Showing all 20EE was changing broken rear step on a 980G loader, When EE was taking off the bolts that hold the step on with a impact wrench, the bracket spun while EE was holding the rubber mount and the Rusty Bracket cut EE's left hand.
While using torch to remove and replace shaker screen, a piece of debris hit the employee's hard hat and fell behind cutting PPE. Employee was taken for evaluation at medical facility and returned to work with no restrictions.
While welding a piece slag got in the cuff of the miners glove. This led to a small burn on the right arm. This later became infected.
While cutting legs free on a refeed hopper at the jaw, a piece of rust popped and went behind employee's PPE. Employee was seen by a physician and diagnosed with a right eye abrasion.
EE started having eye irritation on 10/11 at 0900 hours. It is believed that a shard of metal from grinding the previous day caused the irritation to the eye. The flake of metal was removed and EE given antibiotics for this injury. EE was returned to work with no restrictions. During the grinding process EE was in full and appropriate PPE.
Employee was grinding on a six inch piece of steel pipe. Employee got a piece of metal in right eye. Employee was wearing proper safety protection. The conditions were very windy.
The miner was welding hammers on the primary crusher. He took welding helmet off and using his hands brushed the dust off of himself. When he brushed the dust off the fan he was using to blow welding fumes out of crusher, blew a small piece of metal in his eye. The miner had on all of his safety equipment, including his safety glasses.
Employee was hard surface welding. When he lifted up his welding helmet, the surface he was welding began to pop and a piece of slag got in the corner of his right eye.
Injured was removing a cutting edge on a 988 loader. He was using a 1 impact to remove the bolt. The bolt dropped and spuning hand. Resulting in a cut in his left hand.
Employee was between the two big screens on the stone plant, welding an overhead nut. A piece of slag flew into his right ear and immediately began to burn and sizzle.
WHILE PRESSURE WASHING THAT AFTERNOON ON A WATER TRUCK, EE WAS WEARING EYE PROTECTIVE GOGGLES. AT THE END OF HIS SHIFT EE CAME INTO OFFICE TO CLEAN UP BY WASHING HIS HANDS & FACE TO LEAVE. HE NOTICED HIS EYE WAS IRRITATED AND FELT SOMETHING IN IT.
THE CHAIN SAW JUMPED OFF OF THE BAR. IT HIS A SMALL TREE LIMB WHICH SENT IT INTO HIS LEG CUTTING THROUGH HIS JEANS AND LEAVING A LACERATION THAT REQUIRED STITCHES.
HE WAS KNELT DOWN GRINDING METAL, WHILE GRINDING HE HIT A BURR ON THE METAL AND THE GRINDER DEFLECTED OFF AND HIT HIS LEFT KNEE.
EE WAS ABOVE THE PRIMARY CRUSHER, USING A CUTTING TORCH TO CUT OFF A RUSTY BOLT. A PIECE OF SLAG FLEW INTO HIS RT. EAR. HE COULDN'T HEAR OUT OF THAT EAR THE NEXT MORNING, BUT WAITED TO SEE IF IT GOT BETTER. HE WENT TO FAMILY DR.ON 5/13/04. THEY FOUND THE METAL ON HIS EAR DRUM, & REPORTED THAT IT WAS INFECTED. PRESCRIBED ANTIBIOTICS & REFERRED HIM TO SPECIALIST TO REMOVE METAL.
HE WAS CUTTING AND GRINDING METAL FOR HOPPER. A PIECE OF MATERIAL GOT BEHIND HIS SAFETY GLASSES.
EE WAS CUTTING A PIECE OF METAL WITH CUTTING TORCH. HE FINISHED HIS CUT AND TURNED TO SHUT HIS TORCH OFF, WHEN A PIECE OF SLAG POPPED OFF THE COOLING METAL AND FLIPPED INTO HIS RIGHT EYE BEHIND HIS SAFETY GLASSES.
EE GRINDING OLD WELD OFF FEEDER WHEEL. LATE IN SHIFT EE NOTICED SOMETHING IN HIS EYE. HE THOUGHT IT WAS DUST. LATER IN EVEING AT HOME, HIS EYE BOTHERED HIM MORE AND HE WENT TO THE ER. DR. REMO VED A FOREIGN OBJECT FROM LEFT EYE. FOLLOWUP WITH OPHTHALMOLOGIST.
PERSON WAS USING A CUTTING TORCH TO CUT A COLLECTION/DISTRIBUTION BOX OFF OF A BEAM. THE BEAM WAS NOT SECURE, AND FELL ON EE'S FOOT AFTER BOX WAS CUT OFF.
EMPLOYEE WAS GRINDING A LIFTING EYE OFF SIDE PANEL OF 980F CATLOADER BUCKET WITH A 4" GRINDER. AFOREIGN OBJECT LODGED IN HIS RIGHT EYE. HE WAS WEARING HIS SAFETY GLASSES.
