finger while employee was using pendant to let it rest on the wedge. Laceration right index finger, sutures. No lost time.
Employee was wedging a block while standing on block already wedged. When the line split, it caused chain reaction of rough backs to fall, causing employee to go over the edge of the block. EE fell 3 to 4 feet, causing dislocated right shoulder & inferior glenoid fracture, also abrasions to both forearms. Shoulder was re-set, employee's arm is in sling. As of now, not lost time.
Right thumb was caught between table and piece of iron while trimming to size.
Employee was operating derrick, applying brake of boom to keep load in control. Could feel pressure under knee cap, right knee got swollen. Employee continued to work, knee cap continued to bother him so he saw doctor on August 7th, released no restrictions, returned to work, no lost time. However, was given a prescription for medication.
Employee was scrapping an old 475C Loader, cutting what's called a "dog bone" which supports the quick tach. When cut through, quick tach fell about 6", pinning employee's left foot underneath. It smashed his steel toed boot and fractured four of his toes on his left foot. Scrapping metal was new to employee, mainly runs trim drill @ Charcoal.
Employee was using a blow pipe to clean off an area for lift holes. He was holding pipe with his right arm, also doing some shoveling. He pulled muscle in right arm/shoulder area. He thought pain would go away, it didn't. He went to chiropractor the next day after work and then again the next week.
Employee was pressure washing two engines, something flew up into his right eye and he could not remove it by flushing it. He saw a nurse/doctor and had it flushed, was given a prescription and he returned to work. No lost time.
EE was grinding the edge of surface plate. He set the grinder down, while it was still running with grinding wheel up. When he removed his hand it hit the turning wheel.
Employee was allegedly pressure washing loader, hose got stuck, while getting hose free, spray nozzle was spraying on his foot, heat burned left big toe. Employee returned to work, then on 9-8-05 could no longer wear his boot so is off work until 9-12-05
DOOR ON EXCAVATOR SWUNG OPEN AND HIT EE IN THE FACE. BROKE EE'S TOOTH ON RIGHT SIDE AND LEFT OF MIDDLE UPPER.
Employee was drilling in quarry - steel fell from hammer on the boart drill and onto his left foot, breaking bone in his foot
Employee was welding and when chipping slag, a piece flew in left eye, burning his tear duct. Employee saw a doctor and was prescribed ointment - he returned to work later that day - no restrictions.
EE WAS WELDING IN THE BED OF A GROUT TRUCK BOX WHEN HE WAS FLASHED FROM THE AUTOMATIC TRACK WELDER, CAUSING FLASH BURNS TO BOTH EYES. HE SAW DR. ON 5-7-03, THEN TOLD HIS SUPERVISOR ABOUT IT LA TER. DR. SAID HE COULD WITH NO RESTRICTIONS BEGINNING 5-12-03.
EMPL;OYEE WAS WORKING IN QUARRY WITH A DERRICK TO MOVE A GRANITE BLOCK. THE BLOCK SWUNG AND PINCHED HIM BETWEEN THE BLOCK AND GUARDRAIL. LOWER BACK CRUSH INJURY.
EE REACHED DOWN THROUGH JAW GUIDE ON SLOT DRILL TO PULL ON AIR HOSE AND GRABBED THE WRONG LEVER, WHICH CLOSED JAWS ON HIS LEFT ARM, CRUSHING HIS ARM AND BREAKING THE BONE.
HE WAS CLEANING OUT DRILL HOLE WITH AIR PIPE, A SMALL ROCK HIT HIS LOWER EYELID CAUSING IT TO GET SWOLLEN & IRRITATING.
EE ALLEGES HE WAS WEDGING STONE, COULD FEEL HIS BACK ALL TIGHTENING UP BUT JUST THOUGHT IT WOULDLET UP ONCE HE FINISHED FOR THE DAY. EE DID NOT COMPLETE A FIRST REPORT OF INJURY UNTIL 2-11-02, AT THE SAME TIME SUBMITTED A CLAIM FOR SHORT TERM DISABILITY (NON-WORK RELATED INJURY). COLD SPRING GRANITE DIDN'T RECEIVE REPORTS OF INJURY UNTIL 2-25-02.
EE WAS RUNNING A GRINDER TO POLISH EDGE OF GRANITE, THE GRINDER SLIPPED, CUTTING HIS LEFT MIDDLE FINGER KNUCKLE. HE NEEDED 3 STITCHES.
EE WAS PUTTING GRAB HOLES IN STONE WITH JACKHAMMER, FELT SORENESS IN LOWER BACK. COLD SPRING GRANITE WAS NOT AWARE EE WENT TO THE DOCTOR UNTIL 5-22-01.
EE WAS GRINDING ON METAL ON BACKHOE BUCKET AND APIECE OF METAL ENTERED UNDER HIS SAFETY GLASSES AND GOT STUCK IN LEFT EYE. HE DIDN'T SEE A DOCTOR UNTIL 4/24/01.
EMPLOYEE WAS PULLING OUT STRUCK DRILL STEEL WHEN IT CAME LOOSE IT HIT HIM IN THE MOUTH CHIPPING HIS TOOTH.
EE WAS GETTING READY TO DRILL HOLES IN A PIECE OF STEEL. HIS GLOVE GOT CAUGHT BY DRILL STEEL. HIS LEFT LITTLE FINGER WAS TWISTED AROUND BIT AND WAS CUT, REQUIRING SUTURES.
EE WAS PRESSURE WASHING BACKHOE TRACKS OFF TO SEND BACKHOE IN FOR REPAIRS. WASH SPRAYED GRAVEL BACK INTO HIS MOUTH. HE BIT DOWN ON GRAVEL AND BROKE/CRACKED HIS TOOTH ON THE BOTTOM LEFT SIDE OF HIS MOUTH.
EMPLOYEE WAS GRINDING BURRS OFF SEAM WEDGES WITH A BENCH GRINDER. HE WAS APPLYING PRESSURE TO THE WEDGE, IT SLIPPED OFF GRINDING WHEEL AND WHEEL CUT TOP OF HIS LEFT HAND, REQUIRING STITCHES.
EMPLOYEE PUT HIS GLOVED HAND ON TURNING STEEL, ATTEMPTING TO STEADY IT. STEEL CAUGHT GLOVE AND TWISTED HAND AROUND STEEL, CAUSING LEFT THUMB SPRAIN.
