Over-exertion in lifting objects

Employee was trying to lift and move a large piece of granite, approximately 110 lbs., without the use of lifting equipment. EE felt a pain in right side of back, burning knot. Sought medical treatment, taken off work. Lost time injury.

Employee alleges injury is left shoulder, said doesn't know what happened, woke up one morning & it hurt. Completed First Report of Accident 11-4-2014, he stated was working at New York quarry, standing drill steel up, steel slipped, grabbing with left arm pulled muscle. Also added he'd been treated at chiropractor two times in October.

Employee was lifting a 5 gallon pail of oil up and over the tailgate into back of truck. He strained his right shoulder. Didn't see the doctor right away, thought it would get better. Strain continued to bother him so he went to clinic.

Employee is the Corporate Quarry Manager - he was walking around the quarry, measuring and planning for 2010, a tree branch flew back and hit him under his glasses, scratching his left eye. Left eye corneal abrasion - required a prescription. No lost time from work.

Employee was pulling up sump pump out of saw pit, strained his lower back. Didn't think it was much at first but was in a lot of pain the next morning so went to doctor and received a prescription.

Employee was separating slabs, steel bar slipped and stone fell back onto bar which in turn jerked and pulled employee against direction he was pulling. This caused strain to right arm/shoulder and to right mid back directly below shoulder blade.

Employee was getting off of tractor, his pant leg got caught on brake and he fell, hitting ground, hurting left shoulder. Employee continued working. He had left shoulder surgery on 9-26-06, therefore starting losing time from work.

EMPLOYEE WAS LOOKING AT WATER SOURCE AT NEW YORK QUARRY, WALKING IN THE WOODS, GOT SOMETHING IN HIS RIGHT EYE. HE SAW A DOCTOR THE NEXT DAY AND WAS PRESCRIBED SOME EYE DROPS AND ALSO PAIN RELEIVER.

EMPLOYEE WAS MAKING A DOOR FOR 10 WIRE SAW, PIECE OF SAW DUST FLEW INTO RIGHT EYE. COLD SPRING GRANITE DID NOT KNOW OF INJURY UNTIL 5-2-02 WHEN WE RECEIVED FIRST REPORT.
