While painting 992 loader bucket, the step ladder that was leaning against bucket slipped back and fell flat to the floor. This caused the employee to fall and while trying to catch self EE jammed EE's left shoulder.

EE was up on the plant grizzly welding structure together. After running out of welding rod, stood up & proceeded across beams. When ee reached the big beam, foot slipped causing ee to lose balance, fall into straddled sitting position. This caused right leg to go one side of the beam & left twisted. Somehow striking knee on way down to other side.

RC60forklift lifting 1/4"" steel plate 5'x13'with grab claw & 1/4"" chain. C-clamp used to keep chain from slipping. Plate needed flipped end for end. When turning C-clamp came loose causing it to fall. Another ee clear, but ee pushed forcefully to ground in awkward manner. Plate rested partially across him. Another ee able to remove & get help.

EE was using a 3""x3""x1/4"" angle at 6"" long as a pry bar to lift an 8' H beam at 30' long. His back popped causing him to seek medical attention right away. Doctor subscribed 3 medications for spasms and wants to see him again in two days.

When grabbing a hold of the grissly's rail the metal had a burr on it and when he moved his hand it sliced open his finger. He received four stitches and restriction until he returns to the doctor on Monday, 8/29/11.

While working on the bushhog mower gear box, EE was holding the shaft while another was driving it out with a hammer. When shaft moved injured's small finger was smashed between the shaft and the floor.

While working on a small pull-behind trailer, employee was attempting to raise the front of the trailer up. While he was cranking the jack handle around, the handle broke causing him to move forward and slash the underside of his right arm on the crank shaft that was left sticking up. He received sixteen stitches.

While carrying a shaft part & climbing up stairs, the employee caught his toe on the first step. This caused him to fall forward & smash his right ring finger between the step & shaft, blowing the end out of his finger. EE needed 8 stitches.
While exiting the barn area, employee started walking down a slight incline. He experienced a sharp pain behind his left knee in the bend of his leg. When he wasn't any better by the following Monday morning, he sought medical attention.
A 55 gallon drum of oil was setting in the back of a pick-up truck. Employee was moving the barrel around to strap it down and sprained the muscle in his right forearm/wrist. He chose not to seek medical attention until Oct. 14, 2006. They put a splint on his wrist and gave him a medication for inflamation (no restrictions).
WHILE PREPARING TO DRILL A PIECE OF FLAT STOCK STEEL STRAPPING, EMPLOYEE PLACED STEEL ON A TABLE. THE STEEL SHIFTED & FELL ON THE TOP OF EE'S LEFT FOOT, WHICH CAUSED BRUISING ON THE TOP.

WHILE PRESSURE WASHING THAT AFTERNOON ON A WATER TRUCK, EE WAS WEARING EYE PROTECTIVE GOGGLES. AT THE END OF HIS SHIFT EE CAME INTO OFFICE TO CLEAN UP BY WASHING HIS HANDS & FACE TO LEAVE. HE NOTICED HIS EYE WAS IRRITATED AND FELT SOMETHING IN IT.

EE, OTHER, AND ANOTHER WERE STANDING ON THE CATWALKS OF THE FIRST TWO CONVEYOR THAT WERE LOADED. AS THE CONVEYORS FELL THE EE FELL WITH THEM, STRIKING EE AND OTHER IN THE CHEST AREA AND BRUSING ANOTHER ARM.
WHILE LOADING 3 (50') NEW CONVEYORS ONTO A SEMI TRAILER, THE 3RD CONVEYOR SWAYED AND BUMPED THE TWO LOADED CONVEYORS CAUSING THEM TO SLIDE OFF THE TRAILER. 3 EE'S WERE STANDING ON THE CATWALK S OF THE FIRST 2 CONVEYORS THAT WERE LOADED. AS THE CONVEYORS FELL THE EE'S FELL WITH THEM, STRIKING INJURED IN THE CHEST AREA.

WHILE OPERATING HIS HAUL UNIT A BEE GOT IN HIS POP CAN. WHEN HE TOOK A DRINK HE WAS STUNG ON HIS TONGUE SEVERAL TIMES. THE ALLERGIC REACTION CAUSED SWELLING AND HE WAS DRIVEN TO THE HOSPITAL FOR AN OVERNIGHT STAY IN I.C.U.
WHILE CUTTING CONVEYOR BELTING IWTH A POCKET KNIFE, EE CUT TOWARDS HIMSELF AND STABBED HIMSELF INT HE LEG JUST ABOVE HIS KNEE, WITH THE BLAD GOING DOWN TOWARDS THE BACK OF HIS KNEE CAP.
