Employee was descending a set of stairs on a 980G. As they were transitioning to ground level they experienced pain in their lower back.

Fall onto or against objects

Employee was working from truck bed to remove ramp wall off of portable plant. When removing bolt put in for transport, the wall began to fall towards them. As wall was falling employee fell in the bed and injured left ankle. Wall came to rest on truck bed walls.

Employee was performing a pre operational inspection of a piece of equipment and slipped while in the engine compartment. This led to the employee spraining their right knee.

Employee states that over the past 2 years of operating a loader they have developed a cyst on their left thumb from repetitive motion. Employee will have surgery to remove the cyst.

Employee told manager that they were having bilateral wrist pain from operator the joysticks in the loader. Employee was treated on and off for 20 months before receiving Reportable level treatment.

Fall from machine

Employee was tempting to put antifreeze in Cat740. Employee set container on step, then climbed up the steps, once up on the platform, lost balance forcing employee to jump off the platform. Employee landed on feet but fell/rolled onto left shoulder on the ground. Employee will Have to have surgery on left shoulder.

The Mechanic helper was working on a piece of equipment in a tight spot. He had his leg folded under his thigh for a period of time and his leg went numb. He went home and the feeling never returned, so he went to the hospital. Doctor said it was a pinched nerve and may take a few days for feeling to return 100%.

Employee was driving water truck with window down, particles blowing around in cab got in his left eye.

Employee was working with hand tools disassembling commercial crusher. Employee while using a pry bar, pulled/ strained back muscle.

Employee picking mud from conveyor line, S & G plant left shoulder became sore. Reported to supervisor but no medical was immediately sought. On 08/20/2012 the employee requested to see doctor, shoulder was sore. First doctor's appointment 08/22/2012. Shoulder previously injured. There has been no lost time and employee continues regular job. Surgery is being considered.

EE WAS WORKING NIGHTS AT PLANT AND NOTICED WHILE WORKING THAT HIS LEFT THUMB WAS HURTING. HE CONTINUED TO WORK WENT HOME TO BED AND WHEN HE AWOKE HE HAD SWELLING ON LEFT WRIST. THE INFORMATION I GOT FROM EE WAS THE DR THINKS HE MAY HAVE SOME SORT OF BONE DISEASE THAT MAY BE CAUSING A DETENARATION OF BONE.
