Struck by falling object

While using torch to remove and replace shaker screen, a piece of debris hit the employee's hard hat and fell behind cutting PPE. Employee was taken for evaluation at medical facility and returned to work with no restrictions.

Employee was scraping drive and hit frozen material. Employee was thrown forward and hit steering wheel. Employee had lap belt on during time of accident. Employee did not report injury to IMI till 12/10/2018.

While adjusting the door on secondary crusher, the door fell open and crushed the miner's thumb. Two people were trying to adjust the door when it fell. The door fell open smashing the thumb against a conveyor structure.

EE was working on East Pump and using right hand to put test cover on incoming power, when power flashed back and burned his right hand.

Struck against a moving object

Workers were gathering materials for a job & moving from bin #2 to the tower. One was driving a flat bed service truck & 1 was in the passenger seat. Injured was seated on the rear of the flat bed. As the truck moved thru the yard it hit a bump & ee bounce up and back down on the bed of the truck causing pain in his buttocks, hip, tailbone, & broke the skin behind his right knee.

EE was helping other ee change screens all the plant. He lost his footing causing him to slip and hit bump into a motor mount with his back.

EE IS A MECHANIC AND WAS WORKING ON A TAILGATE OF A HAULING UNIT AT STONEY CREEK. HE WAS WORKING UNDER IT AS WELL AS ALL AROUND. HE WAS WEARING SAFETY GLASSES. AT SOME POINT METAL FLEW BEHIND THE GLASSES INTO HIS LEFT EYE CAUSING IRRITATION. IT GREW MORE IRRITATED AS TIME WENT ON.

During morning clean-up, ee entered a feed box to clean out material build up. As he kicked material, it broke free causing him to twist his back and fall. As he fell he stradled the inner wall.

EE WAS IN THE SHOP. AN EXTENSION CORD WAS HANGING ON A HOOK ATTACHED TO A SHELF. THE CORD WAS NOT IN USE. ONE LOOP OF THE CORD HAD FALLEN TO THE FLOOR. EE REACHED FOR A SCRAPER & STEPPED INSID E THE LOOP OF THE EXTENSION CORD WITH HIS RIGHT FOOT. HE BECAME ENTANGLED, LOST HIS BALANCE & FELL BACK, REACHING BEHIND W/HIS RIGHT HAND TO CATCH HIMSELF, HE FELL & BROKE HIS RIGHT WRIST.

EE WAS CUTTING A PIECE OF METAL WITH CUTTING TORCH. HE FINISHED HIS CUT AND TURNED TO SHUT HIS TORCH OFF, WHEN A PIECE OF SLAG POPPED OFF THE COOLING METAL AND FLIPPED INTO HIS RIGHT EYE BEHIND HIS SAFETY GLASSES.

THE GROUND WAS FROZEN AND SNOW COVERED. EE WAS HELPING REMOVE CONCRETE BLANKETS FROM CONCRETE - GETTING READY TO REMOVE CONCRETE FORMS. HE STEPPED INTO A FROZEN RUT AND TWISTED HIS LEFT ANKLE.

EE WAS BACKING HAULING UNIT UP TODUMP MATERIAL OVER EDGE OF STOCKPILE. REAR TIRES WENT OVER EDGECAUSING HAULING UNIT TO FALL TO THE RIGHT SIDE AND ROLL DOWN STOCKPILE AND LAND ON ITS WHEELS. MR.BALL RECEIVED LACERATION TO HEAD AND CONCUSSION TO HEAD AND BRUISES TO BODY.

EE WAS CLIMBING DOWN THE LADDER OF A FRONT END LOADER. THE LADDER WAS WET FROM RAIN. HIS FOOT SLIPPED ON A WET LADDER RUNG AND HE TWISTED HIS RIGHT KNEE TRYING TO CATCH HIMSELF FROM FALLING.

EE CLIMBED UP ON THE CRUSHER TO REACH AND CLEAN A CAMERA LENSE. AS HE WAS DESENDING, HE STEPPED ON TO THE MOTOR AND JUMPED DOWN ON TO THE DECK, TWISTING HIS LEFT KNEE.

8-8-01-OVER 90 DEGREE HEAT. EE WAS UP ON TOP OF PLANT GREASING. HE BECAME ILL-VOMITING-=WAS NOT SWEATING-CLIMBED DOWN OFF PLANT AND CONTACTED HIS SUPERVISOR. EE WAS TAKEN TO HOSPITAL AND TREAT ED FOR HEAT EXHAUSTION-THEN RELEASED AFTER 1500 THE SAME DAY.

EMPLOYEE WAS WELDING ON A SCREEN USING SCALER TO CLEAN SLAG AND WELD. HE HAD SAFETY GLASSES ON AND WAS PREPARING WELD AREA. A PIECE OF MATERIAL WAS BLOWN IN BEHIND HIS GLASSES BY THE WIND. HE WENT TO EYE AREA, BRUSHED IT AWAY, THOUGHT IT WAS GONE. THE NEXT EVENING HIS EYE STARTED TO BECOME IRRITATED. WENT TO DOCTOR AND PRESCRIPTION WAS GIVEN.

JACKING ANGLE IN PLACE USING AN EXTENTION POST. PUT PRESSURE ON JACK AND JACK KREELED OUT STRIKING IN HEAD CUTTING HIS EYEBROW AND OPENING UP A WOUND THAT REQUIRED STITCHES.
