Employee was removing saw tower from saw rail, after removing it his left shoulder started to hurt. He saw doctor 10-22-10, was given prescription for muscle relaxers, also instructed on home exercise. No restrictions, no lost time.

Employee was setting up grove drill on a slab. He swiveled the drill head a little and the drill head fell off the boom. The employee jumped off the slab to get out of the way so the drill head wouldn't fall on him, and he fell on his left arm, fracturing his forearm.

Employee was lowering a water pump into the water at the quarry when the jack handle slipped and came around, struck his left thumb, breaking it.
Employee was walking on quarry floor which was slippery with ice. His left foot slipped outward. He did not fall but strained something in his left knee. He reported incident right away to his Supervisor but didn't see a doctor until 1-16-07 as pain wasn't getting any better. His doctor referred him to physical therapy.

Employee was hammering on a chisel when a metal sliver from the chisel head came off and struck his left arm just below the wrist. Employee was given one stitch and returned to work, no lost time. Employee normally works at the Rockville Shop, was temporarily working at Milbank.
Employee was climbing down ladder, ladder slid out on ice, he and the ladder ended up flat on ground. He had right shoulder pain and didn't think is was so bad at first, after a few days saw a doctor. Cold Spring Granite found out 2-2-06 he was issued a prescription. He is working and is not on any restrictions at this time.

Employee was lifting exhaust manifold when his hand slipped off to side and bent back his finger. Sprained right little finger. Needs to follow up with hand specialist.
EE was climbing stairway out of quarry. When EE got to top he tripped over a rope and fell, hitting his left knee on ground - left knee bruise. Treated at doctor on 5-20-05 and was released to work - no lost time.
Employee was lifting brake shoes on 475C Michigan loader and felt pressure in lower stomach/groin area. He saw a doctor on 2-1-05 and had hernia surgery on 2-8-05, which was the first day he was unable to work - began losing time.

Employee was preparing to put hydraulic pump in vice. It slipped and fell on his left foot, breaking his large toe and the one next to it.
Employee was pounding stakes into the ground to hold forms straight. He was setting forms to pour concrete for sidewalk at main office and strained left shoulder. He didn't see a doctor until 9-29-04, then saw a chiropractor a couple of times.
Employee was crawling around under truck, twisted neck. Employee did not realize he was treating with chiropractic a lot until employer received billing statements from chiropractor.
Employee received letter from Sandler Occupational Medicine indicating possible silicosis.
Employee was drilling in quarry - steel fell from hammer on the boart drill and onto his left foot, breaking bone in his foot

EE WAS DOING MAINTENANCE ON GROUT TRUCK. HE HAD FIBERGLASS HOOD UP AND THOUGHT IT WAS LATCHED, IT WASN'T AND IT CAME DOWN ON SIDE OF HIS HEAD, TWISTING HIS NECK.
EE WAS CHECKING BATTERY, SWITCHING LOAD TESTER FROM ONE BATTERY TO ANOTHER AND SPARK MUST HAVE CAUSED BATTERY TO BLOW UP, GETTING FOREIGN BODY IN LEFT EYE AND A LITTLE BIT IN RIGHT EYE, REQUI RING PRESCRIPTION MEDICATION TO TREAT.
EMPLOYEE DOES COMPUTER WORK AND DATA ENTRY ALL DAY LONG - EVERY DAY. HAS PAIN IN LEFT FOREARM, TIGHTNESS, TINGLING IN FINGERS, NUMBNESS IN FOREARM AND HAND. EMPLOYEE STATES THIS HAS BEEN COMIN G ON SINCE 3/17/03, SHE SAW A DOCTOR ON 4/15/03.

EE DROPPED A BIT DOWN NED JET CHANNEL AND TRIED TO RETRIEVE IT WITH A LONG PIECE OF STEEL. THE UP AND DOWN MOTION WITH THE STEEL STRAINED HIS CENTER AND LEFT LOWER BACK. HE RETURNED TO WORK ON 6-15-00, WENT TO DR. 6-16 AND BECAME LOST TIME ON 6-20.
EE WAS USING HAMMER TO KN0CK TEFLON OUT OF INSIDE OF DRILL BIT, MISSED THE BIT AND SMASHED RIGHT THUMB, REQUIRING STITCHES.
EMPLOYEE WAS DRIVING LODER, WENT ACROSS ROUGHT SPOT IN ROAD, LOADER BOUNCED BADLY BECAUSE OF WORN CENTER PIVOTS, CAUSED LOWER BACK STRAIN.
EE WAS CARRYING A LADDER, DID NOT SEE A PIECE OF GROUT AND TRIPPED OVER IT, LANDING ON HIS KNEE.THIS CAUSED EFFUSION TO LEFT KNEE.
